Antiphospholipid antibodies in sickle cell disease.

Antiphospholipid antibody formation can be induced in mice by phospholipid in a hexagonal II phase but not by phospholipid in a bilayer phase. Since sickle red cell membranes have increased hexagonal II phase content, we have measured serum antiphospholipid antibody levels in 25 patients with sickle cell disease to determine whether anti-phospholipid antibody may similarly be induced in these patients. Seventeen of the 25 patients (68%) had increased levels of antiphospholipid antibodies. Eleven patients (65%) had IgG and six each (35%) had IgM and IgA isotypes. Antiphosphatidylethanolamine, a

Babesiosis and sickle red blood cells: loss of deformability, altered osmotic fragility, and hypervesiculation.

Babesiosis in sickle cell disease (SCD) is marked by severe anemia but the underlying red blood cell (RBC) rheologic parameters remain largely undefined. Here, we describe altered RBC deformability from both primary (host RBC sickle hemoglobin mediated) and secondary changes (Babesia parasite infection mediated) to the RBC membrane using wild-type AA, sickle trait AS, and sickle SS RBCs. Our ektacytometry analysis demonstrates that the changes in the host RBC biomechanical properties, before and after Babesia infection, reside on a spectrum of severity, with wild-type infected AA cells, despit

Determinants of perception and willingness to uptake premarital screening test for sickle cell disease among health sciences undergraduate students in Dar es Salaam, Tanzania.

Sickle cell disease (SCD) is a significant genetic disorder that imposes a considerable global health burden. The notable prevalence of SCD in Tanzania, coupled with extensive economic, psychological, and social ramifications, underscores the importance of premarital genetic screening to carriers of the sickle cell trait. This study aimed to assess the determinants of perception and willingness to uptake premarital genotype screening test for sickle cell disease carriers (PMGS) among health sciences undergraduate students in Dar es Salaam, Tanzania. An analytical cross-sectional design was use…

Evolutionary history of sickle-cell mutation: implications for global genetic medicine.

Resistance afforded by the sickle-cell trait against severe malaria has led to high frequencies of the sickle-cell mutation [HBB; c.20T>A, p.Glu6Val; OMIM: 141900 (HBB-βS)] in most parts of Africa. High-coverage sequencing and genotype data have now confirmed the single African origin of the sickle-cell gene variant [HBB; c.20T>A, p.Glu6Val; OMIM: 141900 (HBB-βS)]. Nevertheless, the classical HBB-like genes cluster haplotypes remain a rich source of HBB-βS evolutionary information. The overlapping distribution of HBB-βS and other disease-associated variants means that their evolutionary geneti

Chloroquine prophylaxis associated with high prevalence of Plasmodium falciparum pfcrt K76T mutation in people with sickle-cell disease in Benin City, Nigeria.

BACKGROUND & OBJECTIVES: High mortality and morbidity in sickle-cell disease has been associated with malaria infection especially in countries where chloroquine is used. Chloroquine resistance has been associated with the emergence of Pfcrt mutant genes. This study aimed at comparing the prevalence rate of Pfcrt T76 mutation in Plasmodium falciparum isolates from infected individuals with sickle-cell disease and sickle-cell trait. This study was carried out in Benin City between the months of April and June 2006. This period is marked with high transmission rate of malaria. METHODS: The geno

Non-invasive detection of endothelial dysfunction in sickle cell disease by Doppler ultrasonography.

OBJECTIVE: The purpose of the study was to assess endothelial function in sickle cell disease (SCD), to compare endothelial dysfunction between sickle cell anemia (SS) and sickle cell trait (SA) cases and to evaluate correlation of endothelial dysfunction with duration of symptoms and vaso-occlusive crises per year (voc/year) [severity of disease]. METHODS: We investigated 37 steady state SCD cases, of which 19 were SS [mean age = 23.15 + 5.27 years and M/F = 10/9] and 18 were SA cases [Mean age = 22.05 +/- 5.17 years and M/F = 9/9]. Age, sex, and hemoglobin matched 33 controls [15 (Hb < or =…

Protein S deficiency in sickle cell anemia.

