[COPD: new insights].

Both morbidity and mortality due to chronic obstructive pulmonary disease (COPD) are increasing. In the Netherlands there are between 500,000 and 1 million patients. It has now been recognized that airway obstruction can be partially reversed and that inflammation and oxidative stress play a part in the pathophysiology. New disease outcome measures in scientific research and in the clinical setting include symptoms, quality of life, exacerbation frequency and changes in exercise tolerance. Bronchus-dilating medication is now the standard pharmacotherapy and at every stage of COPD bronchodilato…

Late cardiac damage of anthracycline therapy for acute lymphoblastic leukemia in childhood.

Long-term leukemia survivors (46) underwent cardiac evaluation, including physical examination, ECG, exercise testing, and echocardiography. They were 2-17 years old at diagnosis and 5-23 years old after treatment. Thirty-four survivors received anthracyclines (AC) (mean 203 mg/m2), 12 of them had also alkylating agents (AA) and 12 had no AC. Exercise tolerance was bellow predicted values in 21 (48%) survivors and 21 survivors had ECG abnormalities, which were more frequent in those treated with AC. Concomitant AC with AA was correlated with prolonged isovolumic relaxation time (IVRT) and infl…

Interactions between sleep, other body rhythms, immune responses, and exercise.

Biological rhythms can influence both exercise tolerance and immune function. Most studies have focussed upon circadian rhythms, but some circaseptan, circatrigintan, and circannual cycles have also been described. Rhythms may have an endogenous or an exogenous basis. Endogenous rhythms originate in the cells of the suprachiasmatic nucleus. Impulses from this region pass to the pineal gland, where they regulate the production of melatonin, a compound that modulates the sleep/wakefulness cycle. Endocrine, physiological, and psychological parameters all show evidence of periodicity. Most compone…

[A surgical case report of an atrial septal defect combined with stenosis of the left main coronary artery caused by a dilated pulmonary artery in adult].

We encountered a 40-year-old male patient diagnosed to have an atrial septal defect combined with pulmonary hypertension and left main coronary artery stenosis of 50% caused by a dilated pulmonary artery. Preoperative Trazoline tolerance test revealed an increase in pulmonary artery pressure from 77/22(40) to 85/24(42), and a decrease in pulmonary vascular resistance from 4.2 U.m2 to 3.6 U.m2. Moreover, his treadmill exercise test was positive. We performed only a patch closure of the atrial septal defect. The postoperative catheterization examination showed normal pulmonary artery pressure, m

Cardiovascular evaluation after withdrawal from chronic alcohol or cocaine-alcohol abuse.

Alcohol ingestion commonly accompanies cocaine abuse, but the effects of chronic cocaine-alcohol abuse on the circulation are undefined. The … ffered in group III (decreased HR response, while SBP increased inappropriately). Despite excellent exercise tolerance, HR response to exercise in group III (compared to group I) was decreased. These results suggest impairment of certain autonomic nervous system reflexes and, possibly, sinus node dysfunction from cocaine-alcohol abuse.

Cardiopulmonary Fitness and Exercise-Based Cardiac Rehabilitation in Patients With Myocarditis.

BACKGROUND: Cardiorespiratory fitness (CRF) is a powerful predictor of cardiovascular morbidity and mortality. The cardiopulmonary exercise test (CPET) is considered a gold standard in assessing CRF. Patients with myocarditis are recommended to abstain from exercise because of the risk of sudden cardiac death (SCD). Therefore, the clinical benefit of exercise-based cardiac rehabilitation (CR) has not been previously established in this population. OBJECTIVE: The aim of this study is to assess the CRF of myocarditis and the safety and effectiveness of an exercise-based CR program in these popu

Cardiac diastolic maladaptation is associated with the severity of exercise intolerance in sickle cell anemia patients.

This pilot study focusing on Sickle Cell Anemia (SCA) patients offers a comprehensive and integrative evaluation of respiratory, cardiovascular, hemodynamic, and metabolic variables during exercise. Knowing that diastolic dysfunction is frequent in this population, we hypothesize that a lack of cardiac adaptation through exercise might lead to premature increase in blood lactate concentrations in SCA patients, a potential trigger for acute disease complication. SCA patients were prospectively included in PHYSIO-EXDRE study and underwent a comprehensive stress test with a standardized increment

Impact of an Exercise-Based Cardiac Rehabilitation Program on Cardiopulmonary Test Parameters.

INTRODUCTION: Exercise-based cardiac rehabilitation programs (CRPs) represent a multidisciplinary therapeutic approach tailored to improve cardiorespiratory fitness and overall health-related quality of life, while reducing cardiovascular hospitalizations and mortality. Our aim was to determine changes in exercise capacity and cardiac events according to the left ventricular ejection fraction (LVEF) following a CRP. METHODS: This single-center cohort study included 131 patients who completed a center-based CRP from 2015 to 2022. Cardiopulmonary exercise testing (CPET), transthoracic echocardi

Cardiac Rehabilitation in Advanced aGE after PCI for acute coronary syndromes: predictors of exercise capacity improvement in the CR-AGE ACS study.

BACKGROUND: The positive effect of cardiac rehabilitation (CR) on outcomes after acute coronary syndromes (ACS) is established. Nevertheless, enrollment rates into CR programs remain low, although ACS carry a high risk of functional decline particularly in the elderly. AIM: We aimed to determine if a multidisciplinary CR improves exercise capacity in an older population discharged after ACS systematically treated with PCI. METHODS: CR-AGE ACS is a prospective, single-center, cohort study. All patients aged 75+ years consecutively referred to Cardiac Rehabilitation outpatient Unit at Careggi

Responsiveness to exercise training in juvenile dermatomyositis: a twin case study.

BACKGROUND: Patients with juvenile dermatomyositis (JDM) often present strong exercise intolerance and muscle weakness. However, the role of exercise training in this disease has not been investigated. PURPOSE: this longitudinal case study reports on the effects of exercise training on a 7-year-old patient with JDM and on her unaffected monozygotic twin sister, who served as a control. METHODS: Both the patient who was diagnosed with JDM as well as her healthy twin underwent a 16-week exercise training program comprising aerobic and strengthening exercises. We assessed one repetition-maximum

Exercise for patients with congestive heart failure.

Congestive heart failure is a widely prevalent sequel to myocardial infarction and other chronic conditions (including ischaemia without infarction, hypertension, various infections, toxic metabolic and endocrine disorders). Exercise tolerance is severely limited; the cardiac ejection fraction is often less than 20% and the peak oxygen intake may be less than 10 ml/kg x min, with a resulting deterioration in the quality of life. Possible factors contributing to the poor tolerance of exercise include: (i) disturbances of myocardial function (damage to the ventricular wall; decreased inotropic r…
