Syndesmotic Injuries.

Syndesmotic ankle injuries, though rare in isolation, are complex destabilizing injuries often accompanied by fractures. Misdiagnoses, particularly overlooking posterior malleolus fractures, are common in ankle sprains. Thorough physical examinations, emphasizing high fibular pain and anterior tibia palpation, aid in accurate diagnosis. Grading helps assess injury severity and guiding treatment. Initial imaging involves three ankle views, with stress radiographs enhancing accuracy. If conservative care fails, MRI reveals ligament and tendon damage. Physical therapy may suffice for functional i

Biceps and Triceps Ruptures in Athletes.

Although rare, biceps and triceps tendon ruptures constitute significant injuries that can lead to profound disability if left untreated, especially in the athletic population. Biceps rupture is more common than triceps rupture, with both resulting from a forceful eccentric load. Surgical repair is the treatment method of choice for tendinous ruptures in athletes. Nonoperative management is rarely indicated in this population and is typically reserved for individuals with partial ruptures that quickly regain strength and function. Surgical anatomy, evaluation, diagnosis, and surgical managemen

High Median Nerve Injuries.

The median nerve serves a crucial role in extrinsic and intrinsic motor and sensory function to the radial half of the hand. High median nerve injuries, defined as injuries proximal to the anterior interosseous nerve origin, therefore typically result in significant functional loss prompting aggressive surgical management. Even with appropriate recognition and contemporary nerve reconstruction, however, motor and sensory recovery may be inadequate. With isolated persistent high median nerve palsies, a variety of available tendon transfers can improve key motor functions and salvage acceptable

Fibular head-based posterolateral reconstruction of the knee combined with capsular shift procedure.

Posterolateral instability (PLI) is common with posterior cruciate ligament tears, is less common with anterior cruciate ligament tears, and isolated PLI is rare. There are varying degrees of PLI with respect to pathologic external tibial rotation and varus laxity. Surgical treatment of PLI must address all components of the PLI (popliteus tendon, popliteofibular ligament, lateral collateral ligament, and the lateral-posterolateral capsule), the abnormal planes of motion, as well as other structural injuries. Successful posterior cruciate ligament and anterior cruciate ligament surgery depends

Avoiding and treating perioperative complications of distal radius fractures.

Numerous methods of treatment are available for the management of distal radius fractures, with modern trends favoring volar fixed-angle distal radius plates. Whatever the method of fixation, recognition, management, and prevention of the known associated complications are essential to achieve a good outcome. This article reviews the common preventable complications that are associated with operative treatment of distal radius fractures, including tendon injuries, inadequate reduction, subsidence or collapse, intra-articular placement of pegs or screws, nerve injuries, complex regional pain sy…

Orthopedic pitfalls in the ED: fight bite.

Clenched fist injuries to the mouth ("fight bite") are notorious for being the worst human bites. These are often treated as minor injuries, without the recognition that the joint capsule, the extensor tendon, or the deep fascial spaces may have been violated and contaminated with oral bacteria. Significant morbidity can result from late presentation or inadequate initial management. The emergency physician needs to remain vigilant for complications associated with closed fist injury. This review article examines the clinical presentation, diagnostic techniques, and management options applicab

[Magnetic resonance tomography (MRI) of ligament injuries of the upper ankle joint].

This paper discusses MR imaging in injuries to the lateral collateral ankle ligaments. Based on recent publications and our own results MR imaging may be useful for demonstration of ankle ligament tears. However, clinical outcome can probably not be predicted by such diagnosis. MR imaging may still be useful in certain settings, such as suspected additional trauma to medial ligaments, bone, or articular cartilage. In chronic pain MR imaging may be indicated for demonstration of articular bodies, occult fractures, significant cartilage damage, tendon injuries and dislocations, and soft tissue s…

Early mobilisation of acute middle slip injuries.

After operative repair, the conventional management of open injuries to the middle slip of the extensor tendon involves prolonged immobilisation of the P.I.P. joint. We report our results using controlled mobilisation with a Capener splint after two weeks rigid splintage. In a series of 99 patients seen soon after injury, 88% had excellent or good results after six months. Poor results were more common in patients with complex joint injuries and in patients who did not comply with the treatment. We advocate early mobilisation following repair of the middle slip.

Spontaneous rupture of the anterior tibial tendon in a diabetic patient: results of operative treatment.

UNLABELLED: Spontaneous rupture of the tibialis anterior tendon is infrequently seen as a clinical entity. In this report, we describe the case of a diabetic neuropathic patient that underwent successful surgical repair of a spontaneously ruptured tibialis anterior tendon with no other factors that would render the tendon susceptible to injury or rupture. LEVEL OF CLINICAL EVIDENCE: 4.

Acute true trigger wrist following partial extensor carpi radialis brevis tendon rupture.

Triggering in association with movements of the wrist or "true trigger wrist" due to the extensor tendon is rare. There are only few case reports in literature, but none were associated with the acute partial tendon rupture. We present a case of true trigger wrist originating from partial rupture of extensor carpi radialis brevis tendon (ECRB). In contrast to the other reports, the interval between an initial injury and development of triggering was short because the partial tendon rupture was bunching and forming a nodule. The triggering was attributed to the snapping of the nodule under the

Anatomy of the greater trochanteric 'bald spot': a potential portal for abductor sparing femoral nailing?

