Chronic actinic dermatitis: a familial link?

Chronic actinic dermatitis (CAD) is an inflammatory, immune-mediated photodermatosis characterized by erythematous, pruritic, lichenified plaques affecting photo-exposed areas of the body. The pathogenesis of CAD remains unknown, with many postulated theories in the literature. The predominant hypothesis suggests a delayed type IV hypersensitivity reaction caused by photosensitive skin antigens, similar in disease process to allergic contact dermatitis. Despite current evidence suggesting no familial inheritance, we present, to our knowledge, the first reported cases of CAD affecting three mem…

Incontinence-associated dermatitis: Not just babies.

Irritant diaper dermatitis (IDD) is frequently observed in young children but can affect individuals throughout their lifespan. The broader term incontinence-associated dermatitis (IAD) is used to describe inflammatory skin reactions from prolonged exposure to urine and/or feces. This review compares IAD and IDD in pediatric and geriatric populations, focusing on their pathogenesis, clinical considerations, and management. Both conditions involve prolonged bodily fluid exposure disrupting the skin barrier, making infants and older adults particularly susceptible due to their age-related skin c…

Seborrheic Dermatitis: From Adolescence to Adulthood.

Seborrheic dermatitis (SD) is a skin condition that often begins in adolescence, with an estimated prevalence of approximately 10%. The severity of SD varies widely, from mild dandruff to severe erythema, scaling, and flaking, which can be distressing and can impact an adolescent's quality of life. In individuals with darker skin tones, hypopigmentation may be observed. While the exact cause of SD remains unclear, a combination of environmental, medical, and lifestyle factors are believed to contribute to its development. Some researchers identify Malassezia yeast as central to the pathogenesi…

Allergic Contact Dermatitis in Pediatric Practice.

BACKGROUND: Allergic contact dermatitis (ACD) is prevalent among pediatric population, adolescent and young adults. Patients with ACD experience a lot of sociopsychological and qualityof- life (QoL) difficulties. Children and their caregivers alike are vulnerable to the burden of ACD. OBJECTIVES: We have, in this paper, provided an overview of ACD and discussed common and unusual causes of ACD. METHODS: We performed an up-to-date literature review in the English language on "allergic contact dermatitis" via PubMed Clinical Queries, using the keywords "allergic contact dermatitis" in August 2…

Contact Allergy Screening for Atopic Dermatitis.

Atopic dermatitis (AD) and allergic contact dermatitis (ACD) are common inflammatory skin diseases in both children and adults that present similarly and often coexist. Patch testing is the gold standard for establishing the diagnosis of ACD and can often help distinct between the 2 conditions. Patch testing is more challenging in patients with underlying AD due to potential for angry back reactions. In this review, we discuss the current evidence and guidelines regarding the screening for contact allergies in patients with AD. We also discuss the most frequent relevant allergens in adults and…

Evolution of radiation-induced dermatitis treatment.

Radiation-induced skin damage (RID) is the most prevalent, significant side effect of radiotherapy (RT). Nearly 95% of patients experience moderate to severe skin reactions after receiving radiation therapy. However, criteria for acute radiation dermatitis (ARD) treatment remain unavailable. Topical agents with anti-inflammatory properties may protect the skin and facilitate tissue regeneration in patients with RID. Many of these topical agents function through nuclear factor kappa B pathway regulation. They either reduce the levels of inflammatory factors or elicit anti-inflammatory propertie…

Toll-Like Receptors and Contact Dermatitis.

Contact dermatitis (CD) is a common cutaneous inflammatory condition that affects millions of people worldwide. Xenobiotic agents are frequently encountered in substances used in everyday life, making it difficult to avoid personal and occupational exposure. Toll-like receptors (TLRs) are transmembrane receptors that modulate the innate immune system in response to tissue injury or infection. TLRs play a key role in the pathophysiology of contact dermatitis. TLR signaling is involved in three major forms of CD: protein CD, allergic contact dermatitis (ACD), and irritant CD. Of the 10 TLRs foun…

Comprehensive Ophthalmological Evaluation in Atopic Dermatitis.

INTRODUCTION: Atopic dermatitis (AD), a chronic type 2 inflammatory skin disease, is frequently associated with ocular surface diseases (OSD) which may appear or worsen under anti-type 2-targeted treatments. However, the exact prevalence of OSD and the ophthalmology referral criteria remain ill-defined in AD patients before initiating such biotherapies. We aimed to characterize the prevalence, the nature and the factors related to OSD development in AD that may justify an ophthalmological management. METHODS: A total of 98 consecutive AD inpatients without biological treatment were retrospect…

Tacrolimus Ointment in Periorbital Atopic Dermatitis.

Periorbital atopic dermatitis (AD) is a common sign in ophthalmological practice and usually has a persistent and relapsing course. Treatment with topical corticosteroids has various side effects associated with their usage. Tacrolimus topical ointment has unique immunomodulatory properties that decrease skin inflammation and pruritus in AD. In this case series, we present a prospective case series of five patients (three males and two females) who received topical application of tacrolimus ointment 0.1-0.03% in the periorbital area twice daily for one to four weeks. The pre- and post-treatmen…

Hypereosinophilic Dermatitis: Successful Treatment with Dupilumab.

Hypereosinophilic dermatitis (HED) is a subtype of hypereosinophilic syndrome. HED is characterized by eosinophilic granulocytes increased in peripheral blood and bone marrow and infiltrated in skin. The clinical manifestations of HED are diffussed by erythema, papule and maculopapule with severe itching. The etiology of HED is unknown. At present, in addition to HED with FIP1L1-PDGFRA fusion gene positive, whose treatment is tyrosine kinase inhibitor, other types of HED first-line treatment are oral glucocorticoids, supplemented by antihistamines and immunosuppressants. Dupilumab is a human m…

Dermatitis Artefacta: A Challenging Case Report.

We report a 31-year-old female presented with a history of recurrent skin and oral lesions for 10 years. She brought a histopathology report confirming the diagnosis of pemphigus vulgaris (PV), which was found to be faked with no patient information and lacked letterhead. Skin and oral examination only reveal multiple linear upper lip erosions. We believed the patient had a preliminary diagnosis of PV, and we asked the patient to continue her medications. Based on the conflicting history and occurrence of contradictory issues, a diagnosis of dermatitis artefacta was made. The patient improved …

Atopic dermatitis across the life course.

Atopic dermatitis, the most common chronic inflammatory skin disease, can occur at any age, and patterns of disease activity vary over time. Both prevalence and incidence are highest in infancy and early childhood, followed by a second peak in older adulthood. Birth cohort studies from European countries following children through adolescence have identified subgroups of patients with early-onset persistent disease, early-onset resolving disease, and later-onset disease. Parental history of atopy and genetic factors are among the most consistent predictors of more persistent disease. Studies h…

Blister Beetle Dermatitis Outbreaks in Mali.

INTRODUCTION: Meloidae are distributed in temperate and arid regions but are also common in subtropical and tropical savannahs. These insects contain cantharidin, a vesicant substance that can cause poisoning by ingestion and dermatitis by direct contact. MATERIAL AND METHODS: We describe recurrent Meloidae-related dermatitis outbreaks and their health impact by analyzing medical consultation records and meteorological data. RESULTS: Between 2015 and 2019, dermatitis outbreaks took place at a French military base at the end of the rainy season, from July to August, with 100 cases reported in…
