A reproducible animal model is necessary to examine the use of antimicrobial agents for prophylaxis and treatment of bacterial endophthalmitis. We determined the minimum inoculum size of S. aureus and P. aeruginosa that consistently produced endophthalmitis when injected into aphakic rabbit eyes immediately following surgery. Both anterior chamber and intravitreal injections were examined. For S. aureus, an intravitreal inoculum of 19.3 +/- 7.5 CFU and an anterior chamber inoculum of 50.5 +/- 4.0 CFU were required. A simple clinical predictor of the amount of spectacle convex lens power that will be needed to correct the refractive error of an aphakic patient after cataract surgery is developed based on clinical measures of anterior corneal surface power (keratometer measurements) and axial length. This predictor derived from matrix optics is compared to previous ones developed by Binkhorst and Sanders et al. using different methodologies.

Shortly thereafter, he acutely developed pain, nausea, and visual blur with an intraocular pressure of 42 mm Hg. Forty-one consecutive aphakic corneal transplants performed by the same surgeon were studied in order to determine the effects on intraocular pressure and corneal curvature of using 8.0-mm donor buttons in 7.5-mm recipient openings. One half of the grafts had oversize donor buttons, and all were followed for 13 months. The qualitative photostress test is a simple kinetic test for the evaluation of macular function. The physiological basis of the test is discussed and the results of testing 20 normal patients and 14 postoperative cataract patients are presented. Patients with CME have a severe functional defect that is readily demonstrated with photostress testing.

Corneal swelling response was measured for 12 unilateral aphakic subjects wearing high-plus hydrogel contact lenses on both eyes. Significantly less corneal edema was measured for aphakic eyes than for phakic eyes after 2, 4, and 6 hr of lens wear. These results indicate that the cornea of the aphakic eye responds differently to a given physiologic challenge than does the cornea of the phakic eye. Previous estimates of oxygen permeability required of contact lens materials for extended wear may be artificially high for aphakic eyes.

Extended wear contact lenses are sure to gain approval sometime during 1979. Four such lenses are briefly described, and the author's experience with one of them is discussed. The characteristics that differentiate these lenses from daily wear lenses are the high water content and the thinness.

Twenty-eight consecutive cases of aphakic cystoid macular edema in 16 patients were treated with corticosteroid drops. Enough medication was used in each occurrence to achieve intraocular pressure elevation. This increases the hydrostatic pressure on the retina thereby decreasing retinal swelling pressure and reversing the cystoid macular edema. Visual acuity improved in each instance after resolution of the cystoid macular edema.

A 2-year follow-up study was carried out on patients known to have developed aphakic macular oedema 6 weeks postoperatively, and the results were compared with those in a control group who had not developed oedema. Four eyes (12%) still had macular oedema, the visual acuities ranging between 6/5 and 6/9. There was no significant difference in visual acuities at 2 years between the 2 groups of patients.
