Quality of life following femoral head-neck osteochondroplasty for femoroacetabular impingement.

BACKGROUND: Femoroacetabular impingement has been recently described as a common cause of hip pain and labral tears in young adults. We eval … eonecrosis. Nine patients underwent screw removal from the greater trochanter because of persistent bursitis. Six of the thirty-four patients were dissatisfied with the outcome. CONCLUSIONS: Cam-type femoroacetabular impingement is associated with insufficient concavity at the anterolateral head-neck junction and with pathological changes in the labrum. O …

Comparison of therapeutic efficacy between arthroscopic capsular release and transarterial musculoskeletal embolization in patients with adhesive capsulitis of the shoulder.

BACKGROUND: Adhesive capsulitis is a debilitating shoulder disorder characterized by pain and restricted motion. While arthroscopic capsular release is a well-established surgical treatment, transarterial musculoskeletal embolization (TAME) has recently emerged as a minimally invasive alternative. The aim of this study is to compare the short-term clinical outcomes of arthroscopic capsular release and TAME in patients with adhesive capsulitis of the shoulder. MATERIAL AND METHODS: We retrospectively analyzed 40 patients with refractory adhesive capsulitis who underwent either arthroscopic cap

AAOS 2024 best paper in the shoulder and elbow classification: watchful waiting provides higher value with similar functional outcomes to physical therapy for frozen shoulder: a prospective randomized controlled trial.

BACKGROUND: Physical therapy remains a ubiquitous treatment modality for the management of frozen shoulder (adhesive capsulitis) despite limited high-level evidence supporting its use. Leveraging patient-reported outcome measures (PROMs) and healthcare costs, this study compared patient outcomes and value achieved through watchful waiting (WW) vs. physical therapy (PT) for conservative management of frozen shoulder. METHODS: Patients in this prospective, parallel randomized controlled trial who were diagnosed with frozen shoulder were randomized 1:1 to WW or PT. All patients were offered ther

Ultrasound-guided coracohumeral ligament release.

Long-term follow-up of patients with adhesive capsulitis (AC) reveals that approximately half of them suffer from a limited range of shoulder motion, particularly external and/or internal rotation. We report the surgical technique and short-term clinical outcomes of ultrasound-guided release of the thickened coracohumeral (CH) ligament in 8 patients (9 shoulders) with AC. Passive external rotation with the arm by the side significantly increased from an average of 18° preoperatively to 47° immediately after CH ligament release. VAS and ASES scores were improved at 3months follow-up in all 9 sh

Endoscopic repair of tears of the superficial layer of the distal triceps tendon.

PURPOSE: The purpose of this study was to evaluate the results after endoscopic repair of partial superficial layer triceps tendon tears. METHODS: Fourteen patients treated surgically between July 2005 and December 2012 were studied prospectively for 12 months. Indication for surgery was a partial detachment of the triceps tendon from the olecranon that was proved by magnetic resonance imaging (MRI) in all cases. Ten of these patients had chronic olecranon bursitis. All patients were treated with endoscopic surgery including bursectomy and repair of the distal triceps tendon with double-loade…

Physical tests for shoulder impingements and local lesions of bursa, tendon or labrum that may accompany impingement.

BACKGROUND: Impingement is a common cause of shoulder pain. Impingement mechanisms may occur subacromially (under the coraco-acromial arch) or internally (within the shoulder joint), and a number of secondary pathologies may be associated. These include subacromial-subdeltoid bursitis (inflammation of the subacromial portion of the bursa, the subdeltoid portion, or both), tendinopathy or tears affecting the rotator cuff or the long head of biceps tendon, and glenoid labral damage. Accurate diagnosis based on physical tests would facilitate early optimisation of the clinical management approach…

Genetic associations between frozen shoulder and tennis elbow: a female twin study.

OBJECTIVES: To estimate the heritability of frozen shoulder (FS) and tennis elbow (TE) and to examine the two disorders for possible genetic or environmental associations. METHODS: Self-reporting questionnaire data on risk factors for, and a physician diagnosis of, FS or TE were obtained from 865 monozygotic (MZ) and 963 dizygotic (DZ) unselected female twin pairs aged between 20 and 76 yr registered with the St Thomas' UK Adult Twin Registry. The heritability of each disorder was estimated in a classic twin study. The association between FS and TE was then explored by log-linear modelling co

Inflammation of the subacromial bursa in chronic shoulder pain.

