Genetic Insights on the Relation of Vascular Risk Factors and Cervical Artery Dissection.

BACKGROUND: The association between vascular risk factors and cervical artery dissections (CeADs), a leading cause of ischemic stroke (IS) in the young, remains controversial. OBJECTIVES: This study aimed to explore the causal relation of vascular risk factors with CeAD risk and recurrence and compare it to their relation with non-CeAD IS. METHODS: This study used 2-sample Mendelian randomization analyses to explore the association of blood pressure (BP), lipid levels, type 2 diabetes, waist-to-hip ratio, smoking, and body mass index with CeAD and non-CeAD IS. To simulate effects of the most

Systematic review of postpartum and pregnancy-related cervical artery dissection.

BACKGROUND: Cervical artery dissection (CeAD) is responsible of one fifth of cases of ischemic stroke, but is uncommon during pregnancy or the early postpartum period and evidence is derived from published case reports and case series. OBJECTIVES: This systematic review with a prospectively registered protocol was conducted to study the clinical presentation, management and prognosis of this condition. METHODS: Ovid-Medline, PubMed Central, and CINAHL were searched without language restriction. RESULTS: Fifty-seven articles (50 case reports and seven case series) reporting on 77 patients we

Microsurgical anatomy of the perforating branches of the vertebral artery.

BACKGROUND: There is limited data in the literature related to the microanatomic features of the perforating branches of the vertebral artery. METHODS: The 44 vertebral arteries and their branches were injected with india ink or a radiopaque substance and examined under the stereoscopic microscope. RESULTS: The perforating arteries were noted to range in number from 1 to 11 (mean, 6.5) and in diameter between 100 microm and 520 microm (average, 243 microm). They arose from the vertebral artery (VA) (54.54%), 8 from the right, the left or both VAs. The anterior spinal artery (ASA), which was

Reduced Internal Carotid Artery Flow in Color-coded Carotid Duplex Sonography.

PURPOSE: Reduced flow in the internal carotid artery (ICA) is related to cerebral ischemia. We established a classification of reduced ICA flow through color-coded carotid duplex (CCD) sonography. METHODS: We retrospectively reviewed 25,000 CCD images in sonography laboratory. RESULTS: Reduced ICA flow [flow volume (FV) less than 100 mL/min] was found in 1.2% of all studies, and 270 patients were enrolled. We included 8% patients with lesions proximal to the ICA in Group A, 27% with lesions at the proximal ICA in Group B, 49% with lesions distal to the visible ICA in Group C, and 16% with un

Half Trans-Sylvian and Half Subtemporal Approach for Microsurgical Clipping of Basilar Apex Aneurysm.

Basilar apex aneurysms (BAAs) are considered among the most challenging pathologies in open cerebrovascular neurosurgery.1 While often managed endovascularly, complex BAAs or those in which endovascular therapy is not prudent may require microsurgical treatment. Microsurgical approaches include trans-sylvian, subtemporal, and pretemporal. Here, we present a case of a growing BAA in a 65-year-old man with a history of multiple intracranial aneurysms treated with stent-assisted coil embolization. His clinical picture was complicated by left vertebral artery dissection and occlusion during angiog…

Beware of "old" Horner syndrome.

PURPOSE: Chronic Horner syndrome is a rare clinical condition, the etiology of which often remains undiscovered. A patient is presented with an 8-year history of Horner syndrome who was diagnosed with multiple cervical artery dissections. CASE REPORT: A 42-year-old woman presented to our emergency department with a severe occipital headache that woke her up from sleep 3 days earlier. She had a history of headaches and recalled one in particular dating back to 2003. At that time, she sought medical attention at general practitioner's office because of the terrible headache and a noticeable dis

Diagnosis of arterial gas embolism in SCUBA diving: modification suggestion of autopsy techniques and experience in eight cases.

The purpose of this study was to suggest modifications of autopsy techniques in order to improve post-mortem diagnosis of arterial gas embolism (AGE) based on multidisciplinary investigation of SCUBA diving fatalities. Five adult human cadavers from the voluntary donation program of the Human Anatomy Laboratory, and eight judicial autopsied bodies of SCUBA divers from the Forensic Pathology Service were assessed. Before performing any autopsies, we accessed the diving plan and the divers' profiles for each case. We then introduced a new dissection procedure that included identification, isolat

Non-traumatic rupture of the intracranial vertebral artery of a man found dead in a severe car accident - histopathological differentiation by step-serial sections.

A 58-year-old male with untreated hypertension was found dead in his car after a traffic accident on his way to the office. Emergency head CT showed diffuse subarachnoid hemorrhage at cerebral base. On autopsy examination, traumatic injuries were seen on his face and lower extremities. The skull was not fractured and there were no brain contusions except subscalp bleeding at the frontal head. The brain weighed 1510g and showed diffuse subarachnoid hemorrhage due to a rupture of the left vertebral artery (VA). Histopathological examination using serial step sections of every 0.1mm of the whole

Long-Term Results of Macrovascular Decompression Using Suture-Pexy Transposition Technique for the Treatment of Cranial Nerve and Brainstem Compression Syndromes.

