Brain invasion in meningiomas-clinical considerations and impact of neuropathological evaluation: a systematic review.

With the release of the 2016 edition of the World Health Organization (WHO) Classification of Central Nervous System Tumors, brain invasion in meningiomas has been added as a stand-alone criterion for atypia and can therefore impact grading and indirectly adjuvant therapy. Regarding this rising clinical importance, we have reviewed the current knowledge about brain invasion with emphasis on its implications on current and future clinical practice. We found various definitions of brain invasion and approaches for evaluation in surgically obtained specimens described over the past decades. This

Extraneural spread of glioblastoma--report of two cases.

BACKGROUND: Glioblastoma, a highly aggressive tumor, accounts for the majority of all primary brain tumors in adults. Despite a destructive local growth pattern, extraneural spread of these tumors is extremely rare. CASE 1: We describe the case of a 58-year-old man with glioblastoma, in whom an epidural mass was diagnosed 5 months after initial local therapy of the brain. A positron emission tomography (PET) scan revealed multiple metastases in the lungs, in the retroperitoneum, and in the left trochanter minor region. A soft tissue swelling of the right thigh was histologically proven to be m

Retinoinvasive malignant melanoma of the uvea.

AIMS: To define a retinoinvasive phenotype of uveal melanoma based on an informative case and survey of literature. METHODS: A 65-year-old woman developed a circumscribed mixed cell type melanoma of the ciliary body that was locally excised. After 6 years, secondary glaucoma evolved. Three years later a ring melanoma was diagnosed and the eye was enucleated. The histopathological material was analysed by immunohistochemistry. RESULTS: A spindle cell type ring melanoma infiltrated the iris and ciliary body diffusely, and extended through the aqueous outflow channels and iridocyclectomy flap e

[Diagnosis of colon cancer (depth of invasion) by barium enema examination; present condition].

Accuracy of the barium enema examinations in diagnosis of the depth of invasion of the small carcinomas of the colon was reviewed. The materials included 101 sessile carcinomas and 12 small advanced carcinomas that were 20 mm or less in diameter. The carcinomas with stalks and the depressed type early carcinomas (Type IIc) were excluded from the study, because the former were impossible to the nature by X-ray examinations and the latter were extremely rare. Presence of the depressed portions on the surface of the elevated lesions observed in the frontal views and the degree of the deformity ob

Mechanisms by which chromobox homolog 4 regulates angiogenic mimicry and angiogenic activity in osteosarcoma.

Osteosarcoma (OS) is a very aggressive malignant neoplasm characterized by significant metastases and a deficiency of treatment targets. Therefore, it is urgent to find new therapeutic mechanisms and targets for OS. We assessed the capability of chromobox homolog 4 (CBX4) to modulate OS growth using in vivo and in vitro dual tests utilizing short hairpin RNA technology, 3-dimensional tumor sphere building, immunofluorescence co-localization, and other biotechnological methods. Our findings indicated that CBX4 was overexpressed in OS tissues, and the silencing of CBX4 markedly reduced OS cell p

Update on Rectal Neuroendocrine Neoplasms.

Rectal neuroendocrine neoplasms (R-NENs) are increasingly identified in clinical practice, mainly due to the expansion of colorectal cancer screening programs. These tumors are most often detected incidentally as small, well-differentiated lesions. Although many R-NENs exhibit indolent behavior, prognosis and management depend on accurate staging, with particular emphasis on evaluating regional lymph nodes and distant metastases. Cross-sectional imaging is central to this assessment. Endoscopy and endoscopic ultrasound provide essential information regarding tumor size and depth of invasion, w

Diagnostic accuracy of fine-needle aspiration cytology in salivary gland neoplasms: a systematic review and meta-analysis.

INTRODUCTION: Fine-needle aspiration cytology is a widely used minimally invasive tool for salivary gland lesions, though diagnostic challenges arise due to overlapping features between benign and malignant tumors. This systematic review and meta-analysis evaluate its diagnostic performance in salivary gland neoplasms. METHODS: A comprehensive literature search was conducted in PubMed/MEDLINE, Embase, Scopus, and Web of Science. Studies reporting sensitivity, specificity, or sufficient data to construct 2 × 2 tables were included. Data extraction was performed independently by two reviewers,

Diagnostic Yield of CE-EBUS in Mediastinal and Hilar Lymphadenopathy: A Preliminary Study.

