Partial cricoid resection with primary tracheal anastomosis for subglottic stenosis in infants and children.

Fifteen infants and children with a severe subglottic stenosis underwent a partial cricoid resection with primary tracheal anastomosis. The etiology of the stenosis was congenital in 3 cases and was acquired after prolonged intubation in 12. Fourteen patients were tracheotomy dependent at the time of surgery, and 13 (87%) of the 15 were classified grade III (7 cases) and IV (6 cases) according to Cotton. The resection included the cricoid only in 5 cases and the cricoid and a segment of trachea (1 to 4 rings) in 10 cases. Decannulation was achieved after a single open procedure in 14 (93%) of …

Muscular high-energy phosphates and red-cell 2,3-DPG in post-traumatic hypophosphataemia. An experimental study in pigs.

The relationship between post-traumatic hypophosphataemia, muscular high-energy phosphates (HEP) and red-cell 2,3-diphosphoglycerate (2,3-DPG) were studied in an experimental trauma model. Twenty-three anaesthetized pigs were submitted to trauma and observed for 72 hours. In group I (n = 10) and group II (n = 6) major trauma was inflicted with a high-energy missile. No phosphate supplement was given in group I, but group II pigs received 20 mmol phosphate/24 hours parenterally. Group III (n = 7) sustained only minor trauma, consisting of tracheotomy and catheterization of artery, vein and blad…

[Use of the Montgomery prosthesis in the treatment and prevention of anterior commissural synechiae secondary to bilateral cordectomy].

The authors report the results obtained with the use of the Montgomery silicone prosthesis for the prevention or treatment of thick and some … alf months, respectively, with no major irritation or inflammation inside the larynx. No supportive tracheotomy had to be performed, and only in one case was the prosthesis secured externally by percutaneous stitching. On indirect laryngoscopy several months after the prosthesis was removed, two patients showed a perfectly healed larynx; in the third patient …

Transoral operations for craniospinal malformations.

The transoral approach to the lower third of the clivus and to the ventral aspect of the upper cervical spine is used in craniospinal malfor … oral intubation. Splitting of the soft palate or resection of the hard palate are not necessary, a tracheotomy should be performed only in exceptional cases. In congenital craniospinal malformations without dislocation or instability causing a ventral compression of the spinal cord, for instance by the odontoid process, the transoral decompression is prefera …

Aerodigestive tract papillomatosis: bacille Calmette-Guérin (BCG) immunotherapy.

Aerodigestive tract papillomatosis (ADTP) remains a distinct challenge for the otolaryngologist. None of the myriads of therapies utilized to date offers a distinctly improved prognosis with less frequent clinical recurrences or absolute cure rates. Significant complications are avoided by withholding tracheotomy whenever possible and avoiding overly aggressive papilloma removal. Laser technology seems to offer a distinct benefit as regards the latter. Despite much activity in this area, little new knowledge has actually influenced the outcome of patients afflicted with papillomatosis. The dis…

Reconstruction of the prevertebral space with a submental flap: a novel application.

UNLABELLED: The submental island flap is an axial flap based on the submental branches of the facial artery. It is mostly used to recover oral and facial defects. In this report, a case is presented where it is used to reconstruct the prevertebral space during a spine surgery. Its purpose is to act as an interposition tissue between the pharyngeal suture line and the hardware to minimize the risk of wound dehiscence. LEVEL OF EVIDENCE: V.

[Issues of France's "Leonetti Act": Involvement of amyotrophic lateral sclerosis patients in prior discussions concerning respiratory support and end-of-life care].

In accordance with the principle of personal autonomy, expert consensus statements on amyotrophic lateral sclerosis (ALS) recommend early engagement with terminal-phase patients on the type of symptomatic treatment to be administered in the event of respiratory failure, since decompensation progresses too rapidly to allow time for a discussion. The French Parliamentary Act on Patients' Rights and End-of-Life Care (dated 22 April 2005) grants individuals the right to refuse unreasonable treatment and obliges physicians to take account of any prior instructions given by a person before he/she be

[Upper airway obstruction with bilateral vocal cord paralysis secondary to ischemic stroke: report of two cases].

