IMPORTANCE: This is the first comprehensive comparison between ab interno approach and ab externo approach for microcatheter-assisted trabeculotomy in primary congenital glaucoma with clear cornea.
BACKGROUND: Ab externo microcatheter-assisted trabeculotomy is considered to be the gold standard for primary congenital glaucoma. The novel ab interno approach would intuitively be similar to ab externo approach for microcatheter-assisted trabeculotomy in eyes with clear cornea; however, there is no evidence yet.

PURPOSE: To compare circumferential trabeculotomy performed with an illuminated microcatheter with standard goniotomy for the treatment of congenital glaucoma.

Selective laser trabeculoplasty is an effective means of lowering intraocular pressure in patients with open-angle glaucoma with a low risk of complications. The authors report a case of hyphema noted 3 days after selective laser trabeculoplasty in a 77-year-old woman with primary open-angle glaucoma. Her intermittent use of oral nonsteroidal anti-inflammatory medications and chronic topical nonsteroidal anti-inflammatory use may have been a contributing risk factor.

Recent advances in ophthalmic microsurgical instrumentation and techniques make ab-externo trabeculotomy a more attractive alternative to goniotomy in the treatment of congenital glaucoma than was the case in the past. A series of seven consecutive cases (14 eyes) treated by trabeculotomy is presented, with the results of follow-up over a period of two to seven years. This, and other series, suggests that control of developmental glaucoma may be achieved utilizing fewer procedures when trabeculotomy is employed rather than goniotomy.

Arthrogryposis multiplex congenita is a birth defect characterized by multiple joint deformities and sometimes associated with various other congenital anomalies. There have been several reported cases of miscellaneous ocular abnormalities in conjunction with this syndrome. We report the first case of congenital ophthalmoplegia as well as juvenile onset glaucoma associated with arthrogryposis multiplex congenita. This case represented a unique therapeutic dilemma in that it was extremely difficult to surgically treat the patient's glaucoma because of her severe restrictions of ocular motility.

This study measured the strain response to intraocular pressure (IOP) change in the sclera, choroid, and retina of glaucoma patients whose optic nerve head region was imaged by optical coherence tomography (OCT) before and after IOP-lowering by laser suturelysis following trabeculectomy surgery.
