OBJECTIVES: This study aimed to evaluate amblyopic participants with myopic anisometropia who, despite not being initially diagnosed with strabismus, failed to improve their visual acuity (VA) after treatment. The hypothesis is that these individuals present with microtropia (MT) and sensory adaptations. Since they appear orthotropic on the unilateral cover test, the hypothesis was tested through an objective evaluation of fixation, the presence of eccentric fixation (EF) would indicate that these eyes are monocularly deviated.

A patient with congenital ptosis and inferior rectus fibrosis is described. Marked atrophy of the left inferior rectus muscle was demonstrated by orbital computerized tomography (CT). Computerized tomographic findings in this patient and a series of 20 patients with no ocular disease and normal motility indicate that CT is a valuable tool for the evaluation of extraocular muscle size when qualitative comparison is made between corresponding muscles in the same patient.

In a clinic where 250 children have been fitted with contact lenses between 1976 and 1982, one hundred and five were bilateral infantile aphakic patients. A review of the data sheets of 43 bilateral infantile aphakic patients in this group implies that the corneal complications of treatment are substantial but surmountable and that the risks and difficulties of contact lens treatment are probably justifiable if the recent reports of improved visual function in these patients are substantiated.

Fifty patients with aphakia and strabismus were studied. The chief complaint was diplopia following full optical correction. Prism therapy was recommended if the diplopia persisted after 3 to 4 months. When prism therapy was ineffective or impractical, surgery was recommended. Forty-seven eyes from 25 patients with neurological disease were examined with an automatic computerized perimeter primarily intended for glaucoma. Manual kinetic and static perimetry with Goldmann's instrument was used for comparison. In 32 fields, the automatic method disclosed all defects that had been found by manual perimetry. Furthermore, it detected three small defects that had been missed in the initial manual examination. The remaining 12 fields were normally using both methods. Consequently, in detecting field defects, the automatic perimeter was superior to the manual method.
