Endoscopic Ultrasound-Guided Variceal Therapy for Gastric Variceal Rupture After Failed Endoscopic Injection Sclerotherapy: A Case Report.

Endoscopic ultrasound (EUS) has become an essential tool in vascular interventions due to its imaging potential in the proximity of various vascular structures in the mediastinum and abdominal cavity. EUS-guided variceal therapy (EUS-VT) has emerged as a promising approach for treating gastric varices (GV). Although still underused, EUS-VT offers such advantages as precise targeting and real-time variceal puncture visualization, making it a valuable alternative when conventional therapies fail. We report the case of a 63-year-old man with GVs secondary to fatty liver cirrhosis, who experienced

Ovarian hemorrhage as the primary cause of hemoperitoneum in cycling and pregnant Miniature Horse mares.

OBJECTIVE: To determine whether Miniature Horse mares are predisposed to hemoperitoneum from nonneoplastic ovarian hemorrhage and report the clinical characteristics of this disorder. ANIMALS: 51 equines with hemoperitoneum, including 22 nonminiature mares and 9 Miniature Horse mares, identified by searching medical records of a tertiary large animal hospital for confirmed hemoperitoneum cases between 2012 and 2023. CLINICAL PRESENTATION: Signs of hemoperitoneum in miniature mares included lethargy, inappetence, colic, tachycardia, and moderate to severe anemia (PCV ranging from 11% to 19%).

Hepatic artery aneurysm associated with prune belly syndrome: a case report and review of the literature.

Potentially life threatening when they rupture, hepatic artery aneurysms (HAAs) are uncommon and diagnosed at the fifth to sixth decade of life. HAAs are the second most common splanchnic aneurysms after the ones that involve the splenic artery and are more predominant in men than in women with a ratio of 3 to 2. Usually, HAAs are associated with inflammation or traumas to the arteries of the liver. Among the former group, atherosclerosis, arthritis, and collagen vascular disease are the most common, whereas liver biopsy or other radiologic interventions (ie, percutaneous abscess drainage) are

Acute abdomen due to torsion of a pelvic wandering spleen.

Wandering spleen is a rare entity characterized by incomplete fixation of the spleen by lienorenal and gastrosplenic ligaments. Wandering sp … g spleen and microscopic isolated pancreatic tissue (disconnected from the pancreas) with impending splenic rupture in a 23-year-old female patient. Progressively severe chronic or intermittent torsion of the vascular pedicle of the wandering spleen caused progressive intermittent lower abdominal pain. The patient underwent splenectomy with resection of the long …

Spontaneous massive gastrointestinal hemorrhage in patients with pancreatitis.

Spontaneous massive gastrointestinal hemorrhage occurring in patients with pancreatitis is an uncommon and difficult clinical problem, especially in a tropical third world country where the underlying causes of pancreatitis and facilities for its diagnosis and management are different from those in the developed West. Of 7 such cases seen by us in 3 years, 2 were due to hemosuccus pancreaticus, 2 were due to gastric variceal hemorrhage consequent to splenic vein obstruction, and one each due to contiguous pancreatic inflammation involving the duodenum, an aneurysmal bleed from the left gastric

[Diagnosis and management of suspected nephrolithiasis in a primary care setting].

Based on the prevalence of asymptomatic kidney stones (5% in our general ward, in accordance with the literature) the value of abdominal ultrasonography in the clinical assessment of a suspected kidney-colic is discussed. The eminent importance of the stone-analysis is emphasized. In addition, the most common causes of kidney stone formation (low urine output, mechanical urinary obstruction in the renal pelvis, hypercalciuria, hyperoxaluria, insufficient urinary citric acid excretion, hyperuricosuria) are highlighted. The cardinal symptom of the urolithiasis is the presence of micro/macrohemat

[The 506th case: fever of unknown origin, negative PET-CT and hemoperitoneum].

A 48-year-old male was admitted to Peking Union Medical College Hospital presented with intermittent fever for two years. The maximum body temperature was 39 ℃, and could spontaneously relieve. The efficacy of antibacterial treatment was poor. He had no other symptoms and positive signs. He had a significant weight loss, and the serum lactate dehydrogenase increased significantly. It was highly alert to be lymphoma, but bone marrow smear and pathology, and PET-CT had not shown obvious abnormalities. Considering high inflammatory indicators, increased ferritin and large spleen, the patient had

Splenic tear causing a hemoperitoneum after cardiac surgery.

Hemoperitoneum after cardiac surgery is a very rare but life-threatening complication. We have only found a few cases described in the literature, in which the intra-abdominal hemorrhages were caused by liver bleeding, due to direct hepatic trauma or spontaneous hepatic rupture. We describe the first case of hemoperitoneum caused by a spontaneous rupture of the spleen.

Gastrocolic vein. Definition and report of two cases of avulsion.

The gastrocolic v. or Henle's gastrocolic trunk was described in 1868 [9]. We suggest defining this vein as the confluence of the right gastroepiploic and right upper colic vv. We report two original cases of avulsion of the gastrocolic v. occurring during a blunt abdominal trauma. The aim of this paper is a description, based on the literature, of the anatomy of the gastrocolic v. in order to precise the lesional mechanism. The gastrocolic v. is present in 70% of individuals. It is short (less than 25 mm) but of major calibre (3 to 10 mm). The gastrocolic v. is situated close beneath the root

Infectious mononucleosis.

Infectious mononucleosis is a unique disease in its hematologic aspects; it is different from the frequently occurring acute microbial diseases in that it affects primarily the reticuloendothelial system; and it is interesting serologically because of the heterophil antibody reaction, as well as the multiplicity of antibodies which may be produced. The diagnosis should be suspected clinically before hematology is reported - by remembering the prototypes. In fact, a patient between 16 and 25 years old who complains of sore throat and fever is more likely to have infectious mononucleosis than an

Ruptured Sinus of Valsalva Aneurysm Associated with Situs Ambiguus, Isolated Levocardia, and Polysplenia.

Aneurysm of the sinus of Valsalva, a rare cardiac condition, results from dilation of an aortic sinus. Sudden aneurysm rupture can trigger rapidly progressive heart failure. We discuss the case of a 57-year-old woman with situs ambiguus, isolated levocardia, and polysplenia who presented with acute-onset heart failure. Transesophageal echocardiograms revealed an aneurysm of the right coronary sinus of Valsalva that had ruptured into the right atrial cavity. The patient underwent successful surgical repair. To our knowledge, this is the first report of a sinus of Valsalva aneurysm in a patient

Injuries in children associated with the use of nonmotorized scooters.

BACKGROUND/PURPOSE: Since 2000, a substantial increase in injuries related to nonmotorized scooters (kickboards) has been observed among children. The authors conducted a population-based prospective study to characterize these injuries in comparison with skateboard-related injuries during the same period. METHODS: All children (age <16 years) presenting with a scooter-related injury between July and October 2000 in Graz (Austria) and all children presenting with a scooter- or skateboard-related injury between June and September 2001 in Helsinki, Finland were interviewed at the time of seekin
