Familial progressive vestibulocochlear dysfunction.

A kindred is presented in which several members had complaints of head movement-dependent oscillopsia due to acquired vestibular areflexia in combination with progressive hearing loss. History was noncontributory for other neurologic or otologic diseases (including infectious diseases) or use of neuro-ototoxic drugs. The pedigree suggests autosomal dominant inheritance.

Flunarizine treatment in dizzy patients with vestibular hyperreactivity and hyperventilation.

Flunarizine (10 mg/day) was given in an open pilot study to 14 patients with the primary complaint of dizziness, who showed vestibular hyperreactivity in the velocity step (VS) test and were proven to have a hyperventilation syndrome (HVS) without any other disease. The VS test was repeated after 4-6 weeks of treatment. Twelve of these HVS patients reported alleviation of dizziness. Objective evaluation of the results of treatment was attempted by defining suitable response parameters and comparing their values to those obtained in a control group of 14 healthy volunteers tested twice with the

Effect of drug therapy on compensation from vestibular injury.

The influence of selected drugs on compensation from unilateral labyrinthectomy was studied in the cat. The drugs investigated included: a. amphetamine, b. diazepam, c. dimenhydrinate, d. scopolamine, and e. trimethobenzamide. The most beneficial drugs for improving recovery were a stimulant (amphetamine) and a general anti-emetic (trimethobenzamide). It is postulated they had this effect by improving the overall activity level with general exercise being a known positive influence. Conversely the other drugs either had little effect on recovery or hindered it, by presumably suppressing the se

Atypical brainstem encephalitis: magnetic resonance imaging and oculographic features.

A 50-year-old woman had primary position upbeating nystagmus, abnormalities of the lower cranial nerves, mild cerebellar signs, and CSF lymphocytosis. CT and MRI showed enlargement of the pons. Eye movement recordings revealed impaired pursuit and optokinetic nystagmus, and visual-vestibular interactions were absent. The patient improved spontaneously.

A model for signal transmission in an ear having hair cells with free-standing stereocilia. II. Macromechanical stage.

A model of the signal-processing properties of the macromechanical system of the alligator-lizard middle and inner ear is developed. The model is based on measurements (as a function of tone frequency) of ossicular and basilar-membrane motion and of acoustic input-admittance at the tympanic membrane before and after alterations of middle-ear and inner-ear structures. From the structure of the ear, we formulate an equivalent electric network consisting of six admittance blocks and two transformers. The admittance blocks represent the mechanical properties of particular structures, e.g. the tymp

Quantitative analysis of the visual vestibulo-ocular reflex using sinusoidal rotation in patients with peripheral vestibular disorders.

In order to investigate interaction of the vestibular and optokinetic systems in patients with peripheral vestibular disorders, the visual vestibulo-ocular reflex (V-VOR) gain in 49 patients with peripheral vestibular disorders was quantitatively measured by using pendular harmonic sinusoidal rotation with optokinetic stationary stimuli. The patients were rotated sinusoidally both with eyes closed and open at an amplitude of 240 degrees and at a frequency of 0.1 Hz (peak velocity 75.4%s). Comparing the vestibulo-ocular reflex (VOR) and V-VOR gains in the patients with definite Meniere's diseas

Hearing preservation in vestibular neurectomy.

The goal of vestibular neurectomy is to control disabling vertigo while preserving hearing in patients with nonhydropic intractable peripheral vertigo or in patients with Meniere's disease in which an endolymphatic sac procedure has failed. Labyrinthectomy continues to be used to treat patients with intractable vertigo and serviceable hearing. We feel that a labyrinthectomy is contraindicated when any useful hearing remains. Vestibular neurectomy affords the surgeon a means to eliminate the abnormal vestibular input without sacrificing hearing. Two approaches have been used to section the vest

Klippel-Feil syndrome and associated ear deformities.

The Klippel-Feil syndrome is characterized by a short, thick neck with restricted head mobility and a low occipital hairline. The head appears to rest directly on the thorax and the trapezius muscles flare out on either side of the neck, producing a pterygium-like effect. Cervical roentgenograms demonstrate fusion of some or even all of the vertebral bodies into bony blocks. Ear deformities occur in approximately one-third of the Klippel-Feil patient population. We report five cases of Klippel-Feil syndrome and describe their otologic and roentgenographic features. Although no single character

Evaluation of the brain-stem function by the auditory brain-stem response and the caloric vestibular reaction in comatose patient.

