Blood Biomarkers for the Diagnosis of Peripheral Causes of Vestibular Syndrome: A Systematic Review and Meta-Analysis.

BACKGROUND: Peripheral vestibular syndromes (PVS) encompass disorders such as benign paroxysmal positional vertigo, vestibular neuritis, and Ménière's disease, presenting with vertiginous symptoms. Existing diagnostic approaches rely on clinical and vestibular function tests but have overlapping presentations and invasive investigations. Blood-based biomarkers may offer a minimally invasive diagnostic alternative. OBJECTIVES: This systematic review and meta-analysis aims to synthesize current evidence on blood-based biomarkers to identify PVS compared with controls. METHODS: A literature sea…

Diagnostic Delay in Pediatric Vestibular Disorders.

BACKGROUND: Vertigo and dizziness in children can be multi-factorial. Vestibular function tests allow an improved differential diagnosis and treatment. Delay in diagnosis of the diverse etiologies causing dizziness can adversely affect the health of children and is a matter of concern for their families. This study analyzes the delay in diagnosis and the importance of establishing a diagnosis with detailed history and neuro-otological evaluation. METHODS: A total of 241 children presenting with vertigo to a tertiary otoneurology clinic between January 2019 and April 2022 were analyzed for the…

The usefulness of computerized dynamic posturography for the study of equilibrium in patients with Meniere's disease: correlation with clinical and audiologic data.

The use of computerized dynamic posturogaphy (CDP) in the evaluation of patients with balance alterations not only allows quantification of the subject's capacity to maintain a stable centre of gravity, but also analysis of the degree to which the subject is able to use different types of sensory information. The present study investigated the possible use of CDP for clinical staging of vestibular diseases, specifically Meniere's disease (MD). We applied CDP sensory organization tests to 75 patients with definitive MD (AAO-HNS 1995 criteria). A total of 98 CDP sessions were included in the ana

The systemic application of diazepam facilitates the reacquisition of a well-balanced vestibular function in a unilateral vestibular re-input model with intracochlear tetrodotoxin infusion using an osmotic pump.

Diazepam is a popular medicine used in the treatment of acute vertigo. In the past, many studies investigating the effect of diazepam in peripheral vestibular destruction have been reported. However, no previous study has yet investigated the effect of diazepam on a model with a transient and reversible vestibular function similar to recurrent vertigo as seen in Meniere's disease. We thus made a peripheral vestibular re-input model by the unilateral intracochlear administration of tetrodotoxin (TTX) using an osmotic pump and then examined the influence of diazepam on the vestibular system in t

Multiple positional nystagmus suggests multiple canal involvement in benign paroxysmal vertigo.

CONCLUSION: Video-oculography demonstrates a higher occurrence of atypical positional nystagmus in patients with benign paroxysmal positional vertigo (BPPV). This includes anterior and horizontal canal variants and multiple positional nystagmus, suggesting combined lesions affecting several canals. OBJECTIVE: To analyse the video-oculographic findings of positional tests in patients with BPPV. MATERIAL AND METHODS: Seventy individuals with symptoms of BPPV and positional nystagmus were included in this study. The diagnosis was based on a history of brief episodes of vertigo and the presence

Ocular counterrolling abnormalities in spasmodic torticollis.

Spasmodic torticollis is a focal dystonic movement disorder of unknown origin, long hypothesized to have some vestibular involvement. An examination of otolith function, ocular counterrolling, was performed on eight patients with this disorder. The test consisted of photographing both eyes while the subject underwent rotation in both naso-occipital and earth-horizontal long axes. Measurements of eye torsion were made with a superimposition technique accurate to 0.1 degrees. Results showed all eight patients had abnormal ocular counterrolling. The most notable defect was the lack of sustained e

Subjective visual vertical in first time seafarers experiencing sea sickness.

OBJECTIVES: Sea sickness, a form of motion sickness, is a common condition among sailors on various sailing vessels, primarily due to their roll and pitch movements. While the exact neurobiological mechanisms remain unclear, the most widely accepted explanation is the sensory conflict theory, which identifies two main sources of conflict: discrepancies between visual and vestibular inputs and conflicts within the vestibular system itself. This study aims to evaluate otolith-mediated verticality perception in first-time seafarers on board a naval frigate ship using the Subjective Visual Vertica

Retroviral-vector-mediated gene therapy to mucopolysaccharidosis I mice improves sensorimotor impairments and other behavioral deficits.

Mucopolysaccharidosis I (MPS I) is a lysosomal storage disease due to α-L-iduronidase (IDUA) deficiency that results in the accumulation of glycosaminoglycans (GAG). Systemic gene therapy to MPS I mice can reduce lysosomal storage in the brain, but few data are available regarding the effect upon behavioral function. We investigated the effect of gene therapy with a long-terminal-repeat (LTR)-intact retroviral vector or a self-inactivating (SIN) vector on behavioral function in MPS I mice. The LTR vector was injected intravenously to 6-week-old MPS I mice, and the SIN vector was given to neona

Nerve origin of the acoustic neuroma.