Background/Objectives: Contrast-enhanced endobronchial ultrasound (CE-EBUS) is a minimally invasive technique that combines traditional endobronchial ultrasound (EBUS) with a contrast agent (sulfur hexafluoride), enhancing the visualization of blood flow in mediastinal and hilar lymph nodes. This study aimed to assess the use of CE-EBUS in patients with advanced neoplasms and hilar or mediastinal lymphadenopathy, particularly to improve diagnostic accuracy and expedite sample collection. Methods: A retrospective observational study was conducted from April 2021 to December 2023, involving 49 p

Indications and techniques for minimally invasive spleen-preserving distal pancreatectomy.

Spleen-preserving distal pancreatectomy (SPDP) has gained increasing attention as a safe and effective alternative to distal pancreatectomy with splenectomy, particularly for benign and low-grade malignant lesions of the pancreatic body and tail. This narrative review evaluates the current evidence, technical considerations, and disease-specific indications for SPDP. Literature was reviewed using up to date scientific evidence and most recent national and international guidelines. Studies addressing SPDP outcomes, splenectomy complications, and disease-specific oncologic principles were includ

The Infectious Clue: Linking Bacterial Infections to Underlying Malignancies.

Emerging evidence suggests some bacterial infections may be early signs of hidden cancers rather than random events. Yet this link remains under-recognized in practice, representing an often-missed diagnostic opportunity. Large registry studies show that certain infections are linked to a sharply increased short-term risk of cancer detection, with most of the excess diagnoses clustering in the first 6 months after the index episode. Key associations include the following: (i) anaerobic or gut-derived bacteremia with Bacteroides, Clostridium, Fusobacterium, or pks+Escherichia coli before colore

Challenges in Diagnostic Imaging of Appendiceal Signet Ring Cell Carcinoma: A Case Report.

Appendiceal signet ring cell carcinoma (ASRC) is an exceptionally rare and aggressive malignancy, representing < 0.1% of gastrointestinal neoplasms. Its nonspecific imaging manifestations frequently mimic inflammatory conditions such as appendicitis, leading to delayed diagnosis. We report a diagnostically challenging case of a 75-year-old woman with recurrent right lower abdominal pain over 6 months. Initial ultrasonography revealed appendiceal wall thickening (19.5 mm diameter) without layered structure loss, suggestive of chronic appendicitis. Follow-up computed tomography (CT) demonstrated

Malignant Perivascular Epithelioid Cell Tumor (PEComa) of the Uterus: A Rare Type of Mesenchymal Tumors and a Management Challenge.

Gynecologic perivascular epithelioid cell tumors (PEComas) are rare mesenchymal neoplasms characterized by the co-expression of melanocytic markers (HMB-45 and Melan-A) and smooth muscle markers (SMA, desmin, and caldesmon). The uterus is the most common organ affected, with approximately 110 cases reported worldwide, while occurrences in the cervix, vagina, ovary, and other gynecologic locations are exceptionally rare. These tumors typically present with nonspecific symptoms such as abnormal uterine bleeding and pelvic pain, often mimicking other uterine neoplasms. Histopathologically, PEComa

Surgical Resection and Abdominal Wall Reconstruction of a Rapidly Progressive Desmoid Tumor: A Case Report.

Desmoid tumors (DTs) are rare, locally invasive soft tissue neoplasms characterized by fibroblastic proliferation and a lack of metastatic potential. Despite their histologically benign nature, their infiltrative behavior and unpredictable progression present significant therapeutic challenges, particularly when located in the abdominal wall, where resection and reconstruction may be necessary. We present the case of a 37-year-old woman with a rapidly enlarging, painful mass in the right rectus abdominis muscle. Imaging and biopsy confirmed desmoid-type fibromatosis. After a period of surveill

Diagnostic accuracy of ultrasound guided core needle biopsy in parotid gland neoplasms: A retrospective comparative study.

Ultrasound-guided Core Needle Biopsies (CNBs) are minimally invasive procedures for diagnosing parotid gland neoplasms, aiming to reduce diagnostic uncertainty and guide management. This study evaluates the efficacy and reliability of CNBs in this context. We retrospectively analyzed 89 patients undergoing 98 CNB procedures. Diagnostic agreement and accuracy were assessed in two distinct subgroups with definitive histological confirmation from resected specimens. The first subgroup evaluated the initial CNB result, while a second subgroup assessed the final conclusive CNB result obtained throu

[Current status and considerations on clinical application of function-preserving pancreatic surgery].

For pancreatic neoplasms, the current clinical treatment strategy is mainly using standard surgical methods, including pancreaticoduodenectomy, distal pancreatectomy with splenectomy, and total pancreatectomy. Standard surgical methods require a larger resection, including resection of some surrounding organs and a large amount of pancreatic parenchyma. The endocrine and exocrine functions of the pancreas are easily damaged. Moreover, since the standard surgical procedure involves the reconstruction of the digestive tract at multiple anastomoses, there is a high risk of pancreatic, biliary, an
