OBJECTIVE: To investigate the effects of sleep apnea (SA) on the quality of life (QOL).
DESIGN: A prospective study of QOL in patients with and without SA as defined by an apnea-hypopnea index (AHI) >5.
SETTING: University-based outpatient clinics.
PATIENTS: Primary care patients followed in a general internal medicine clinic as well as those referred to a sleep disorders clinic at the University of Wisconsin Hospital and Clinics were consecutively recruited and classified into 3 groups of subjects: (1) patients without SA (AHI<5) (n=46), (2) patients with mild SA (AHI 5-15) (n=16), and (3)…

The interactions between sleep and epilepsy are well known. A nodal point of the relationship between sleep and epilepsy is represented by pharmacological treatment. Sleep disturbances such as drowsiness are among the most frequent side effects of treatment with antiepileptic drugs, since they can deeply modify both sleep architecture and the sleep-wake cycle. Severe daytime somnolence affects patients' activities and it may facilitate the occurrence of seizures. These considerations underline the importance of antiepileptic drugs having anticonvulsant properties that do not negatively influen…

Sleep disordered breathing (SDB), i.e., obstructive, central or mixed sleep apneas, has been recognized as a common occurrence in the elderly. Aging is per se associated with a decrease in the quality of sleep; SDB may further disrupt the sleep architecture in older subjects. The prevalence of obstructive sleep apnea (OSA) increases with aging; available studies report prevalence rates of 11-62%. Furthermore, OSA has been associated with increased mortality in older adults. Central apneas and periodic breathing occur with increased frequency either in subjects with neurological disorders such …

Obstructive sleep apnoea was first brought to prominence by Henri Gastaut, a French epileptologist. Since that time the interface between epilepsy and sleep disorders has received less attention than might be justified, recognizing that sleep deprivation is a poignant provocateur for seizures. Sleep deprivation is often used as a diagnostic procedure during electroencephalography (EEG) when waking EEG has failed to demonstrate abnormality. Patients referred to an outpatient neurological clinic for evaluation of possible seizures in whom sleep disorder was suspected, either due to snoring durin…

The purpose of this study was to determine, in a large sample of adults of all ages (17-80 years), the effect of morningness/eveningness on sleep/wake schedules, sleep needs, sleep hygiene and subjective daytime somnolence. A total of 617 subjects (219 subjects per chronotype group) matched for age, sex and employment status, completed an abridged morningness/eveningness questionnaire, a questionnaire on sleep habits and the quality of sleep, and the Epworth Sleepiness Scale. Eveningness was associated with a greater need for sleep, less time in bed during the week compared to ideal sleep need…

The present article reviews direct and indirect evidence of the effects of sex steroids on different aspects of sleep. It begins with a review of what is known about the effects of steroid hormones on sleep and on central nervous system processes related to sleep, such as the GABA-ergic system, in animals. It continues with a review of the effects of exogenous hormones on human sleep and a review of studies comparing sleep during hypogonadal states secondary to surgical or natural menopause. The article proceeds to review the data on the effects of the menstrual cycle on both subjective and ob…

Overnight sleep studies were performed in 20 subjects with achondroplasia to document further the respiratory abnormalities present in this group. Somatosensory evoked potentials (SEPs) were recorded in 19 of the subjects to screen for the presence of brainstem abnormalities, which are one of the potential aetiological mechanisms. Fifteen children aged 1 to 14 years, and five young adults, aged 20 to 31 years were included. All had upper airway obstruction and 15 (75%) had a pathological apnoea index (greater than five per hour). Other sleep associated respiratory abnormalities, including part…

The discovery in 1953 of rapid eye movement (REM) sleep and the appreciation that sleep is a heterogeneous physiological state stimulated major research into sleep disorders. Electroencephalographic studies have shown that the amount of REM sleep changes with age. While newborns spend almost 50% of their sleep time in REM, the percentage of REM sleep decreases to 30% by the age of 3 months and to 20% by the age of 6 months. In addition, newborns enter REM sleep soon after the initiation of sleep, but by the age of 4 months entry into sleep assumes the adult pattern in which a significant perio…

Sleep disorders and disturbances are common problems in children and adolescents and frequently occur concurrently with psychiatric symptomatology. There appears to be a complex relationship between the regulation of sleep and emotional and behavioral problems in children and adolescents. This article focuses on appropriate use of medications in the treatment of these common disorders through a comprehensive approach of careful evaluation, diagnosis, and the use of behavioral and other nonpharmacologic treatments.

In the United States, non-A, non-B hepatitis accounts for 20-40% of acute viral hepatitis. Although it has traditionally been considered a transfusion-associated disease, non-A, non-B hepatitis is more likely to occur outside the transfusion setting. Surveillance data from the Centers for Disease Control show that in 1988 6% of patients with non-A, non-B hepatitis reported a history of blood transfusion, 46% parenteral drug use, 10% household or sexual exposure to a contact who had had hepatitis or exposure to multiple sex partners, 2% medical or dental employment involving frequent blood cont

While there are many research questions still requiring performance of research sleep studies in patients with COPD, their use is not advocated in routine clinical practice, except in patients who have symptoms of the sleep apnoea/hypopnoea syndrome or possibly in those without daytime hypoxaemia (PaO2 greater than 60 mmHg) who have marked polycythaemia or marked cor pulmonale.

The sleep apnoea syndrome (SAS) is characterised by somatic, in particular cardiopulmonary and psychosocial, symptoms, the latter severely impairing the patient's social life. Excessive daytime somnolence and the resulting problems severely stress patients with SAS. Among 24 patients with SAS, 79.2% stated that they fell asleep during the day, frequently in 41.8% and occasionally in 37.4%. In addition, 70.8% of the patients with SA did not feel adequately rested on waking in the morning. Of the 24 patients, 17 had a job, and 11 of these experienced difficulty staying awake at the workplace. Su…
