Acromegaly in humans and cats: Pathophysiological, clinical and management resemblances and differences.

OBJECTIVE: Acromegaly is a disorder associated with excessive levels of growth hormone (GH) and insulin-like growth factor-1 (IGF-1). In general, GH/IGF-1 excess leads to morphologic craniofacial and acral changes as well as cardiometabolic complications, but the phenotypic changes and clinical presentation of acromegaly differ across species. Here, we review the pathophysiology, clinical presentation and management of acromegaly in humans and cats, and we provide a systematic comparison between this disease across these different species. DESIGN: A comprehensive literature review of pathophy

Chronic intestinal pseudo-obstruction in a child with Treacher Collins syndrome.

BACKGROUND: Treacher Collins syndrome (TCS) mainly presents with severe craniofacial developmental abnormalities characterized by a combination of bilateral downward-slanting palpebral fissures, colobomas of the lower eyelids, hypoplasia of the facial bones, cleft palate, malformation of the external ears, atresia of the external auditory canals, and bilateral conductive hearing loss. It is due to mutations in Treacher Collins syndrome 1 (TCOF1) (5q32-q33.1) and Polymerase RNA 1 polypeptides D and C (POLR1D [13q12.2], and POLR1C [6p21.1]) genes, which are responsible for increased neuroepithel

A novel MGP mutation in a consanguineous family: review of the clinical and molecular characteristics of Keutel syndrome.

Keutel syndrome (KS) [OMIM 245150] is a rare autosomal recessive condition, characterized by abnormal cartilage calcification. Mutations in the matrix Gla protein gene (MGP) have been previously reported in three unrelated KS families. MGP is an extracellular matrix protein that acts as a calcification inhibitor by repressing bone morphogenetic protein 2 (BMP2). Loss-of-function mutations of MGP result in abnormal calcification of the soft tissues, a cardinal feature of KS. We report the fourth MGP mutation (IVS2 + 1G > A) in a consanguineous Arab family, which results in the loss of the conse

The Dlx5 and Dlx6 homeobox genes are essential for craniofacial, axial, and appendicular skeletal development.

Dlx homeobox genes are mammalian homologs of the Drosophila Distal-less (Dll) gene. The Dlx/Dll gene family is of ancient origin and appears to play a role in appendage development in essentially all species in which it has been identified. In Drosophila, Dll is expressed in the distal portion of the developing appendages and is critical for the development of distal structures. In addition, human Dlx5 and Dlx6 homeobox genes have been identified as possible candidate genes for the autosomal dominant form of the split-hand/split-foot malformation (SHFM), a heterogeneous limb disorder character

An Alternative Posterosuperior Auricular Fascia Flap for Ear Elevation During Microtia Reconstruction.

BACKGROUND: Advances in staged total auricular reconstruction have resulted in improved anterior auricular appearance; however, satisfactory postreconstruction esthetics of the retroauricular fold remain challenging. The postauricular appearance of the reconstructed ear depends largely upon optimizing the covering material. When used as the covering soft tissue for ear elevation, a flap containing primarily the upper portion of the retroauricular fascia has potential advantages over the conventional book cover-type retroauricular fascia flap. METHODS: We developed a geometrically designed, po

Patient Satisfaction and Its Influencing Factors of Microtia Reconstruction Using Autologous Cartilage.

BACKGROUND: This study aimed to investigate patient perceptions of and satisfaction with surgical outcomes of microtia reconstruction using autologous cartilage. It also analyzes associations between patient background characteristics and their evaluation. METHODS: Lobule-type microtia patients who had received first-stage surgery were interviewed using a questionnaire, which included assessment of nine auricle substructures, superior and inferior parts and overall impression. Answers were recorded on a five-point Likert-style scale. Satisfaction rate and average scores were used to compare s

[VEXAS syndrome mimicking relapsing polychondritis: A case report].

This article reports the diagnosis and therapeutic management of a 53-year-old male with VEXAS syndrome mimicking relapsing polychondritis. The patient presented with multiple subcutaneous nodules and auricular/nasal chondritis. Blood routine examination revealed leukopenia, moderate macrocytic anemia and thrombocytopenia. Inflammatory markers were elevated, including erythrocyte sedimentation rate, C-reactive protein (CRP) and interleukin-6. Serological tests were negative for antinuclear antibody (ANA), anti-extractable nuclear antigen antibody spectrum (ENA), and anti-neutrophil cytoplasmic

Management of tympanosclerosis with ossicular fixation: review and presentation of long-term results of 30 new cases.

OBJECTIVE: Surgery for tympanosclerosis has always been challenged with the risk of deterioration of hearing or the recurrence of the disease. If surgery is indicated, controversy exists regarding staging versus nonstaging and whether to perform stapedectomy or mobilize the fixed ossicles by removal of plaques. The aim of this study was to review the previous reports and to analyze the long-term surgical outcome of 30 patients who have been operated on owing to tympanosclerosis. METHODS: Nine hundred twelve cases with chronic otitis media with or without cholesteatoma that have undergone eith

A Rare Case of Cutaneous Leiomyosarcoma Arising From the External Auditory Canal.

Leiomyosarcoma (LMS) is a malignant tumor that originates from smooth muscle. Cutaneous LMS is a distinct, superficial sarcoma that arises from dermal smooth muscle. It generally has a more favorable prognosis than subcutaneous or deep soft tissue LMS. Head and neck LMS accounts for a small proportion of all LMS cases, and primary tumors in the external auditory canal are extremely rare. We report a case of cutaneous LMS arising from the external auditory canal. The patient was a 32-year-old woman. She presented with a two-month history of a left external auditory canal mass, hearing loss, and

Reconstruction of lower half defects of the nose with the lateral nasal artery pedicle nasolabial island flap.

BACKGROUND: The superiorly based nasolabial flap is a well-known procedure. However, random pattern or subcutaneous pedicle flaps are insufficient for reconstruction of large defects. Artery pedicle flaps have a satisfactory blood supply, and for that reason, the required tissue can be raised for reconstruction of large defects of the nose. METHODS: Twelve cases were operated on with this technique. Reconstruction was performed with a lateral nasal artery pedicle island flap. The mean age of the patients was 68.4 years. Three patients required cartilage grafts for alar rim support. The cartil
