Analysis of junctional beats during slow pathway ablation: Illuminating the mechanism of typical and atypical AV nodal re-entrant tachycardia.

BACKGROUND: Junctional rhythm (JR) frequently occurs during radiofrequency (RF) ablation procedures targeting the slow pathway (SP) for atrioventricular nodal re-entrant tachycardia (AVNRT), signaling successful ablation. Two types of JR have been noticed: typical JR as His activation preceding atrial activation, and atypical JR as atrial activation preceding the His activation. Nevertheless, the origin and characteristics of JR remain incompletely defined. OBJECTIVE: This study aimed to investigate whether JR induced by RF ablation at the anatomical SP position could reveal preferential cond

Respiratory complications of Ehlers-Danlos syndrome type IV.

We describe a case of Ehlers-Danlos syndrome (EDS) type IV in a male in early half in his twenties, who experienced recurrent and eventually fatal pulmonary hemorrhage. EDS type IV is a rare disorder of type III collagen synthesis that is characterized by unusual facies, thin translucent skin with a venous vascular pattern, easy bruising, and hypermobility of the small joints. Autopsy findings showed hypermobility of the joints and distensibility of the skin. Microscopically, the abdominal skin showed substantially decreased dermal thickness. Moreover, the reticular dermis showed fine collagen

Diagnostic approach to cardiac arrhythmias.

The diagnostic approach to cardiac arrhythmias should be logical and starts with the clinical history, which provides two types of information: (a) the presence of symptoms, and (b) the clinical context, including the presence of an underlying heart disease. Clinical history and examination are helpful in the choice of pertinent invasive or noninvasive tests. The tolerance of the arrhythmia is not helpful in determining the type of arrhythmia because ventricular tachycardia, for example, may be well tolerated or even asymptomatic. The electrocardiogram (ECG) in sinus rhythm may be suggestive o

A congenital abnormality in the arrangement of muscle bundles in a segment of the distal ileum, producing obstruction: a variant of the so-called "giant Meckel's diverticulum".

This 4-year-old male child was diagnosed at birth as having several minor congenital anomalies. X-rays taken during the first year of his life showed a single, massively dilated loop of bowel in the upper abdomen, not appreciated at the time. In 1980 he was admitted to Jackson Memorial Hospital with the diagnosis of small bowel obstruction. An upper gastrointestinal series showed one tremendously dilated loop of distal ileum. At laparotomy, the patient was found to have extreme segmental dilatation of one loop of distal ileum which ended abruptly; there was no evident external cause for obstru

Long-term management of bradyarrhythmias following open heart surgery: surgical A-V block and sick sinus syndrome after surgery for secundum atrial septal defects treated with permanent cardiac pacing.

Postoperative clinical findings from 25 patients with surgical A-V block and 12 with SSS following surgery for ASD who received permanent pacemakers were analyzed in order to consider the long-term management of postoperative bradyarrhythmias. Surgical A-V block: Episodes of Adams-Stokes were observed in 11/25 patients before pacemaker implantation, and in 6 of these (55%) the onset of episodes occurred more than a year after open heart surgery. Of 8 cases for which ECG's during Adams-Stokes were available, 2 had bifascicular or trifascicular block which progressed to complete A-V block below

Electrophysiologic studies in the denervated transplanted human heart. II. Response to norepinephrine, isoproterenol and propranolol.

Five patients who had received a transplanted human heart 1 to 3 years previously were studied to determine the effects of norepinephrine, isoproterenol and propranolol on the atrioventricular (A-V) conduction system. Using the His bundle technique, atrial, His bundle and ventricular electrograms were recorded, and central aortic pressure was monitored during the administration of these drugs. Norepinephrine was given by continuous infusion to four patients in doses ranging from 4 to 8 mug/min, with the systolic arterial pressure increasing by an average of 72 mm Hg. Concomitantly, there was a

Initial experience with ablation of the innervation surrounding sinus and atrioventricular nodes to treat paroxysmal bradyarrhythmia.

BACKGROUND: The symptomatic bradyarrhythmia is Class I indication for pacing therapy which is not a radical cure. The present study aimed to assess the feasibility and to present the initial results of the restricted ablation of the parasympathetic innervation surrounding sinus and atrioventricular (AV) nodes for treating patients with bradyarrhythmia. METHODS: A total of 13 patients with cardiogenic syncope were included from May 2008 to June 2015. Under the guidance of fluoroscopy and /or three-dimensional geometry by 64-slice spiral computed tomography, atrial activation sequence in sinus

Long-term follow-up of incessant supraventricular tachycardia treated with oral encainide.

The long-term follow up (32 to 44 months, mean 37 months) of 9 patients with incessant supraventricular tachycardia treated with oral encainide is reported. During that follow up period, 1 patient developed an 'escape' phenomenon to encainide and incessant supraventricular tachycardia recurred in spite of increasing doses of encainide. Two patients required electrical fulguration of the bundle of His because of inability to control incessant supraventricular tachycardia. One patient discontinued treatment without suffering from recurrences of the arrhythmia. In 6 of the original 11 patients tr…

Patent Foramen Ovale as a Rare Focus of Origination of Atrial Tachycardia.

Supraventricular tachycardia refers to a group of arrhythmias whose mechanism involves tissues from the His bundle or above. Repetitive focal atrial tachycardia (AT) (FAT) accounts for less than 10% of supraventricular tachycardia cases. FAT originating from the patent foramen ovale (PFO) has not been well-described and is a rarely reported phenomenon to date. Here, we report a rare case of FAT arising from the PFO. To the best of our knowledge, this is the first detailed report of AT arising from the PFO. We have included the description of the ablation procedure and postulated the possible e

Intermittent manifestation of ventricular preexcitation with right bundle branch block induced by interpolated ventricular premature contractions: A report of 3 cases.