BACKGROUND AND OBJECTIVES: Macrovascular compression caused by a dolichoectatic vertebral or basilar artery represents a distinct subset of cranial nerve (CN) and brainstem compression disorders. Standard interpositional microvascular decompression (MVD) may not adequately decompress a large dolichoectatic vessel, necessitating vessel transposition. This study reports 2 decades of institutional experience with macrovascular decompression (MaVD) using a vascular mobilization and suture-pexy technique and evaluates long-term outcomes. METHODS: We retrospectively reviewed all patients undergoing

Cerebral intraventricular haemorrhage in a young adult.

A case of a 26-year-old man who suffered a fatal intraventricular cerebral haemorrhage following an episode of trauma is described. The initial appearance at necropsy suggested a traumatic subarachnoid haemorrhage and initial investigation was directed towards the anterior neck structures and the vertebral arteries with negative results. Dissection of the fixed brain showed a massive intraventricular bleed with secondary involvement of the subarachnoid space and dissection into the cerebral parenchyma. No bleeding points or natural disease of the cerebral vessels could be identified. The pract

Posterior circulation stroke in childhood: risk factors and recurrence.

OBJECTIVE: To ascertain whether posterior circulation stroke in children has distinctive clinical or radiologic features. METHODS: Patients were identified retrospectively from two pediatric neurology centers. Clinical details were ascertained by chart review, and radiologic data were reviewed by three neuroradiologists. RESULTS: Twenty-two cases were identified (17 boys). Twenty children had evidence of vertebrobasilar arterial abnormalities, which were multifocal in 12. The etiology of these was vertebral artery dissection in 10 cases and unclear in the remaining 10. Cardiac abnormalities …

Reversible angiography.

A 26 year old woman presented with a subacute onset of headache, hypertension, and neurologic impairment. Angiography revealed severe extracranial carotid and vertebral artery disease. Long segmental stenoses, lumen irregularities, and aneurysmal dilatations were noted. She improved with medical management and was followed for one year. Because recurrent neurologic symptoms developed, angiography was repeated and disclosed normal vasculature. "Spontaneous dissections" with recovery are the most probable explanation for this phenomenon.

Right vertebral artery entering the third transverse foramen diagnosed by magnetic resonance angiography: a report of two cases.

The vertebral artery (VA) usually enters the sixth transverse foramen (TF). The VA sometimes enters the fifth, and rarely enters the fourth or seventh TF. Reports describing a VA entering the third TF, identified by anatomical dissection or computed tomography angiography, are extremely rare. We herein report two cases in which magnetic resonance (MR) angiography showed the right VA entering the third TF. Coronal reformatted MR angiography source images were useful for identifying the level at which the VA entered the TF. Because the anomalous VA takes an anterior course, it poses a risk durin

Dissection of the V3-V4 due to whiplash injury: description of a non-fatal case.

Whiplash injuries, a common sequel of road traffic accidents, may be defined as a musculo-ligamentous sprain resulting from forced acceleration/deceleration and flexion/ extension neck injury. We report a non-fatal case of vertebral right artery dissection after a minor rear-lateral collision. The injury was detected twenty-four days after trauma when a computerized tomography (CT) scan and cerebral angiography revealed multiple ischemic lesions of the right cerebellum. Digital subtraction angiography (DSA) has been the most sensitive imaging modality to describe the types and degree of vascul

Congenital anomaly of the posterior arch of the atlas: a rare risk factor for posterior circulation stroke.

We report an extremely rare case involving a posterior arch anomaly of the atlas, causing a vertebral artery dissection (VAD) induced posterior circulation stroke. A 16-year-old girl was admitted to hospital because of new onset dizziness. VAD related multiple infarction in the posterior circulation was revealed. The congenital posterior arch anomaly of the atlas, along with instability of the atlantoaxial joint, were discovered accidentally during follow-up. This is the first case of ischemic stroke related to atlantal posterior arch aplasia and atlantoaxial instability. Although rare, it rem…

High-flow bypass and trapping for ruptured intracranial internal carotid artery dissection in fibromuscular dysplasia: illustrative case.

BACKGROUND: Ruptured intracranial internal carotid artery dissecting aneurysm (IICDA) occurring in association with fibromuscular dysplasia (FMD) is exceptionally rare, and the optimal treatment strategy remains undefined. OBSERVATIONS: A 43-year-old woman presented with sudden severe headache and was transferred to the authors' hospital 2 days later after developing impaired consciousness. Initial CT revealed a diffuse subarachnoid hemorrhage with right-sided predominance. Digital subtraction angiography demonstrated a "string-of-beads" appearance in the bilateral cervical internal carotid a

Pictorial Essay of Cervical Duplex Ultrasonography.

Objectives: Cervical duplex ultrasonography (CDU) is a simple, non-invasive, portable technique, that provides valuable high-quality visual information about the integrity of the carotid and vertebral vessels, plaque morphology and flow hemodynamics. CDU is useful in the assessment and follow up of patients with cerebrovascular disease as well as other conditions like inflammatory vasculitis, carotid artery dissection and carotid body tumours. CDU is inexpensive and invaluable in smaller centres. Methods: CDU was performed in all patients in both longitudinal and transverse planes in the out-p
