Mental disorders in childhood and young adulthood among children born to women with fertility problems.

STUDY QUESTION: Is the risk of hospital admission or outpatient contact for mental disorders increased in children born to women with fertil … e risk of several severe mental disorders not previously studied (e.g. neurotic, stress-related and somatoform disorders and disorders of adult personality and behaviour). These important findings should be investigated further in large epidemiological studies designed to differentiate between factors related to fertility treatment and to the underlying infertility. …

Longitudinal association of mental-health problems with subsequent diagnosis of persistent somatic symptom disorders: A large-scale registry-based observational study in primary care.

BACKGROUND: Somatic symptoms of common persistent somatic symptom (PSS) syndromes like irritable bowel syndrome (IBS), chronic fatigue syndrome (CFS), and fibromyalgia (FM) are not fully attributed to well-established biomedical pathological processes. These syndromes are often under-recognized and diagnosis is often delayed. This study assessed the extent to which mental health problems precede the onset of IBS, CFS, and FM diagnoses using large-scale registry-based data from primary care settings. METHOD: Data from 11,409 patients were anonymously extracted from primary care data-bases in t

High Somatization Rates, Frequent Spontaneous Recovery, and a Lack of Organic Biomarkers in Post-Covid-19 Condition.

INTRODUCTION: Many patients report neuropsychiatric symptoms after SARS-CoV-2 infection. Data on prevalence of post-COVID-19 condition (PCC) vary due to the lack of specific diagnostic criteria, the report of unspecific symptoms, and reliable biomarkers. METHODS: PCC patients seen in a neurological outpatient department were followed for up to 18 months. Neurological examination, SARS-CoV-2 antibodies, Epstein-Barr virus antibodies, and cortisol levels as possible biomarkers, questionnaires to evaluate neuropsychiatric symptoms and somatization (Patient Health Questionnaires D [PHQ-D]), cogni

Psychological and physical factors involved in chronic idiopathic prostatitis.

Idiopathic prostatitis is a common, often chronic condition in which psychological factors are suspected to play a role. Men with chronic prostatitis (n = 51) and a control group of 34 men without any chronic pain condition, equivalent on demographic characteristics, were compared on psychological and perineal muscle tension measures. Prostate-specific antigen and expressed prostatic secretion cell counts were also measured. Chronic prostatitis patients were consistently more elevated than controls on hypochondriasis, depression, and hysteria (MMPI), and on somaticization and depression (Brief

Similar but Different: Psychological and Psychopathological Features of Primary and Secondary Hikikomori.

Recently, there has been an increase in reports of hikikomori around the globe, and Ukraine is not an exception. The development of hikikomori is often spurred by a history of aversive or traumatic childhood experience, for example, dysfunctions between parents or between a parent and a child (ambivalent attachment) and difficulties at school (peer rejection). Previously described models of hikikomori development mostly were based on research of mixed cohorts of patients (with and without psychiatric comorbidity). To test whether there was a difference in psychological and psychopathological f

Vestibular migraine in children and adolescents: clinical findings and laboratory tests.

INTRODUCTION: Vestibular migraine (VM) is the most common cause of episodic vertigo in children. We summarize the clinical findings and laboratory test results in a cohort of children and adolescents with VM. We discuss the limitations of current classification criteria for dizzy children. METHODS: A retrospective chart analysis was performed on 118 children with migraine related vertigo at a tertiary care center. Patients were grouped in the following categories: (1) definite vestibular migraine (dVM); (2) probable vestibular migraine (pVM); (3) suspected vestibular migraine (sVM); (4) benig

Long-term course and relapses of vestibular and balance disorders.

The long-term course and the frequency of relapses for various peripheral vestibular disorders and somatoform phobic postural vertigo are discussed with respect to the clinically most important questions for thus afflicted patients. This review is mainly based on our own long-term follow-up studies and takes into consideration the most relevant literature. The following syndromes are discussed in detail. Vestibular neuritis: the recovery rate of peripheral vestibular function lies between 40-63% depending on early-onset treatment with corticosteroids; the recurrence rate within 10 years is 2%.

Prevalence and risk factors for depression in somatic symptom disorder patients: a cross-sectional clinical study in China.

