Benign clear cell "sugar" tumor of the lung in a patient with Birt-Hogg-Dubé syndrome: a case report.

BACKGROUND: Birt-Hogg-Dubé (BHD) syndrome is a rare inherited autosomal genodermatosis and caused by germline mutation of the folliculin (FLCN) gene, a tumor suppressor gene of which protein product is involved in mechanistic target of rapamycin (mTOR) signaling pathway regulating cell growth and metabolism. Clinical manifestations in BHD syndrome is characterized by fibrofolliculomas of the skin, pulmonary cysts with or without spontaneous pneumothorax, and renal neoplasms. There has been no pulmonary neoplasm reported in BHD syndrome, although the condition is due to deleterious sequence var

A Case of Micro-medullary Thyroid Carcinoma Presenting as Cancer of Unknown Primary.

Cancer of unknown primary (CUP) with neuroendocrine features is classified as a favorable subset, and treatment is recommended based on the grade of the neuroendocrine tumor (NET). Although rare, medullary thyroid carcinoma (MTC) can present as CUP with neuroendocrine features and should be carefully considered as a crucial differential diagnosis in such cases. A 65-year-old man with no relevant medical history presented with a gradually enlarging left cervical mass. Laboratory evaluation revealed hypokalemia due to adrenocorticotropic hormone (ACTH)-dependent Cushing's syndrome. Contrast-enha

Case report of CT-guided lung biopsy complicated by air embolism.

RATIONALE: Cerebral arterial air embolism is a rare but potentially fatal complication of computed tomography (CT) guided lung biopsy. Hyperbaric oxygen (HBO2) is the first line of treatment for arterial gas embolism and needs to be administered immediately after the event. Early HBO2 can reduce the mortality rate of cerebrovascular air embolism. PATIENT CONCERNS: A 65-year-old woman was diagnosed with a pulmonary nodule with a diameter of approximately 0.8 cm in the right lower lung. The patient developed consciousness, convulsions, and arrhythmia after CT-guided lung biopsy. DIAGNOSIS: Cra

Development of a multipotent diagnostic tool for chest X-rays by multi-object detection method.

The computer-aided diagnosis (CAD) for chest X-rays was developed more than 50 years ago. However, there are still unmet needs for its versatile use in our medical fields. We planned this study to develop a multipotent CAD model suitable for general use including in primary care areas. We planned this study to solve the problem by using computed tomography (CT) scan with its one-to-one matched chest X-ray dataset. The data was extracted and preprocessed by pulmonology experts by using the bounding boxes to locate lesions of interest. For detecting multiple lesions, multi-object detection by fa

Solitary pulmonary nodules in patients with extrapulmonary neoplasms.

PURPOSE: To determine the frequency of single lung metastasis, primary lung cancer, and benign lesions in patients with a solitary lung nodule and a primary extrapulmonary neoplasm. MATERIALS AND METHODS: The authors evaluated the electronic charts of 149 patients with an extrapulmonary malignant neoplasm and a solitary pulmonary nodule. The histologic characteristics of the nodule were correlated with those of the extrapulmonary neoplasm and with patient age and smoking history. RESULTS: Patients with carcinomas of the head and neck, bladder, breast, cervix, bile ducts, esophagus, ovary, pr

Basal ganglia cryptococcomas presenting as parkinsonism in an immunocompetent patient.

Cryptococcal disease typically presents as meningitis in immunocompromised hosts, whereas basal ganglia cryptococcomas causing parkinsonism in immunocompetent patients are uncommon and easily overlooked. We describe a 61-year-old apparently immunocompetent man with subacute meningoencephalitis, abnormal cerebrospinal fluid (CSF), and a pulmonary nodule initially managed as tuberculous meningitis, who subsequently developed severe asymmetric parkinsonism. Neuroimaging later demonstrated diffuse leptomeningeal enhancement and multiple basal ganglia and brainstem cryptococcomas and repeat CSF and

Pulmonary Resection for Metastasis of Hepatocellular Carcinoma Recurring After Liver Transplant: An Italian Multicenter Experience.

Background and aim: Liver transplantation (LT) is a validated treatment for hepatocellular carcinoma (HCC). HCC recurrence occurred between 8 and 20% of patients and lung is the most frequent site. Pulmonary metastases resection (PMR) prolongs survival, however in LT-setting the impact on survival is unclear. To give new lights on this issue, we report the experience of three Italian LT Centers. Methods: All consecutive HCC transplanted patients in three Italian LT Centers, who developed pulmonary metastasis from HCC (PM-HCC), as first metastasis, from 2008 to 2018, were included whenever trea

Preoperative concurrent chemoradiotherapy followed by extrapleural pneumonectomy for patients with non-small cell lung cancer with malignant pleural effusion and/or pleural nodules: Ten-year results of a prematurely terminated single institute phase II trial.

BACKGROUND: Malignant pleural effusion and/or pleural nodules are a final stage of disease extension of non-small cell lung cancer (NSCLC), which is currently classified as M1a-Stage IV disease. The role of surgery, especially extrapleural pneumonectomy (EPP) for local disease control and its impact on the survival of patients remains uncertain. PATIENTS AND METHODS: This single institute phase II trial was performed from March 1997 to March July 2004 to assess the effects of induction concurrent chemoradiotherapy (CRT) using a pro-drug of 5-FU, uracil-tegafur (UFT(®), Taiho Pharmaceutical Co

Forceps biopsy and suction catheter for sampling in pulmonary nodules and infiltrates.

