Postnatal depression--an examination of psychosocial factors.

Postnatal depression (PND) has been underreported in South Africa. This retrospective study investigated factors which appear to predispose women to PND. Two groups, one consisting of women who suffered from PND and the other of women free of this complaint, provided information on a number of biological, psychological and social factors. In line with current opinion it was found that no single causative factor could be isolated but that a variety of factors may contribute to the problem. Among the factors which distinguished the two groups were the mothers' emotional health during pregnancy,

Community health workers can improve child growth of antenatally-depressed, South African mothers: a cluster randomized controlled trial.

BACKGROUND: Maternal antenatal depression has long-term consequences for children's health. We examined if home visits by community health workers (CHW) can improve growth outcomes for children of mothers who are antenatally depressed. METHODS: A cluster randomized controlled trial of all pregnant, neighbourhood women in Cape Town, South Africa. Almost all pregnant women (98 %, N = 1238) were recruited and assessed during pregnancy, two weeks post-birth (92 %) and 6 months post-birth (88 %). Pregnant women were randomized to either: 1) Standard Care (SC), which provided routine antenatal care

Acute psychiatric disturbance in pregnancy and the puerperium.

Psychiatric disturbance occurring during pregnancy or the puerperium can occur as a first presentation or as a recurrence or deterioration of a previously diagnosed psychiatric illness. Whatever the history or aetiology, the occurrence of psychiatric disturbance during this period can have profound, long-term implications not only for the mental health of the mother, but also for her relationship with her child and other family members, as well as for the child's development. The treatment of psychiatric disturbance at this time can present difficult dilemmas and should always involve a carefu

Peripartum Cardiomyopathy Leading to Multiorgan Dysfunction in a Previously Healthy 30-Year-Old Puerto Rican Woman.

Peripartum cardiomyopathy (PPCM) is a rare and potentially life-threatening cause of systolic heart failure that usually arises in the late stages of pregnancy or the early postpartum period. Its clinical presentation can range from moderate symptoms to severe forms of heart failure, such as cardiogenic shock. However, there is scarce data on severe forms of PPCM leading to multiple organ dysfunction and its management. We report a case of a 30-year-old G2P2 previously healthy Puerto Rican woman postpartum day #56 who presented to our Institution with respiratory symptoms. Further evaluation c

Incorporation of transperineal ultrasonography and pelvic electromyography into the real clinical setting for the prediction of postpartum stress urinary incontinence.

AIM: Risk factors for predicting postpartum stress urinary incontinence (SUI) attract increasingly more attention. Here, we incorporate transperineal ultrasonography (US) and pelvic floor electromyography into the real clinical setting, aiming to evaluate various risk factors comprehensively and objectively and weigh their roles in predicting postpartum SUI. MATERIALS AND METHODS: A total of 530 subjects were enrolled. SUI was diagnosed using the International Consultation on Incontinence Questionnaire-Short Form. The clinical, ultrasonographic, and myoelectric data of the subjects were colle

Left ventricular recovery in an African cohort of patients with peripartum cardiomyopathy.

Peripartum cardiomyopathy (PPCM) is a rare and potentially life-threatening disease associated with pregnancy. There are limited data regarding the outcome of PPCM and its predictive factors in sub-Saharan African patients. We prospectively conducted a double-center (cardiology unit of the department of medicine, Regional Hospital Center of Tenkodogo, Burkina Faso and the department of cardiology of the National Referral Teaching Hospital of N´Djamena, Chad) cohort study in patients with PPCM. Patients were consecutively enrolled from January 2015 to December 2017. Outcomes of interest were le

Prevalence of postpartum complications and associated factors among postpartum women in Uganda, a cross-sectional study.

BACKGROUND: Postnatal care exhibits the lowest coverage levels in the obstetric continuum of care. The highest rates of maternal and newborn morbidity and mortality occur within 24 h of birth. Assessment of women in this time period could improve the detection of postpartum complications and maternal outcomes. This study determined the patterns of maternal assessment and the factors associated with postpartum complications. METHODS: This was a cross-sectional study involving observations of immediate postpartum care provided to women following uncomplicated vaginal births at three health faci

Implementation of a universal postpartum blood pressure monitoring program: feasibility and outcomes.

BACKGROUND: New-onset postpartum preeclampsia has a higher risk of maternal morbidity and mortality than preeclampsia with antepartum onset, underscoring the need for earlier identification of elevated blood pressure among patients with this condition. Given the decrease in healthcare engagement, which is typical of the postpartum period, new-onset postpartum hypertension often goes unrecognized. Currently, there are no recommendations for universal postpartum blood pressure surveillance in women without hypertensive disorders of pregnancy. With the shift to telemedicine because of the COVID-1

Trends, Predictors, and Outcomes of Cardiovascular Complications at Delivery Associated With Gestational Diabetes: A National Inpatient Sample Analysis (2004-2019).

Background Gestational diabetes (GD) is associated with increased risk of long-term cardiovascular complications. However, data on acute peripartum cardiovascular complications are not well established. Hence, we aimed to investigate the association of GD with acute cardiovascular outcomes at the time of delivery admission. Methods and Results We used data from the National Inpatient Sample (2004-2019). International Classification of Diseases, Ninth Revision (ICD-9) or Tenth Revision (ICD-10) codes were used to identify delivery hospitalizations and GD diagnosis. A total of 63 115 002 weighte

Thyroid Disease in Pregnancy: ACOG Practice Bulletin Summary, Number 223.

Both thyrotoxicosis and hypothyroidism are associated with adverse pregnancy outcomes. There also is concern about the effect of overt maternal thyroid disease on fetal development. In addition, medications that affect the maternal thyroid gland can cross the placenta and affect the fetal thyroid gland. This document reviews the thyroid-related pathophysiologic changes that occur during pregnancy and the effects of overt and subclinical maternal thyroid disease on maternal and fetal outcomes. This Practice Bulletin has been updated with information on the diagnosis and the management of thyroi

Peripartum cardiomyopathy: basic mechanisms and hope for new therapies.

Peripartum cardiomyopathy (PPCM) is a life-threatening cardiomyopathy characterized by acute or slow progression of left ventricular (LV) systolic dysfunction (LV ejection fraction of <45%) late in pregnancy, during delivery, or in the first postpartum months, in women with no other identifiable causes of heart failure. PPCM patients display variable phenotypes and risk factor profiles, pointing to involvement of multiple mechanisms in the pathogenesis of the disease. The higher risk for PPCM in women with African ancestry, the prevalence of gene variants associated with cardiomyopathies, and