Subacromial bursal tissue was studied in 12 patients operated on for painful (10 patients with constant pain and 2 patients with pain on motion) rotator cuff tendinitis/impingement syndrome. The Neer acromioplasty technique was used. Six patients had moderate inflammatory changes and one had a slight inflammation. In three of the five remaining patients, the subacromial bursa did not show any signs of inflammatory involvement, but patients experienced pain at rest and at night, reflecting clinical inflammation in tissues other than the bursa. The two patients with pain only on strain did not s

Computed tomography of the knee joint: clinical results.

Computed tomography (CT) was used to examine the knee joint in 175 patients with various pathological conditions. In 118 of these patients a unique technique of study was employed. This technique is based on thin slices, a high resolution reconstruction program, and routine multiplanar reconstructions, without intraarticular contrast medium administration. In 36 patients studied with this technique, surgery was performed, thus permitting verification of the CT patterns. We concluded that CT may identify lesions of menisci, cruciate and collateral ligaments, and other pathologic conditions with

Hypertension, Dyslipidemia, and Adhesive Capsulitis: A Bidirectional Two-Sample Mendelian Randomization Study of the European Population.

BACKGROUND: Whether hypertension and dyslipidemia are risk factors for adhesive capsulitis (AC) remains controversial. Many observational studies have reported conflicting results. However, observational studies are susceptible to confounding and reverse causation, limiting the ability to establish causality. Therefore, a robust method is needed to clarify these relationships. METHODS: We first conducted a hospital-based case-control study that included 200 AC patients and 200 controls. Multivariate logistic regression was used to examine the associations of hypertension and dyslipidemia with

Effect of adding median nerve mobilization to glenohumeral joint mobilization in patients with adhesive Capsulitis. Randomized controlled trial.

BACKGROUND: Little is known about neural mobilization techniques (NMTs) for addressing mobility restriction in patients with adhesive capsulitis (AC). This randomized, single blinded controlled trial was designed to determine the effect of adding median nerve NMTs to the glenohumeral (GH) joint mobilization in patients with AC. METHODS: Fifty-six patients with AC, aged 40 to 70 years, were included and randomly assigned into two groups. The study group (A) received median nerve NMTs and GH joint mobilization while the control group (B) received only GH joint mobilization. Both groups received

Efficacy of Pericapsular Platelet-Rich Plasma Injection Combined With Brachial Plexus Block-Assisted Manipulation in Adhesive Capsulitis: A Randomized Controlled Trial.

Adhesive capsulitis causes severe shoulder pain and functional limitation, and single treatment modalities often fail to prevent relapse after manipulation under anesthesia. In this randomized controlled trial, we examined whether adding ultrasound-guided multitarget platelet-rich plasma injections to manipulation under brachial plexus block could improve pain, function, and health-related quality of life. We enrolled 68 adults with adhesive capsulitis confirmed by magnetic resonance imaging and randomly assigned them to manipulation plus standardized rehabilitation or to the same regimen plus

Mid-term outcomes after corticosteroid injection for adhesive capsulitis in patients with type-2 diabetes: a retrospective cohort study.

AIMS: The mid-term clinical course after intra-articular corticosteroid injection for adhesive capsulitis ("frozen shoulder") in patients with type-2 diabetes mellitus (T2DM) remains unclear. Therefore, we evaluated coded shoulder-related outcomes and selected coded complications after such injections in patients with T2DM. METHODS: This retrospective TriNetX Research Network study identified patients with T2DM and first-time adhesive capsulitis (2004-2024). Patients receiving a coded intra-articular corticosteroid injection within 6 months after diagnosis were compared with patients without

A Double Blind Randomised Controlled Trial Comparing Glenohumeral Cortisone With and Without Hydrodilatation in Patients With Frozen Shoulder.

BACKGROUND: Frozen shoulder (FS) is a common condition causing pain and stiffness in the shoulder. There is controversy over whether hydrodilatation (HD) gives improved outcomes compared to corticosteroid injection (CSI) alone. The aim of this study was to investigate if HD improves outcomes compared to CSI alone. METHODS: This was a double-blinded RCT. Patients aged 18-90 with FS were included. The CSI group received an injection of contrast followed by local anaesthetic and steroid. The HD group received the same, followed by saline until capsular rupture or severe resistance. The primary o

Releasing Forces in Adhesive Capsulitis Are Important Indicators of Shoulder Stiffness and Postoperative Function.

BACKGROUND: Manipulation under anesthesia is a widely used treatment for frozen shoulder, but the factors that influence patient outcomes after manipulation remain unclear. The degree of shoulder stiffness, a critical feature of frozen shoulder, likely reflects the severity of the condition but currently lacks standardized, objective assessment methods. QUESTIONS/PURPOSES: (1) What are the releasing forces in patients with frozen shoulder, and do the forces vary across different stages of frozen shoulder? (2) Are there differences in postoperative outcomes of manipulation under anesthesia amo