Two hundred and seventy-one Japanese patients who underwent surgical removal of neuroma from the cerebello-pontine angle using the translabyrinthine approach were retrospectively reviewed in order to investigate the nerve origin on an anatomical basis. In 269 out of the 271 cases, the origin of the neuromas was identified. Among these cases, 84.8 per cent of tumours originated from the inferior vestibular nerve (IVN), followed by the superior vestibular nerve (SVN) (8.9 per cent). Of the 5.6 per cent of tumours arising from the vestibular nerve, however, it could not be determined whether they

Audiovestibular involvement in systemic sclerosis.

In order to evaluate the nature and association of audiovestibular disturbances and systemic sclerosis (SSC), 37 unselected SSC patients were studied with a detailed audiological and vestibular examination since November, 1987. Pure-tone audiometry, speech audiometry, impedance audiometry, brainstem response audiometry and vestibular function using electronystagmographic recording were performed. We found a rather frequent audiovestibular involvement (41%). A hearing loss was found in 14 SSC patients; hearing loss was sensorineural in 10 cases and mixed in 4 cases. The latter revealed a findin

A cerebellar-vestibular explanation for fears/phobias: hypothesis and study.

To clarify and test the cerebellar-vestibular (CV) basis of fears/phobias, responses of 4000 learning disabled children, adolescents, and adults with neurological and electronystagmographic (ENG) evidence of CV-dysfunction were analyzed for anxiety-related symptoms. Of this sample, 64.6% indicated fears/phobias; females were significantly more predisposed; mixed-handedness was significantly related to fears of heights and reduced vestibular response or asymmetric vestibular functioning. Also, adults had a higher incidence of the specific fears/phobias characterizing agoraphobia than children a

A Rare Case of Hearing Impairment due to Cerebello-Pontine Angle Lesion: Trigeminal Schwannoma.

Schwannoma of the trigeminal nerve is a rare condition. Even rarer is hearing loss occurring as a result of this lesion. The aim of this study is to highlight this rare cause of hearing impairment. Here we report the clinical features and findings of the imaging and audiological investigations of a case of trigeminal schwannoma diagnosed at our institution. Our patient presented with headache, giddiness, tinnitus, left-sided facial weakness, left-sided hearing loss, right-sided hemiplegia, and unintelligible speech. Radiological studies revealed a large well-defined mass lesion in the left cer

Impaired vestibulo-ocular reflex (VOR) in spinocerebellar ataxia type 3 (SCA3): bedside and search coil evaluation.

Vestibulo-Ocular Reflex (VOR) abnormalities in cerebellar ataxias are a matter of renewed interest. We have previously reported vestibular areflexia in a group of Yemenite-Jews with Spinocerebellar Ataxia Type 3 (SCA3) who had clear bilateral pathological horizontal Head Impulse Test (HIT). The objective of this study was to evaluate the VOR of ten SCA3 patients who have variable bedside HIT responses by recording their eye movements using magnetic search coils and to correlate these results with their clinical and genetic data. Eight out of the ten patients have abnormal horizontal HIT detect

Hyperactive VOR and hyperventilation after whiplash injury.

Otoneurological and respiratory tests were performed on 32 patients after whiplash injury caused by a rear-end car collision. Oculomotor and cervico-ocular test results were generally normal. In a significantly large number of case, the vestibulo-ocular reflex (VOR) was hyperactive (n = 17; 53%) and the respiratory test results were typical of the hyperventilation syndrome (HVS) (n = 12; 38%). Hyperactive VOR and the HVS occurred significantly more often in combination (n = 7; 22%) than could be accounted for by combined false positivity. Most of the significant findings were due to high relat

Neurotological follow-up studies upon Minamata disease.

During 1986-87, 35 patients with organic mercury intoxication, who had been recognized as having Minamata disease by the Arbitration Committee in Niigata during 1968-78, were followed up neurotologically in Kido Hospital, Niigata, Japan. Characteristic follow-up features were served, as follows. 1. In 6 cases, spontaneous nystagmus had disappeared, but had newly appeared in 9 cases. However, positional nystagmus was markedly improved in 15 cases. 2. In OKN tests, especially in the vertical OKN test, relatively significant deterioration was observed at the follow-up examinations. 3. There was l

Internal auditory canal vascular loops: audiometric and vestibular system findings.

Prominent loops of the anterior inferior cerebellar artery in the cerebellopontine angle are found frequently during anatomic studies of this region. These vascular loops are suspected of causing hearing loss, tinnitus, and vertigo, and surgery has been advocated to separate the vascular loop from the eighth cranial nerve. Previous reports have described pathologic anatomy, surgical approaches, and results of treatment. In this study, we report the results of a uniform battery of audiometric and vestibular system test results administered to fifteen patients with prominent vascular loops in th
