Circular Fixation in Charcot.

Static circular fixation is a valuable tool for patients with Charcot foot and ankle deformities. The versatility of circular fixators allows for dynamic adjustments over time and can allow for off-loading of flaps or ulcers. The circular fixator can be used as a primary fixation device in cases of osteomyelitis or to facilitate lengthening when a segmental bone defect exists such as loss of the talus. As a secondary fixation device it can protect the internal fixation or be used when there is a compromised soft tissue envelope.

Arthrofibrosis of the knee following a fracture of the tibial plateau.

The aim of this study was to report the incidence of arthrofibrosis of the knee and identify risk factors for its development following a fracture of the tibial plateau. We carried out a retrospective review of 186 patients (114 male, 72 female) with a fracture of the tibial plateau who underwent open reduction and internal fixation. Their mean age was 46.4 years (19 to 83) and the mean follow-up was16.0 months (6 to 80). A total of 27 patients (14.5%) developed arthrofibrosis requiring a further intervention. Using multivariate regression analysis, the use of a provisional external fixator (o

Percutaneous opening wedge varus osteotomy with a multidrilling guide in Perthes' disease: a technical note on two cases.

We report a technique of percutaneous opening wedge intertrochanteric femoral osteotomy using a multidrilling guide with application of an Ilizarov external fixator in the management of Perthes' disease, and describe the early results obtained in two children. The average operative time was 96.5 min and the duration of external fixation was 51.5 days. Intraoperative blood loss was less than 20 ml. There were no significant complications. This percutaneous technique offers several advantages over currently available methods for surgical containment of Perthes' disease. These include a small inc

Reconstruction of gap non-union tibia with composite use of extramedullary fixation and bone transport by monorail fixator: a prospective case series.

INTRODUCTION: Non-union is a complex, multifactorial orthopaedic problem that requires multiple treatment modalities for its management. It can result from infection, segmental bone loss, soft-tissue loss, and associated host factors. External fixators as management of gap non-union are bulky and give rise to a number of complications In our study, we have described a technique where we have used extramedullary fixation in the form of a tibial locking plate and transportation of part of the tibial bone after corticotomy by a monorail fixator. MATERIALS AND METHODS: The procedure was done in t…

Limb lengthening history, evolution, complications and current concepts.

Limb lengthening continues to be a real challenge to both the patient and the orthopaedic surgeon. Although it is not a difficult operative problem, there is a long and exhausting postoperative commitment which can jeopardize early good results. I aim to review the history, evolution, biology, complications and current concepts of limb lengthening. Ilizarov's innovative procedure using distraction histeogenesis is the mainstay of all newly developing methods of treatment. The method of fixation is evolving rapidly from unilateral external fixator to ring fixator, computer assisted and finally

We here report a case of a 50-year-old male with ankle osteoarthritis and lower limb deformity, for which simultaneous deformity correction and arthrodiastasis were performed. The patient initially experienced an open fracture on the left tibia at 19 years, but it was malunited. The Japanese Society for Surgery score of the foot for the left ankle was 53 points. X-ray and CT imaging showed rotational and angular tibial deformities with shortening by 1.6cm and end-stage osteoarthritis of the left ankle. An external fixator was applied to correct the lower limb deformity, and ankle arthrodiastas

Deltoid Ligament Augmentation Replacing Syndesmotic Fixation for the Treatment of Ankle Fracture With Syndesmotic Instability and Deltoid Ligament Rupture: A Cadaveric Study.

OBJECTIVES: Ankle fracture with both deltoid ligament (DL) rupture and syndesmotic diastasis is often treated by syndesmotic fixation after fibular fixation. However, a second operation may be needed to remove the internal fixation, and screw breakage/misplacement may occur. The present study aimed to explore the mechanism and feasibility of DL augmentation instead of syndesmotic fixation from the perspective of biomechanics. METHODS: The CT data (in DICOM format) of a 33-year-old man were used to create a finite element model. External rotation stress and eversion stress were applied to the

'Rail and Nail' bifocal management of atrophic femoral nonunion.

AIMS: The management of a significant bony defect following excision of a diaphyseal atrophic femoral nonunion remains a challenge. We present the outcomes using a combined technique of acute femoral shortening, stabilized with a long retrograde intramedullary nail, accompanied by bifocal osteotomy compression and distraction osteogenesis with a temporary monolateral fixator. PATIENTS AND METHODS: Eight men and two women underwent the 'rail and nail' technique between 2008 and 2016. Proximal locking of the nail and removal of the external fixator was undertaken once the length of the femur ha

Lower limb lengthening over an intramedullary nail: a long-term follow-up study of 28 cases.

BACKGROUND: Limb lengthening using an external fixator requires a long period of external fixation and may be associated with several complications such as axial deformity, fracture of the regenerated bone, and joint stiffness. With the goal of reducing the time of external fixation as well as some of these complications, we performed femoral or tibial lengthening over an intramedullary nail, according to Paley's technique, in 28 patients, followed up after a mean period of 8 years. MATERIALS AND METHODS: Twenty-eight patients treated for lower limb discrepancy by limb lengthening over an int

Empyema of the gall bladder: reappraisal in the laparoscopy era.

BACKGROUND: Flap-based and podoplastic limb salvage in the foot and ankle is difficult due to the presence of biomechanical forces that can impact flap healing and complication rates and, in turn, limb salvage rates. For this reason, external fixation is indicated to neutralize forces across the flap interface and allow for optimal flap take and healing. Although external fixation for flap immobilization is the current standard of care, not much is known about how duration and timing may impact complication and salvage rates. METHODS: We retrospectively identified and analyzed complication an
