Treatment of Dentofacial Deformities Secondary to Osteochondroma of the Mandibular Condyle Using Virtual Surgical Planning and 3-Dimensional Printed Surgical Templates.

PURPOSE: One-stage treatment for condylar osteochondroma and secondary facial deformities by resection and reconstruction of the mandibular condyle, orthognathic surgery, and mandibular contouring has been reported recently. This study investigated the clinical feasibility of treating osteochondroma of the mandibular condyle and secondary dento-maxillofacial deformities by virtual surgical planning and 3-dimensional (3D) printed surgical templates. MATERIALS AND METHODS: A composite skull model with accurate dentition was obtained with data from spiral computed tomography (CT) and surface sca

Adverse events reported after the use of recombinant human bone morphogenetic protein 2.

PURPOSE: The US Food and Drug Administration has approved recombinant human bone morphogenetic protein 2 (rhBMP-2) (Infuse Bone Graft; Medtronic Sofamor Danek, Minneapolis, MN) as an alternative to autogenous bone graft for sinus augmentations and for localized alveolar ridge augmentations for defects associated with extraction sockets. The objective of this analysis was to characterize adverse events reported after the use of rhBMP-2 in oral and maxillofacial procedures. MATERIALS AND METHODS: The US Food and Drug Administration's Manufacturer and User Facility Device Experience database con

Virtual planning of composite mandibular reconstruction with free fibula bone graft.

Functional mandibular reconstruction after tumor resection is challenging. Currently, voxel-based craniofacial surgery and virtual planning of craniofacial surgical procedures are becoming increasingly popular. We report on a 56-year-old patient with an infiltrating recurrent squamous cell carcinoma of the mandible and buccal mucosa. Virtual resection of the mandible was accomplished using virtual reality techniques. Based on virtual geometric data, a metal template was configured for optimal contouration of a fibular bone graft while it was still pedicled in the donor field. Our operative pro

Outcomes of the pectoralis major muscle flap for covering a mandibular reconstruction plate: Our experience at the Leiden University Medical Center.

BACKGROUND: Free vascularized fibula reconstruction is the gold standard for oromandibular defect reconstruction following tumor ablative surgery. However, in patients with poor prognosis, compromised general health, or inadequate vascular status in the lower extremities or chest, less invasive alternatives may be preferable. This study evaluated outcomes and complication rates of mandibular defect reconstruction using a reconstruction plate covered with a pedicled pectoralis major muscle flap (PMMF), performed after tumor ablation or for osteoradionecrosis. METHODS: A retrospective review wa

[Benign odontogenic tumours: epidemiological analysis of 97 cases in the Algerian population].

INTRODUCTION: Odontogenic tumours are often benign. They are originating from dento-maxillary structures and their embryonic remnants. They represent a wide variety of lesions derived from epithelial cells, mesenchymal, or both. The series reported in literature show a distinct geographic variation. However, there has been no concrete study and detailed epidemiological profile of these tumours in the Algerian population. MATERIALS AND METHODS: Our prospective study based on the classification of the World Health Organization (WHO) in 2005, focused on the epidemiological analysis of 97 cases o

Use of gas combination cryosurgery for treating ameloblastomas of the jaw.

This study evaluated the results of curettage followed by spray propane, butane, and isobutane gas combination cryosurgery in 10 patients with ameloblastoma. The patients' age ranged from 7 to 87 years (mean, 31 years), with equal prevalence in both men and women. Five cases were diagnosed as solid ameloblastomas and 5 as unicystic ameloblastomas. Before enucleation and cryosurgery, the unicystic lesions received marsupialisation to decrease their size. No patient showed evidence of clinical or radiographic recurrence, pathologic fracture, or infection after treatment with enucleation and cryo

[Part III: free periosteal flaps as a treatment for mandibular osteoradionecrosis].

INTRODUCTION: The vascularized periosteal free flap transposes a non-irradiated soft tissue with neo-angiogenesis, bone induction, and osteogenesis qualities. Its technique and interest are studied for early osteoradionecrosis. MATERIAL AND METHOD: A retrospective study was made between 1992 and 2002. Thirty-four patients were selected among the 59 cases operated for osteoradionecrosis; all patients were classified stage II and I on the Epstein scale. Twenty-six patients were reconstructed using a non-periosteum free flap (bone, muscular or cutaneous flap), and eight patients were reconstruct

[Long-term complications of radiotherapy after mandibular reconstruction with vascularized bnoe graft].

