Feasibility of medial parapatellar approach in unicompartmental knee arthroplasty for moderate to severe varus deformity.

RESEARCH BACKGROUND AND PURPOSE: Unicompartmental knee arthroplasty (UKA) is well-established for mild varus deformity, but its application in moderate to severe varus cases remains technically challenging. This study aims to preliminarily evaluate whether adopting the traditional medial parapatellar approach in UKA procedures for these complex deformities can yield acceptable early outcomes. METHODS: A retrospective study was conducted. Between January 2023 and March 2024, 9 patients with moderate to severe varus deformity underwent medial UKA using a cemented fixed-bearing Link prosthesis b

Associations between TT-TG distance and rotational and coronal alignment in recurrent patellar dislocation.

PURPOSE: To evaluate the relationship between TT-TG distance and lower-limb anatomical parameters in recurrent patellar dislocation. METHODS: Weight-bearing long-leg radiographs and knee CT scans of 99 patients with recurrent patellar dislocation were reviewed. Trochlear dysplasia was classified according to the Dejour version 2 classification. Tibial tubercle-trochlear groove (TT-TG) distance was measured together with coronal alignment (mFTA and related angles) and rotational alignment (tibiofemoral rotation and tibial torsion). Patients were stratified by TT-TG distance and mFTA-based alig

Thin flap sulcus-deepening trochleoplasty in patellar instability yields good functional outcomes without progressive cartilage deterioration in the short-term follow-up-A retrospective single-surgeon cohort study.

PURPOSE: Sulcus-deepening trochleoplasty (TP) effectively treats patellofemoral (PF) instability (PFI) caused by high-grade trochlear dysplasia (TD), but current evidence is based on small case series. We hypothesised, that TP would result in significant functional improvements and a low re-dislocation rate but would not accelerate the progression of PF cartilage deterioration. METHODS: We retrospectively reviewed all TP cases performed by a single surgeon between 2015 and 2021. Inclusion criteria were postoperative Magnetic resonance imaging (MRI) >6 and >12 months and a clinical follow-up >

Flexible Flat Foot, Short Tendo-Achilles, and Altered Gait.

Background Pediatric patients often present with vague complaints involving the anterolateral foot and ankle, the medial knee, the lower back, and the hip. In our experience, closer examination of these patients reveals a constellation of symptoms that involve pathology in the ankle, knee, back, and hip. This study aimed to detail the identification and treatment of patients with the triad of flexible flat feet, tight Achilles complex, and altered gait, and their clinical course over time. Methods All patients of age 18 years or younger who presented to our urban academic center outpatient cli

Fibular nonunion after closed-wedge high tibial osteotomy.

UNLABELLED: Closed-wedge valgus high tibial osteotomy (HTO) has been reported to be an effective procedure for the treatment of medial compartment osteoarthritis of the knee. It requires shortening the fibula, for which many techniques have been described. Dislocation of the proximal tibiofibular joint limits the correction angle of the procedure and the osteotomy of the fibular head runs the rare but dramatic risk of common fibular nerve palsy, which is why many surgeons perform the osteotomy more distally at the shaft. However, the potential complications of fibula shaft osteotomy in closed-

[Osteochondral fractures of the distal femur].

BACKGROUND: Chondral or osteochondral avulsions of the lateral distal femur edge after luxation of the patella are well known. Less common are impression fractures of the retropatellar joint or the lateral trochlea. Furthermore, on standard knee X-rays these injuries may not be seen and thus not properly be treated. By presenting our cases we show clinical symptoms, diagnostic pathways and the therapeutic approach for osteochondral fractures of the distal femur. MATERIALS AND METHODS: The first patient had a large osteochondral impression fracture of the lateral femur trochlea and retropatell

[Knee joint hemarthrosis--differential diagnostic considerations for planning an operation].

From 1980 to the end of 1988, arthroscopy has been performed on 620 patients with acute hemarthrosis of the knee joint. Of all the intraarticular lesions, 89.4% required surgery. Arthroscopy has changed in the last decade from diagnostic screening to invasive instrumentation for exact operative planing and alternative operative techniques. Hemarthrosis in stable knee joints was caused by minor lesions (7.84%) that did not require surgical procedures. Seventy-three patients had traumatic patellar dislocations--in 33 cases associated with chondral or osteochondral fractures. In 54.8% of the isol

Cementless reconstruction of massive tibial bone loss in revision total knee arthroplasty.

Reconstruction of massive proximal tibial defects caused by failed cemented total knee arthroplasty (TKA) was evaluated using cancellous allograft and a rigidly fixed tibial component. Twenty patients with massive tibial defects requiring revision TKA were operated on using this technique from December 1984 to December 1986. All these had tight mediolateral capsular structures so that satisfactory varus-valgus stability was achieved. Only three had competent posterior cruciate ligaments. The tibial component had a cobalt chromium tray with a six-inch-long smooth stem, porous undersurface, and

Lower re-revision rates after UKA-to-TKA than UKA-to-UKA revision for failed medial unicompartmental knee arthroplasty at mid-term follow-up.

PURPOSE: Revision following unicompartmental knee arthroplasty (UKA) remains challenging, and optimal revision strategy is still debated. This study analysed failure mechanisms after aseptic medial UKA revision and compared mid-term outcomes between UKA-to-UKA and UKA-to-total knee arthroplasty (TKA) revision procedures. METHODS: A retrospective single-centre study included 113 consecutive aseptic UKA revisions performed between 2001 and 2020. Patients were stratified into UKA-to-UKA (n = 38) and UKA-to-TKA (n = 75) groups. Progression of lateral osteoarthritis (31%) and aseptic loosening (27

4in1 Procedure in Treating Congenital Dislocation of Patella in Children.

