Relationship of the infraorbital nerve and vessels in the pathogenesis of idiopathic trigeminal neuralgia: a theoretical discussion and cadaveric study.

Many etiologies have been postulated to explain the pathogenesis of Trigeminal Neuralgia (TN). Most theories focus on a demyelinating process resulting in ephaptic transmission and recruitment of small pain fibers or a restrictive relationship of the superior cerebellar arteries and the trigeminal nerve roots at the level of the pons which may lead to demyelination. It is postulated that vasodilation or constriction of these vessels in a confined bony canal can create localized regions of demyelination and restrictive relationships creating painful stimuli. It is the purpose of this study to g…

[Mechanism of hypometria caused by pontocerebellar infarction].

By contrast to cerebellar hypermetria, the pathophysiology of cerebellar hypometria associated with a focal lesion is poorly understood. We describe a patient presenting an ischemic stroke involving the left lateral pons and middle cerebellar peduncle and exhibiting severely hypometric ballistic movements of the left wrist. Hypometria was associated with two distinct abnormalities of the triphasic electromyographic (EMG) pattern: a prolongation of the duration of the antagonist EMG activity and a reduced intensity of the agonist EMG activity. The first mechanism has been described previously i…

Listeria rhombencephalitis: report of two cases with early diagnosis and favourable outcome.

We present 2 cases of Listeria monocytogenes rhombencephalitis (L-RE), both affecting previously healthy adult men. Each of them developed a diphasic syndrome first characterized by fever, nausea and headache, followed, in a second phase, by severe brain stem dysfunction at the level of the pons, with meningism, multiple cranial nerve palsies, ataxia, and, in one case, seizures. The early examination of the cerebrospinal fluid (CSF) demonstrated the presence of Gram-positive bacilli whose typical characteristics were compatible with those of Listeria, allowing for immediate administration of a…

[Secondary and late development of hematoma of the brain stem].

The functional and social outcome in 15 patients with brainstem hematomas surviving on the 15th day is reported. At the end of the first year post stroke, the main neurological deficits were sensory, postural and paresis of the 6th and 7th cranial nerves. This resulted from the preferential location of the lesions in the lateral and posterior pons. On a functional point of view, 8 patients were independent in daily living. Seven of them were regularly engaged in more elaborated activities, at home and outside: 1 had recovered his previous professional activities. Those functions evolved more s…

The location of descending fibres to sympathetic neurons supplying the eye and sudomotor neurons supplying the head and neck.

Evidence is given of the location in man of the fibres going to the sympathetic neurons of the lateral horn that supply the intrinsic and extrinsic muscles of the eye and the sweat glands of the head and neck. For the region of the pons and medulla, the evidence is abstracted from the literature. For the cervical spinal cord, the evidence is from our cases of anterolateral cordotomy. In the medulla, thrombosis of the artery of the fossette latérale destroys the fibres; this locates the fibres in the posterolateral retro-olivary area. But not all fibres to the sudomotor neurons lie there: some …

Brainstem auditory evoked responses in brainstem infarction.

Brainstem auditory evoked responses (BAERs) were recorded in 40 patients with clinically definite brainstem infarction, and results were compared to localizations from physical signs and CT scans. The BAER was abnormal in 92% of patients with evidence on physical examination of dysfunction of lateral structures in the pons or midbrain. Normal BAERs were seen with medially-situated or medullary lesions. When both rostrocaudal level and lateralization were considered, the BAER indicated damage in additional areas not evident on physical examination in 25% of patients. However, physical signs ind…

Neuropathologic features, including pontine schwannosis, in a four-year survivor of probable Reye's syndrome with secondary Möbius syndrome.

Frequent seizures and loss of motor, language, and intellectual skills necessitated care in an institution for the physically and mentally handicapped for 4 years after diagnosis of presumed Reye's syndrome in a 10-month-old boy. M obius syndrome was diagnosed and bilateral tarsorrhaphy performed for exposure keratitis. Postmortem examination revealed multiple old boundary zone infarcts in the cerebral cortex, small cavitated infarcts in basal ganglia and thalami, diffuse neuronal loss and gliosis, and a focus of old necrosis in the pons; brain stem herniation during the acute phase of Reye's …

Bilaterally recorded brain stem auditory evoked responses. Their asymmetric abnormalities and lesions of the brain stem.