INTRODUCTION AND OBJECTIVES: The comorbidity of somatic symptom disorder (SSD) and depressive symptoms is common; however, the demographic and clinical factors correlated with depression among SSD patients remain unclear. The purpose of this study was to explore the prevalence and associated factors of depressive symptoms in Chinese Han patients with SSD. METHODS: In this cross-sectional study, 899 outpatients diagnosed with SSD were included. Demographic data were collected, and clinical assessments were conducted, which included blood pressure measurements and laboratory tests for thyroid-st

Impact of hypnotic safety on disorders of gut-brain interaction: A pilot study.

Functional gastrointestinal disorders, i.e., abdominal conditions without identifiable structural etiologies, are seen frequently in primary care and specialty practices. As subtle physiological processes have been identified as potential contributing factors to these functional disorders, these disorders have been recently relabeled, Disorders of Gut-Brain Interaction (DGBI). Moreover, some of these processes, e.g., sympathetic nervous system activity and inflammation, are being increasingly related to psychosocial factors such as situational stress and histories of trauma, abuse, and neglect

Symptom Preoccupation in Fibromyalgia: Prevalence and Correlates of Somatic Symptom Disorder in a Self-Recruited Sample.

BACKGROUND: Somatic symptom disorder (SSD) is characterized by a persistent and distressing psychological reaction to somatic symptoms. In pain disorders, the preoccupation with physical symptoms is associated with poor long-term outcomes. SSD may therefore be of use to identify and help chronic pain patients with particular needs. OBJECTIVE: To test the hypothesis that in fibromyalgia, SSD is associated with higher anxiety sensitivity, health anxiety, and reactivity to pain, in addition to lower nonreactivity to inner experiences. In addition, to investigate if individuals with SSD report a

Unexplained chronic cytopenia: is it idiopathic cytopenia of undetermined significance or myelodysplastic syndrome.

Idiopathic cytopenia is a condition where there is a decrease in peripheral blood counts causing either anaemia, leucopoenia and thrombocytopaenia. Most cases of cytopenia reveal a cause on further workup. But very rarely, in some cases, a definitive cause could not be identified. Unexplained cytopenia becomes challenging and poses difficulty in diagnosis and management. Discriminating these groups of bone marrow failure disorders from myelodysplastic syndrome (MDS) becomes an important clinical question. We describe a case of a middle-aged Hispanic woman who presented with pancytopenia and on

Physicians' gender bias in the diagnostic assessment of medically unexplained symptoms and its effect on patient-physician relations.

Nonspecific, functional, and somatoform (NFS) syndromes is an umbrella term for various diagnoses with medically unexplained symptoms. These syndromes are more prevalent among women than among men, and associated with negative preconceptions that can impede rehabilitation. In two studies, we quantitatively assess how patients' gender affects the diagnostic assessment of NFS syndromes, as well as the healthcare experiences of individuals diagnosed with NFS syndromes. In the first study, our vignette-based experiment showed that Swedish general practitioners (N = 90) were gender biased in their

Anatomical distance affects cortical-subcortical connectivity in first-episode, drug-naive somatization disorder.

BACKGROUND: Brain structural and functional alterations in the cortical-subcortical circuits have been observed in somatization disorder (SD). However, whether and how anatomical distance affects the cortical-subcortical connectivity in SD remain unclear. This study aims to examine whether anatomical distance affects the cortical-subcortical in first-episode, drug-naive SD. METHODS: Twenty-five first-episode, drug-naive patients with SD and twenty-eight healthy controls were recruited for a resting-state scan. Regional functional connectivity strength (FCS) was calculated for each voxel in th

Psychological distress longitudinally mediates the effect of vertigo symptoms on vertigo-related handicap.

OBJECTIVE: Vertigo symptoms can lead to more or less vertigo-related handicap. This longitudinal study investigated whether depression, anxiety, and/or somatization mediate the relationship between vertigo symptoms and vertigo-related handicap. METHODS: N=111 patients with vertigo/dizziness provided complete data on the following measures: Vertigo symptoms at baseline, depression at 6-month follow-up, anxiety at 6-month follow-up, somatization at 6-month follow-up, and vertigo handicap at 12-month follow-up. Mediation analyses with bootstrapping were performed to investigate the mediating rol