INTRODUCTION: The use of vascularised composite free flaps (VCFF) has become a widely accepted method for primary reconstruction of mandibular defects. Adjuvant or neo-adjuvant radiotherapy (RTH) increases susceptibility to trauma and infections. The aim of this study is to compare the incidence of local complications after mandibular reconstruction with vascularised composite free flaps, related to pre- or postoperative radiotherapy. The effects of these complications on functional rehabilitation with a dental prosthesis fixed on bone implants are also studied. METHODS: Between 1990 and 1999

Dental extractions in relation to radiation therapy of 224 patients.

The case histories of 224 patients treated with radiation therapy for head and neck malignancies at the Radiotherapy Clinic of the University Central Hospital in Turku during the years 1974-77 are reviewed. Of the 92 patients surviving for 5 years, 90 were available for re-examination. The median radiation dosage was 61 Gy in 6 to 8 weeks in patients with squamous cell carcinoma and other solid tumours and 43 Gy in 5 weeks in patients with lymphoma. The oral status of the patients was examined clinically and radiographically. From these patients, 45 teeth had been extracted before irradiation

Cancer of the floor of the mouth: surgical management.

After discussing the changing incidence and known difficulties in the management of mouth floor cancer, reasons are given for the increased use of surgery in treating the more extensive lesions. The significance of the topographic anatomy, patterns of lymph drainage and surgical pathology is emphasized. Principles of treatment, indications and types of operation are discussed for the main lesions encountered. Following this the preparation of patients for surgery, the methods of anaesthesia and certain technical details of surgical treatment are given. The importance of after-care and particul

Feasibility of Flexor Hallucis Muscle-Based Fibula Bone Segment for Ramus Reconstruction in Head and Neck Cancers.

INTRODUCTION: In mandibular reconstruction with a free fibula flap, ramus reconstruction is very important to match the ramus height, which eventually helps in good mouth opening and mastication. The fibula bone segment attached to the pedicle is used for ramus reconstruction. But when the bone requirement is large and the pedicle is short, we propose an alternate method in which the bone segment is separated from the pedicle but is attached to the flexor hallucis longus (FHL) muscle from which it receives its vascularity indirectly. Through this article, we aim to examine the non-inferiority

Scapular osteocutaneous free flap for total lower lip and mandible reconstruction.

PURPOSE: Total lower lip and mandible defects following tumor resection present challenging reconstructions. The use of dual free flaps leads to increased donor-site morbidity and risk of flap failure. We present a single scapular free flap approach with unique use of osteotomies for lip reconstruction in a small series of patients. METHODS: A case series was conducted from 2007 to 2012 on three patients with squamous cell carcinoma of the oral cavity requiring large resection. Patients ranged in age from 34 to 64 years. A scapular free flap was used for reconstruction of mandible, floor of m

[Temporomandibular joint primitive tumors and pseudo tumors].

The temporomandibular joint (TMJ) can be the site of bone, cartilaginous, or synovial tumors. There is no well-defined histological classification. We listed all benign tumors, malignant primitive tumors, and rare pseudo tumors of the TMJ. We provide a list to help for the diagnosis and the differential diagnosis of non-tumoral lesions by far the most frequent.

Removal of infratemporal fossa schwannoma via a transmandibular transpterygoid approach.

Trigeminal schwannomas are commonly located in the intracranium, and extracranial schwannomas limited to the infratemopral fossa (ITF) are extremely rare. Trigeminal mandibular schwannoma was found in the left ITF of a 48-year-old man who presented with a 1-year history of left facial pain and trismus. The schwannoma was completely removed by a transmandibular transpterygoid approach with no major complications. This case is discussed with a review of the literature on this rare occurrence, focusing on the surgical approach.

Odontogenic tumors in mice carrying albumin-myc and albumin-rats transgenes.

Odontogenic tumors that produce abnormal tooth-like structures are repeatedly observed in mandibles of mice that carry both albumin-myc and albumin-ras transgenes. The earliest lesions appear among the periodontal ligament mesenchymal cells, but later lesions include an epithelial component. Subsequent tumor development recapitulates the process of normal tooth formation, which requires multiple sequential cell signals, and results in cell differentiation, matrix secretion, and mineralization. Tumor cells with epithelial morphology produce ras oncoprotein, consistent with an epithelial origin

Implications of radiation dosimetry of the mandible in patients with carcinomas of the oral cavity and nasopharynx treated with intensity modulated radiation therapy.

Intensity modulated radiation therapy (IMRT) is a newer method of delivering highly conformal, salivary gland sparing radiation treatment that is finding increasing applications in head and neck malignancies. However, the radiation dose distribution to the mandible is rarely considered with IMRT, and the potential risks of osteoradionecrosis or osseointegrated implant failure are not well characterized for this modality. In a series of 10 patients with oral cavity and nasopharyngeal cancers who previously underwent IMRT, examination of the three-dimensional mandibular dose distribution was und