We present herein two patients with bilateral vocal cord paralysis that occurred during the subacute phase of brain infarction. Patients were a 73-year-old man and an 82-year-old woman who suffered from infarction of the basilar artery and the right middle cerebral artery, respectively. The former was diagnosed as atherothrombotic infarction, but the patient experienced repeated aggravation. The latter was diagnosed as cardioembolic infarction. After both patients took clopidogrel and warfarin for the secondary prevention of stroke, upper airway obstruction developed at Day 29 and Day 19, resp

Contemporary assessment of laryngotracheal trauma.

OBJECTIVES: Laryngotracheal trauma is a rare and potentially deadly spectrum of injuries. We sought to characterize the contemporary mechanisms, diagnostic modalities, and outcomes common in laryngotracheal trauma today. METHODS: We performed a retrospective analysis of all laryngotracheal trauma cases at 2 major metropolitan hospitals between 1996 and 2004, detailing mechanisms, associated injuries, diagnostic modalities, and outcomes of laryngotracheal trauma. RESULTS: We identified 71 patients with a mean age of 32.8 +/- 13.3 years (range, 15-71 years). In our series penetrating trauma wa

[Immunity in children with acquired scarry stenosis of the larynx and cervical trachea. Communication 2. The status of T-cell immunity].

T-cell immunity was studied in 59 children (39 boys-66.1% and 20 girls-33.9%) with acquired stenosis of the larynx and cervical trachea at the age of 1 year 9 months to 14 years 8 months. Compromised T-cell immunity was revealed in the examinees, especially in those who had acute infectious-inflammatory diseases or CNS damage before critical care responsible for development of the stenosis. These changes in T-cell immunity should be considered as a factor of risk to develop immune and autoimmune diseases.

[Laser CO2 arytenoidectomy by endoscopic approach in bilateral laryngeal paralysis. Apropos of 45 cases].

Between 1980 and 1993, 45 patients with bilateral cord abductor paralysis were treated by carbon dioxide endoscopic laser arytenoidectomy. Thyroid surgery was the main cause of bilateral laryngeal palsy (67%). Seven patients, who were tracheotomised before treatment, were decanulated. Ninety-one percent of the patients recovered physiologic respiration. The follow up period lasted from one month to thirteen one-half years. Advantages and disadvantages of laser arytenoidectomy are compared with other techniques.

Laryngotracheal paratubal air envelope: a useful sign in voice and airway reconstructive surgery.

Aphonia, dysphonia, and laryngotracheal stenosis with upper airway compromise are diagnosed with increasing frequency. Endoscopic surgical reconstructive techniques accompanied by tracheal T-tube stenting successfully restore voice, airway function, and structure. Timing of tracheal T-tube removal remains controversial, particularly when the tip of the upper arm of the optimally sized T-tube is placed in a critical supraglottic position. The appearance of an air envelope, manifested as linear air densities between the outside wall of the T tube and the inside wall of the larynx and trachea, ha

[Vocal cord paralysis with stridor caused by interarytenoid fibrosis--a complication of intensive care].

Translaryngeal long-term intubation in intensive-care patients was followed in three cases by immobilisation of the vocal cords and severe stridor, caused by a fibrotic scar between the processus vocales and the posterior commissure. Different surgical approaches are described. Endoscopic incision of the scars is recommended. Most important is a bolting technique for several consecutive days.

Epiglottitis in the adult. Recognizing and treating the acute case.

Acute epiglottitis in adults is probably commoner than is generally appreciated. Although upper airway obstruction can occur, the course most often is benign. Acute epiglottitis should be suspected in all patients with a sore throat and dysphagia, especially if symptoms are out of proportion to pharyngeal findings. Diagnosis can be established by mirror or flexible fiberoptic laryngoscopy, lateral radiography of the neck, or both. Treatment consists of maintenance of a patent airway and use of humidified oxygen and antibiotics (ampicillin and chloramphenicol [Chloromycetin] ). The role of cort