Soft tissue injury occurs when using a piriformis portal for femoral nailing. Standard trochanteric portals also can injure the gluteus medius and external rotator tendons, which may be a source of hip pain after nailing. On the lateral facet of the greater trochanter, a "bald spot" may exist that is devoid of tendon insertion. This may be a potential portal for intramedullary nail insertion. We defined the dimensions and location of this region. Cadaveric specimens were dissected to expose the tendon insertions on the greater trochanter. A computer navigation system was used with a stylus and

Predictors of Tunnel Widening After Anterior Cruciate Ligament Reconstruction.

BACKGROUND: Posterior tibial slope (PTS) has been implicated in tunnel widening after anterior cruciate ligament reconstruction (ACLR), yet its precise effect remains unclear. Additionally, the influence of lateral extra-articular tenodesis (LET) and meniscus root injuries on tunnel widening has not been well established. PURPOSE: To evaluate the effect of medial and lateral PTS, LET, and meniscus root injuries on tibial and femoral tunnel widening after ACL reconstruction using a hamstring tendon graft. STUDY DESIGN: Cohort study; Level of evidence, 3. METHODS: A total of 307 patients who

Fracture dislocation of the ankle with posterior tibial tendon entrapment within the tibiofibular interosseous space: A case report.

In rare cases of ankle fracture dislocation, the posterior tibial muscle tendon (TP tendon) is incarcerated between the tibia and fibula, thereby impeding reduction. Here we describe a case that presented with such a condition, in which ankle reduction was achieved and surgical repair of the incarcerated TP was delayed. The subject was a 30-year-old male who sustained a fracture dislocation of the left ankle (AO:44-C1.3) in a motorbike accident. After repairing the ankle dislocation, external fixation was performed and osteosynthesis was conducted 10 days after the injury. Plate fixation for t

Isolated rupture of the patellar tendon in athletes.

The purpose of this study was to review patellar tendon ruptures in athletes treated with surgical repair. Six patellar tendon ruptures in five athletic patients were identified at our institution between 1980 and 1990. All patients had operative debridement and primary repair of the ruptured patellar tendon using nonabsorbable sutures passed through patellar drill holes at an average of 2.5 days after injury. Personal interview, physical examination, roentgenographic evaluation, and isokinetic strength testing were performed at an average of 6 years after surgery. At final followup, no patien

A Neglected Extensor Hallucis Longus Tendon Rupture Caused by Arthritic Adhesion.

Extensor hallucis longus tendon injury is relatively rare and is principally caused by a laceration when a sharp object is dropped on the instep. Primary suturing is possible if the injury is acute, but if the tear is chronic, tendon contracture causes the space between the edges of the tear to widen, disrupting the end-to-end connection. In particular, a claw toe or checkrein foot deformity may develop over time due to adhesion of the lower leg tendons near the fracture site or scar. We report on a 44-year-old man who visited our outpatient clinic complaining of pain in the right foot and a h

The Surgical Burden of Animal Bites Referred to Orthopaedics: A 10-Year Cohort Study of 168 Patients From a UK Major Trauma Centre.

Introduction Animal bites impose a substantial, yet under-recognised, burden on orthopaedic services. We characterised the epidemiology, injury patterns, management, and short-term outcomes of bite injuries requiring orthopaedic input at a UK Major Trauma Centre over 10 years. Methods We performed a retrospective cohort study of consecutive patients referred to orthopaedics with animal-bite injuries between December 2015 and March 2025. Eligible cases were identified from a prospectively maintained trauma database and verified against electronic records. Inclusion encompassed mammalian, reptil

Irreducible Anteromedial Dislocation of Radial Head with Biceps Tendon Interposition.

The case presents an isolated irreducible anteromedial dislocation of radial head due to biceps tendon interposition on a 14-year-old female patient. After an unsuccessful closed reduction, a lateral approach of the left elbow was carried out through Kocher's interval. Given that no pathology was found on the radiohumeral joint, the approach was extended distally. This revealed that the biceps tendon was displaced laterally around the radial neck, preventing the reduction. Once the tendon was taken back to its anatomical position, the radial head reduction was performed successfully. The patie

Patellar ligament rupture in an adolescent.

Rupture of the patellar tendon is a well-known injury in the orthopaedic literature. However, it is unusual and rarely reported in adolescent children. On the one hand, in the immature skeleton, the most frequent lesion above the kneecap is the sleeve fracture. On the other hand, in the distal region, avulsion of the tibial tuberosity is more common. Patellar tendon rupture in an adolescent is a rare lesion. We report a case in which an adolescent sustained a fall when jumping. No predisposing factors have been found. The injury was treated with surgical repair with transosseous suturing and r

Elbow injuries in athletes. A review.

Elbow injuries are becoming more common as increasing numbers of people participate in throwing and racquet sports. The understanding and treatment of elbow injuries is becoming more sophisticated in conjunction with better noninvasive and invasive diagnostic techniques. The majority of injuries to the elbow in the athlete are chronic, overuse injuries. These injuries are the result of repetitive intrinsic or extrinsic overload, or both, resulting in microrupture of soft tissue such as ligament or tendon. In children, apophyses, being the weakest link in the immature musculoskeletal system, ar