Transbronchial lung biopsy with forceps is a standard procedure in bronchoscopic tissue sampling. Suction catheter aspiration is another technique, but it is not widely known and almost no data exist regarding its diagnostic efficiency. 272 patients were included in a prospective and randomised study between February 2007 and October 2009. All were referred for bronchoscopic evaluation of pulmonary nodules/masses or infiltrates. We compared the diagnostic yield of forceps biopsy and suction catheter aspiration for a definite diagnosis and looked at whether such a diagnosis depends on the under

Post-thoracotomy Fibrin Glue Pleurodesis for Persisting Air Leaks: A Case Study.

Persistent air leaks (PALs) secondary to residual pleural spaces (RPSs) are the most common complication post-thoracotomy and positively correlate with the length of hospital stay. Among numerous treatment strategies, fibrin glue pleurodesis has the benefit of filling the RPS and can be used multiple times without anesthesia or sedation. A 71-year-old male presented with a 1 cm positron emission tomography positive nodule in the right upper lobe, who had undergone prior thoracotomy for a lung mass in 2011. Thoracotomy with wedge resection was performed with findings of diffuse lung parenchymal

Unusual metastatic pattern of endometrial carcinoma with distal foot involvement.

INTRODUCTION: Endometrial carcinoma typically metastasizes to the lungs, liver, and bones of the axial skeleton. Metastatic spread to the bones of the foot is exceedingly rare and may present with nonspecific musculoskeletal symptoms, often mimicking infection. Recognizing such atypical metastatic patterns is essential for timely diagnosis and management, particularly in patients with high-risk disease. CASE REPORT: A 62-year-old woman with FIGO stage IIIB endometrioid adenocarcinoma, previously treated with surgery and radiotherapy, presented with progressive pain and swelling of the right f

LungNet22: A Fine-Tuned Model for Multiclass Classification and Prediction of Lung Disease Using X-ray Images.

In recent years, lung disease has increased manyfold, causing millions of casualties annually. To combat the crisis, an efficient, reliable, and affordable lung disease diagnosis technique has become indispensable. In this study, a multiclass classification of lung disease from frontal chest X-ray imaging using a fine-tuned CNN model is proposed. The classification is conducted on 10 disease classes of the lungs, namely COVID-19, Effusion, Tuberculosis, Pneumonia, Lung Opacity, Mass, Nodule, Pneumothorax, and Pulmonary Fibrosis, along with the Normal class. The dataset is a collective dataset

Metastatic adenocarcinoma mimicking 'target sign' of cerebral tuberculosis.

We present a 74-year-old male ex-smoker presenting with a 6-week history of personality change, confusion and headache. Magnetic resonance imaging revealed multiple supratentorial and infratentorial parenchymal masses, predominately in the frontal and parietal lobe white matter. A thin enhancing halo was demonstrated with central low signal intensity on T(1)-and T(2)-weighted imaging compatible with calcification. A tiny extra-axial lesion was also noted near the right cerebellopontine angle. Computed tomography (CT) scan confirmed the finding of a 'target' lesion with a central core of calcif

Malignant Peritoneal Mesothelioma: A Case Report.

Malignant peritoneal mesothelioma (MPM) is a rare tumor of the serous membranes of the peritoneum and has been linked to exposure to asbestos and other risk factors. The clinical manifestations are vague, with a wide clinical spectrum, predominantly related to the abdominal involvement of the disease. Localized mesothelioma is an uncommon manifestation of the disease. Common symptoms include abdominal pain or abdominal distention, nausea, anorexia, and weight loss. Rarely, patients present with paraneoplastic syndrome. Due to the nonspecific symptoms, many patients already have advanced diseas

Comparative assessment of image quality and radiation dose: spectral purification computed tomography (Sn100kV) with adaptive iterative reconstruction for tuberculosis follow-up.

BACKGROUND: During tuberculosis treatment, multiple chest computed tomography (CT) scans are required for follow-up, each with an effective radiation dose (ED) of approximately 7 mSv. If treatment outcomes are poor or complications arise, additional scans may be necessary, thereby increasing the cumulative radiation burden. This study aimed to evaluate the effects on image quality of low-dose CT using spectral purification technology, in combination with varying levels of sinogram-affirmed iterative reconstruction (SAFIRE) algorithms, with the goal of identifying the optimal SAFIRE level for i

A lung squamous cell carcinoma demonstrating spontaneous partial regression of the primary tumor with lymph node metastasis: a case report with analysis of the tumor immune microenvironment.

BACKGROUND: Spontaneous regression (SR) of malignant tumors is a well-known but rare phenomenon in lung cancer. Previous studies have suggested that antitumor immune responses may contribute to SR. CASE DESCRIPTION: We report the case of a 74-year-old man with a right lower lobe pulmonary nodule that demonstrated regression during observation, accompanied by mediastinal lymph node (LN) metastasis after nondiagnostic bronchoscopic and needle biopsies. Surgical resection confirmed squamous cell carcinoma with subcarinal LN metastasis. The primary tumor was largely replaced by fibrosis, with res

An Uncommon Link: Syndrome of Inappropriate Antidiuretic Hormone Secretion (SIADH) in the Setting of Myeloproliferative Neoplasms.

Syndrome of inappropriate antidiuretic hormone secretion (SIADH) is a well-known cause of hyponatremia, frequently associated with small-cell lung carcinoma. Its association with myeloproliferative neoplasms (MPNs), however, is rare and underreported. We describe the case of an 81-year-old female with a history of chronic hyponatremia attributed to SIADH, requiring multiple hospitalizations. During her hospitalizations, she would be on fluid restriction and urea tablets. Her medical history included paroxysmal atrial fibrillation, a stable pulmonary nodule, and seropositive rheumatoid arthriti