OBJECTIVES: Congenital dislocation of patella (CDP) is a rare condition and surgery is needed to treat CDP once a diagnosis is made. The 4in1 procedure includes lateral release, correction of quadriceps, Roux-Goldthwait procedure, and medial patello-femoral ligament (MPFL) reconstruction. This study was aimed at evaluating the efficacy and outcome of the 4in1 procedure in the operative treatment of CDP. METHODS: CDP patients were retrospectively collected from two operative centers from January 2013 to December 2024. For all patients, patellae on both sides were examined by x-ray, CT, and MR

Is fascia lata a viable graft for MPFL reconstruction? An overview of surgical technique and scoping literature review.

Lateral patellar dislocation is irrefutably one of the most common knee injuries, while subsequent medial patellofemoral ligament (MPFL) damage requires proper orthopedic care. Treatment of these injuries is regularly associated with the need for MPFL reconstruction surgery. This operation, often in combination with other procedures, can result very fruitful in restoring knee kinematics. There is a considerable variety of options for the orthopedic surgeon to choose from regarding graft selection when performing MPFL reconstruction surgery, including the semitendinosus tendon, the gracilis, an

Gait analysis of adult patients with complete congenital dislocation of the hip.

Comprehensive gait analysis is valuable in understanding the performance of patients with lower limb disorders. The gait pattern of adult patients with untreated congenital dislocation of the hip (CDH) has not yet been reported. We studied the gait pattern in nine women (mean age 31.4 years) with Crows group IV CDH. Six had unilateral and three had bilateral involvement. They were not treated during childhood and had no pain at the time of study. A control group comprised 15 normal female subjects of the same age group. Gait was studied using a motion-analysis system, force plateforms, and com

Double-Bundle PCL Reconstruction in the Multiple-Ligament Injured Knee.

BACKGROUND: Multi-ligament knee injuries and knee dislocations are potentially devastating injuries that typically result in injury to both cruciate ligaments and variable injury to collateral ligament complexes. INDICATIONS: Surgical intervention typically leads to better patient outcomes than nonoperative management. We describe our preferred technique for bicruciate ligament reconstruction with a focus on double-bundle posterior cruciate ligament (PCL) reconstruction. TECHNIQUE DESCRIPTION: A transtibial approach is used to drill the PCL tibial tunnel. A safety incision on the proximal me…

Effect of surgical treatment on relative tibial external rotation in patients with recurrent patellar dislocation.

BACKGROUND: In the knee joints of patients with recurrent patellar dislocation (RPD), an increased relative tibial external rotation (rTER) is often observed. However, the changes in this parameter pre- and postoperatively remain unclear. PURPOSE: The purpose of this study was to reveal preoperative and postoperative changes in rTER in patients with RPD. METHODS: A total of 48 cases with RPD in the knee joint were included in the study group, while 50 cases with normal knee joints were included in the control group. The measured parameters included tibial tubercle-trochlear groove distance (

Mid-to long-term results of total hip arthroplasty after contralateral lower extremity amputation.

OBJECTIVE: There is limited literature concerning the outcomes and role of THA as a surgical option for amputee patients. The aim of this study is to determine the mid-to long-term survival and complication rates of cementless total hip arthroplasty (THA) in patients with contralateral below knee amputations. METHODS: A retrospective review of 54 patients with below knee amputation were perfomed who underwent THA for osteoarthritis of the contralateral hip over a 5-year period between 1999 and 2014. Patients were monitored for at least 5 years and assessed with the Harris Hip Score and activi

Revision of failed unicompartmental knee replacement to total knee replacement.

Recent reports suggest good outcome results following unicompartmental knee replacement (UKR). However, a number of authors have commented on the problem of osseous defects requiring technically difficult revision surgery. We reviewed clinical outcomes following revision total knee replacement (TKR) for failed UKR and analysed the reasons for failure and the technical aspects of the revision surgery. Between 2001 and 2010 our institute performed 132 UKR's out of which 33 required revision to TKR during a period 6 years. Demographics, details and indications for primary and revision surgery, th

Management of patellar problems in skeletally mature patients with nail-patella syndrome.

PURPOSE: Nail-patella syndrome (NPS) or hereditary onychoosteodysplasia is a rare autosomal dominant disease, characterized by a tetrad of findings, which include fingernail abnormalities, hypoplasia of the patellae, radial head dislocation and prominent iliac horns. Most of the literature on the treatment of patellar problems in NPS concerns paediatric patients, and there is no standard treatment algorithm for adult patients. METHODS: We reviewed the charts of skeletally mature patients with NPS who presented to our clinic. We reviewed the presenting complaints, the physical examination find

Glenohumeral translation in the asymptomatic athlete's shoulder and its relationship to other clinically measurable anthropometric variables.

To determine the degree of shoulder translation in uninjured athletes, we examined 76 Division I collegiate athletes (44 women and 32 men) for passive range of motion in both shoulders and for knee and elbow hyperextension. Translation was based on a scale of 0 to 3+. Shoulders with symptoms of pain or a history of instability or dislocation were excluded from this study. Forty-six shoulders had 0 anterior translation, 75 had 1+, and 31 had 2+. Thirteen shoulders had 0 posterior translation, 56 had 1+, and 83 had 2+. Thirty-eight shoulders had 0 inferior translation, 105 had 1+, and 9 had 2+.