Simultaneous bilateral recordings (C3 to A1 and C4 to A2) of brain stem auditory evoked responses have been studied in 67 supratentorial lesions, nine midbrain lesions, 21 intrinsic pontine lesions, and 23 extrinsic compressions of the pons. The responses in supratentorial lesions showed completely normal records. In midbrain lesions, wave V was specifically altered. As wave 1 has been shown to be a far-field seventh nerve potential, and wave V the midbrain potential, waves II to IV can be inferred to originate in the central auditory pathway between the seventh nerve and the midbrain. Alterat…

[Trigeminal neuralgia: interpretation of the results of the Dandy method of rhizotomy].

The patterns of sensory loss in 29 cases of Juxta pontine trigeminal rhizotomy are reported. The section of 70% of the portio major gave a similar impairment of tactile, thermal and pain sensibility in the facial area corresponding to the portio of the nerve section in all the patients. These findings do not support Jannetta and Rand's statement that partial to complete section of the porto major at the pons with complete preservation of the intermediate fibre does not affect the perception of light touch. The reported data suggest: 1--intermediate fibres are not mode specific fibres for light…

Comparison of HDL-Associated Antioxidant Activities and Anti-Inflammatory Effect Between Ozonated Sunflower Oil (OSO) and Ozonated Olive Oil (OOO) Under Carboxymethyllysine-Induced Acute Phase in Zebrafish Adults and Embryos.

This study compares the efficacy of ozonated sunflower oil (OSO) and ozonated olive oil (OOO) in terms of antioxidant properties, modulation of high-density lipoprotein (HDL) functionality, and protective effects against carboxymethyllysine (CML)-mediated stress in zebrafish embryos and adults. The spectral and electronic nose (e-nose) analyses revealed that OSO and OOO possessed markedly distinct physicochemical characteristics and volatile and olfactory constituents compared with non-ozonated sunflower (SO) and olive oil (OO). The fluorescence spectrum analysis of HDL treated with OOO and OS

The effect of thyroid hormone status on selected antioxidant parameters in patients with Graves' disease and active thyroid-associated orbitopathy.

INTRODUCTION: Oxidative stress has been implicated in the pathogenesis of thyroid-associated orbitopathy (TAO) in patients with Graves' disease (GD). This study assessed the effect of thyroid hormone abnormalities on selected antioxidant parameters in patients with active TAO. MATERIAL AND METHODS: The study group consisted of 56 patients with GD and active TAO treated with antithyroid medication. Depending on the thyroid hormone level, they were subdivided into two groups: Group 1 - hyperthyroid patients (n = 34) and Group 2 - euthyroid patients (n = 22). The total oxidant status expressed a

Evaluation of oxidant/antioxidant balance in Iberian ibex (Capra pyrenaica) experimentally infested with Sarcoptes scabiei.

Oxidative stress (OS) is an imbalance between radical-generating and radical scavenging activity, resulting in oxidation products and tissue damage. Although some studies have been done in other species, there is a lack of information about the oxidative/antioxidant status in the Iberian ibex (Capra pyrenaica) affected by sarcoptic mange. To clarify this fact, albumin, catalase (CAT), glutathione peroxidase (GSH-Px), superoxide dismutase (SOD), paraoxonase-1 (PON-1), glutathione reductase (GR), reduced glutathione (GSH): oxidized glutathione (GSSG) ratio, thiobarbituric acid reactive substance

Selected markers of oxidative stress in rats with active Heymann nephritis.

BACKGROUND: Reactive oxygen species play an important role in the pathogenesis of different glomerulopathies. The purpose of this study was to examine selected markers of oxidative stress and antioxidant defense in active Heymann nephritis (AHN), which is a model of human membranous nephropathy. MATERIAL AND METHODS: AHN was induced in female Wistar rats by i.p. injection of proximal tubule brush border antigen (Fx1A). The control animals received an equivolume injection of saline. The animals were sacrificed at 6, 9, 15, 17, 18, 19, 22 and 23 weeks after Fx1A administration. We assayed the p

Bilateral adduction palsy with a paramedian midbrain lesion is most likely wall-eyed bilateral oculomotor subnuclear ophthalmoplegia: a case report of a brainstem infarction.

A 73-year-old female was hospitalized in our Department of Neurology for diplopia due to multiple infarctions in the midbrain and pons. A neurological examination revealed bilateral adduction palsy during horizontal gaze with nystagmus of the contralateral abducting eye. Exotropia and impaired convergence were also noticed. The distinctive symptoms indicated wall-eyed bilateral internuclear ophthalmoplegia (WEBINO). We speculate that the cause of WEBINO in our case was bilateral medial rectus palsy via bilateral damage to the medial rectus subnuclei (MRSN) due to the paramedian midbrain infarc…