To investigate the status of the protein C-protein S anticoagulant pathway in sickle cell disease, we measured protein C, total and free protein S, and C4b-binding protein levels in 20 subjects with sickle cell disease (Hb SS or SC). Mean total and free protein S levels were both significantly lower in subjects with sickle cell disease than in normal individuals, but greater reductions were observed for free S. The free protein S level was below the mean -2 SD for normal subjects in 12 subjects with sickle cell disease; the total protein S level was below this level in three subjects. Mean C4b

Association between rs267196 and rs267201 of BMP6 gene and osteonecrosis among sickle cell aneamia patients.

AIMS: The skeletal manifestations of sickle cell disease are the result of changes in bone and bone marrow caused by chronic tissue hypoxia that is exacerbated by episodic occlusion of the microcirculation by the abnormal sickle cells. Furthermore, the occurrence of osteonecrosis is under the control of some modifier gene. BMP6 (Bone morphogenetic protein) has been reported as associated with osteonecrosis in sickle cell anemia (SCA). Herein, we intend to study the impact of rs267196, rs267201, rs408505 and rs449853 of BMP6 gene in the occurrence of osteonecrosis among sickle cell patients in

Gene expression profiling of renal medullary carcinoma: potential clinical relevance.

BACKGROUND: Renal medullary carcinoma is a rare kidney tumor with highly aggressive behavior. This tumor occurs exclusively in young patients with sickle cell trait or disease. To the authors' knowledge, very little is known to date regarding the underlying molecular genetics of this tumor, and no effective therapy has been established. METHODS: The authors analyzed the gene expression profiles of 2 renal medullary carcinomas from patients with sickle cell trait using microarrays containing 21,632 cyclic DNA (cDNA) clones and compared them with the gene expression profiles of 64 renal tumors.…

A Multifactorial Presentation of Infection and Thrombosis: Temporal Clustering of Pneumonia, Hepatic Abscess, Bilateral Deep Vein Thromboses (DVTs), and Appendicitis in a Child.

Multisystem infectious and thrombotic presentations are rare in pediatric patients and often raise concern for an underlying immunologic or hematologic disorder. We report a case of a 13-year-old male with sickle cell trait and beta-thalassemia trait heterozygote who developed extensive bilateral deep vein thromboses (DVTs), a large hepatic abscess, right-sided pneumonia with empyema, and subsequently perforated appendicitis with intra-abdominal abscesses. The patient required broad-spectrum antimicrobial therapy, anticoagulation, percutaneous abscess drainage, and emergent surgical interventi…

A Triad of Hematologic Diseases in a Military Member Requiring Splenectomy Case Report.

Hereditary spherocytosis and sickle cell disease are two distinct hematologic diseases that affect the morphology of the red blood cell, causing implications related to splenic sequestration. Concurrent disease is very rare, with even fewer reported instances requiring surgical intervention. Furthermore, an individual with a third concomitant hematologic abnormality such as Factor VII deficiency, which degrades the normal clotting cascade, has not previously been reported. We describe an active-duty servicemember with known hereditary spherocytosis, sickle cell trait, and Factor VII deficiency…

[Adhesion of diabetic or sickle cell erythrocyte populations to human endothelium in culture].

Erythrocyte adhesion to endothelium was measured using human endothelial cells in culture and a radiometric technique. Erythrocyte adhesion was found to be significantly increased in diabetes mellitus and sickle cell anemia. In both diseases the extent of adhesion was correlated with the clinical severity of the disease. Using 3H Leucine radio-labelled reticulocytes or red cells separated by density gradient according to their age it was possible to further investigate the red cell abnormality responsible for increased adhesion. A population of abnormal reticulocytes in sickle cell anemia exhi

Hemolysis in sickle cell disease as measured by endogenous carbon monoxide production. A preliminary report.

To detect and quantitate temporal variations of the hemolytic rate in sickle cell disease, the authors measured endogenous carbon monoxide (CO) production in five normal subjects, nine patients with sickle cell anemia (SS) in steady clinical state, and two patients with sickle cell-hemoglobin C (SC) disease in and out of pain crises. The red blood cell life span calculated from these data (RCLSco) ranged from 81.2 to 102.9 days (mean +/- standard deviation [SD] 88.0 +/- 9.2, coefficient of variation [CV] 10.2%) for the normal subjects and 8.0-24.7 days (mean +/- SD 12.1 +/- 5.1, CV 42.1%) for