Electrocardiographic (ECG) analysis becomes challenging in the presence of concurrent ventricular preexcitation (WPW) and right bundle branch block (RBBB) due to their mutually interfering ECG manifestations. Interpolated ventricular premature contractions (IPVCs) can transiently alter the conduction velocity and refractory period of the WPW accessory pathway and the His-Purkinje system, thereby leading to intermittent manifestation of WPW or RBBB. In Case 1 and Case 2, IPVCs converted sinus beats from the WPW + RBBB pattern to the pure RBBB pattern; in Case 3, sinus beats after IPVCs changed

Myocarditis in a traveler returning from the Dominican Republic: an unusual presentation of dengue fever.

Myocarditis is an uncommon manifestation of dengue fever. We describe a case of a 69-year-old Hispanic male who presented to an emergency room in New York City 3 days after returning from a trip to the Dominican Republic complaining of a 1-day history of chest pain and fever. His first electrocardiogram showed a new left bundle branch block, and initial cardiac enzymes included troponin of 5 ng/dL, creatine kinase-MB of 9 ng/mL, and myoglobin of 234 ng/mL. Dengue fever antibodies were found to be elevated: immunoglobulin M (IgM) titer was 2.48 (reference range < 0.9), and immunoglobulin G (IgG

Conduction system abnormalities in alcoholics with asymptomatic valvular disease who suffer sudden death.

We examined the cardiac conduction system of three alcohol abusers who died suddenly. Cases 1 and 2 showed mitral valve disorder (mitral valve prolapse and rheumatic valvular disease), while Case 3 showed mild Ebstein's anomaly. On examination of the conduction system, Cases 1 and 2 showed severe fibrofatty infiltration into the conduction system, and we conclude that these findings were probably a complication of alcohol abuse. Both cases also demonstrated severe small artery disease. The conduction system of Case 3 showed an anomalous location of the bundle of His with its fragmentation. The…

[Usefulness of adenosine to detect advanced paroxysmal atrioventricular block as a cause of syncope].

We present the case of a 66 years old man with recurrent episodes of syncope, up to three times during the last two months without previous symptoms. An ECG after the syncope showed a bilateral block (left anterior fascicle block and right bundle branch block) and first grade atrioventricular block. The exercise test did not demonstrate either AV conduction disorders or tachyarrhythmia episodes. Holter monitoring showed premature ventricular complexes; tilt testing and carotid sinus massage were normal. The electrophysiologic study revealed no alteration in the conduction system. Device implantation complicated by a retrosternal goiter.

Central venous obstruction in the cardiac implantable electronic devices (CIED) population is commonly due to thrombosis and fibrosis secondary to the passage of pre-existing leads. However, vein occlusion before CIED implantation is uncommon, and one cause is retrosternal goiters. We report a case where the failure of the initial implantation of a primary CIED led to an unusual implantation route without goiter excision. The patient had an indication for cardiac resynchronization therapy (CRT) given his left ventricular (LV) function was impaired and had second-degree heart block Mobitz Type

A case of acute myocardial infarction in paced rhythm. Utility of the Barcelona algorithm.

We describe the case of a 70-year-old man with a pacemaker implant who presented to us with atypical chest pain. His admission ECG did not satisfy the criteria for myocardial infarction according to standard Sgarbossa criteria, and Smith modified Sgarbossa criteria. However, we applied the recently developed Barcelona algorithm, which is used to diagnose acute myocardial infarction in the presence of intrinsic left bundle branch block.Electrocardiographic criteria for an acute myocardial infarction were met. Lead V6 displayed a low voltage QRS complex (3 mm) with 1 mm ST depression, satisfying

Wide complex irregularly regular tachycardia with QRS alternation.

A 44-year-old white man was referred to our service because of a history of symptomatic rapid palpitations that had begun on January 2007. He had no previous history of syncope or family history of sudden cardiac death. The patient was otherwise well and had not been taking any medications. The electrocardiogram (ECG) recorded during sinus rhythm was within normal limits. A 12-lead ECG was performed during the last episode on July 2007 while he was working at his office. An irregular wide complex tachycardia with varying QRS morphology (left bundle-branch block with an alternating pattern) was

Atrial and ventricular activation sequence after ventricular induction/entrainment pacing during fast-slow atrioventricular nodal reentrant tachycardia: New insight into the use of V-A-A-V for the differential diagnosis of supraventricular tachycardia.

BACKGROUND: The atrial and ventricular response observed immediately after cessation of ventricular induction/entrainment pacing is commonly analyzed to discriminate atrial tachycardia from other supraventricular tachycardias during electrophysiologic studies. However, the response in fast-slow atrioventricular nodal reentrant tachycardia (F/S-AVNRT) remains poorly investigated. OBJECTIVE: The purpose of this study was to analyze the atrial and ventricular activation patterns after ventricular pacing in F/S-AVNRT. METHODS: We enrolled 28 patients with F/S-AVNRT incorporating a typical slow p

Long-term outcome after radiofrequency catheter ablation of atrioventricular nodal reentrant tachycardia with the anterior-approach method.

Previous studies have reported only short-term (6 to 10 months) follow-up after ablation of atrioventricular (AV) nodal reentrant tachycardia by using the anterior approach. The objective of this study was to determine the long-term efficacy and safety of radiofrequency catheter ablation of AV nodal reentrant tachycardia with the anterior-approach method. In 56 patients (16 men and 40 women; mean [+/-SD] age, 44 +/- 19 years) with symptomatic AV nodal reentrant tachycardia. In 53 patients, ablation was performed initially by using a standard 7F, 2 mm-tipped tripolar His bundle catheter when th