The auditory brain-stem response (ABR) and the caloric vestibular reaction (CVR) were investigated in 100 patients in deep coma to evaluate the brain-stem function of these patients precisely and to predict their prognosis accurately. In the first ABR examination, 54 patients showed normal and 46 abnormal ABRs. Among the latter, five showed partial wave disappearance and 21 exhibited no ABRs. Twenty-six of the 30 patients who recovered (87%) showed normal ABRs and 21 of the 57 who died (37%) also exhibited normal latency at that time. On the other hand, all the patients who showed wave disappe

Clinical and experimental studies on the incorporation of materials applied as covering for the labyrinth window.

From among the materials used for covering the labyrinth window, of the foreign materials gelfoam proved ideal from clinical and experimental points of view, while of the materials of interstitial origin, fascia, perichondrium, and periosteum seem to be suitable for covering the labyrinth window, resp. for transplantations in the tympanic cavity.

Validating the Utility of High Frequency Ocular Vestibular Evoked Myogenic Potential Testing in the Diagnosis of Superior Semicircular Canal Dehiscence.

INTRODUCTION: Ocular vestibular evoked myogenic potential (oVEMP) is a diagnostic test employed in the evaluation of superior semicircular canal dehiscence (SSCD) syndrome. Previous work showed that the presence of the n10 component of oVEMP at 4000 Hz was diagnostic of SSCD with perfect sensitivity and specificity of 1.0 in a series of 22 patients. This study sought to validate the diagnostic accuracy of high-frequency oVEMP with comparison to 500 Hz oVEMP and cervical vestibular evoked myogenic potential (cVEMP) in a larger series of patients. METHODS: Retrospective chart review of 171 pati

Superior divisional vestibular paresis in anterior inferior cerebellar artery infarction.

A 74-year-old woman with sudden vertigo and dysarthria showed spontaneous nystagmus beating rightward, upward and clockwise. She also exhibited dysfunction of the left utricle and superior and horizontal canals, which was documented by head impulse and heave tests, measurements of ocular torsion and subjective visual vertical tilt, bithermal caloric tests, and rotation axis analysis of the spontaneous nystagmus. In contrast, functions of the left posterior canal, saccule, and cochlea were remained intact, as evidenced by normal head impulse test, audiometry and vestibular evoked myogenic poten

Vestibular-evoked myogenic potentials in the diagnosis of superior canal dehiscence syndrome.

Patients with superior canal dehiscence (SCD) syndrome have vertigo and oscillopsia induced by loud noises and by stimuli that result in changes in middle ear or intracranial pressure. We recorded vestibular-evoked myogenic potentials (VEMP responses) in 10 patients with SCD syndrome. The diagnosis had been confirmed in each case by evoked eye movements and by high-resolution CT scans of the temporal bones that showed a dehiscence overlying the affected superior canal. For the 8 patients without prior middle ear disease, the VEMP threshold from the dehiscent ears measured 72 +/- 8 dB NHL (norm

Perilymph fistula: the Stanford experience.

Seventy-eight tympanotomies were performed to determine the presence of perilymph fistulas (PLFs); of these, 51% were found. The oval and round windows of all patients were grafted, whether PLFs were present or not. Of those patients with PLFs, 64% had resolution of their major symptom; when no PLF was found, 44% had a similar outcome. We concluded that 1) PLFs often behave as if they are epiphenomena in relation to hearing and balance, 2) PLFs can be intermittent, 3) PLF surgery is disappointing for restoration of hearing in sudden hearing loss when compared to the rate of spontaneous recover

Acute intracranial vascular accidents.

Cerebellar hemorrhage, cerebellar infarction, pontine hemorrhage, and pontine infarction are four clinical conditions representative of intracranial posterior fossa accidents, which can mimic acute vestibular disturbances. It is exceedingly important to recognize these clinical entities early and to differentiate them from peripheral vestibulopathies so that appropriate diagnostic tests and therapeutic measures can be instituted early to prevent the high mortality rate seen without appropriate treatment. It is appreciated that without computed axial tomography, rapid diagnosis of these clinica

Effects of vestibular habituation training on synaptic plasticity in the medial prefrontal cortex of mice with motion sickness.

BACKGROUND: Motion sickness (MS) is triggered by vestibular conflict, yet how vestibular habituation training (VHT) engages higher-order cortical circuits remains unclear. OBJECTIVE: To test whether VHT modulates medial prefrontal cortex (mPFC) activity and structural plasticity in a rotational MS model. METHODS: Male C57BL/6J mice were assigned to control (CON), motion sickness (MS), or vestibular habituation training (VHT) groups. MS was induced by a single 4-g session (four 1-min bouts; total 4 min), whereas VHT mice received 14 daily sessions (30 × 1-min bouts/day; total 30 min/day). Beh
