Multi-class classification model for psychiatric disorder discrimination.

BACKGROUND: Physicians follow-up a symptom-based approach in the diagnosis of psychiatric diseases. According to this approach, a process based on internationally valid diagnostic tools such as The Diagnostic and Statistical Manual of Mental Disorders (DSM) or International Classification of Diseases (ICD), patient reports and the observation and experience of the physician is monitored. As in other fields of medicine, the search for biomarkers that can be used in processes related to diseases continues in psychiatry and various researches are carried out in this field. OBJECTIVES: Within the

Co-existing mental and somatic conditions in Swedish children with the avoidant restrictive food intake disorder phenotype.

BACKGROUND: Avoidant restrictive food intake disorder (ARFID) is a feeding and eating disorder, characterized by limited variety and/or quantity of food intake impacting physical health and psychosocial functioning. Children with ARFID often present with a range of psychiatric and somatic symptoms, and therefore consult various pediatric subspecialties; large-scale studies mapping comorbidities are however lacking. To characterize health care needs of people with ARFID, we systematically investigated ARFID-related mental and somatic conditions in 616 children with ARFID and >30,000 children wi

Sex- and estrous-related response patterns for alcohol depend critically on the level of compulsion-like challenge.

Alcohol use disorder is a substantial social and economic burden. During the last years, the number of women with drinking problems has been increasing, and one main concern is that they are particularly more vulnerable to negative consequences of alcohol. However, little is known about female-specific response patterns for alcohol, and potential underlying differences in brain mechanisms, including for compulsion-like alcohol drinking (when intake persists despite adverse consequences). We used lickometry to assess behavioral microstructure in adult Wistar male and female rats (n = 28-30) dur

A transdiagnostic framework for neurological intervention in maladaptive repetitive behaviors.

From daily rituals to pathological patterns of behavior and thought, a wide spectrum of habitual behaviors emerges through repeated reinforcement learning and becomes embedded in cortico-basal ganglia circuits. Once established, these behaviors operate automatically and resist conscious control. This review summarizes neurological intervention strategies for correcting pathological habits (e.g., addiction, obsessive-compulsive disorder) and restoring adaptive behavioral patterns. Pharmacological approaches target the plasticity of habit circuits via modulation of dopamine, serotonin, and gluta

Mechanisms of Selective Serotonin Reuptake Inhibitors in Treatment-Resistant Obsessive-Compulsive Disorder: Cortico-Striatal Circuitry, Glutamatergic Dysregulation, and Neuroplastic Constraints.

Obsessive-compulsive disorder (OCD) is a chronic and disabling neuropsychiatric condition for which selective serotonin reuptake inhibitors (SSRIs) remain the first-line pharmacological treatment. Despite their widespread use, a substantial proportion of patients fail to achieve adequate symptom remission following appropriately dosed and prolonged SSRI therapy, highlighting the clinical challenge of treatment-resistant OCD. Traditional serotonin-centric models do not fully account for delayed therapeutic onset, incomplete response, or the frequent need for pharmacological augmentation. This r

We need to start thinking about thinking: The impact of rumination on OCD symptom improvement in exposure and response prevention.

While rumination has been recognized in clinical settings as prevalent and problematic for individuals with obsessive compulsive disorder (OCD), its relationship to OCD symptom trajectories has been understudied. In a large sample of N = 315 adults receiving exposure and response prevention (ERP) for OCD we evaluated the relationship between rumination and OCD symptoms. In our first aim, we characterized change in rumination across treatment and calculated the percentage of patients exhibiting change in rumination. In aim 2, we characterized how change in rumination impacts OCD symptom traject

The rollercoaster of obsessive-compulsive disorder: How chronotype and time of day affect behavioral inhibition in adults with OCD.

Chronotype, a person's circadian preference, reflects individuals' natural pattern of alertness levels throughout the day. It has been shown that chronotype contributes to within-day symptom fluctuations in obsessive-compulsive disorder (OCD). However, the underlying mechanism of this effect is not yet clear. Inhibitory control, an executive function crucial for suppressing unwanted behaviors and thoughts, is essential for managing OCD symptoms and is highly influenced by alertness levels. Hence, the current study investigated the impact of chronotype and time of day on inhibitory control in O

Development of the scrupulosity inventory: A factor analysis and construct validity study.

BACKGROUND AND OBJECTIVES: Scrupulosity, despite its considerable prevalence and morbidity, remains under-investigated. The present study develops and examines the psychometric properties of a comprehensive assessment tool, the Scrupulosity Inventory (SI). METHODS: The SI, along with other measures of obsessive-compulsive disorder (OCD) and perfectionism, were administered to a sample (N = 150) of college undergraduates similar in size to other scale development studies of related measures. We conducted exploratory and confirmatory factor analyses of the SI, examined its convergent and diverg

Immediate reactions to the covid-19 pandemic in adults with obsessive-compulsive disorder: a self-report survey.

BACKGROUND: Limited studies are available that investigate the reactions to COVID-19 pandemic by people suffering from obsessive-compulsive disorder (OCD). Due to the nature of the pandemic and the heightened focus on contamination, cleaning, and social distancing, it is likely that a deterioration of OCD symptoms and severity will be seen. AIMS: Our aims were to evaluate (1) self-reported changes in OCD symptom severity of adults with OCD during the COVID-19 pandemic outbreak, (2) whether the COVID-19 pandemic would trigger self-reported contamination symptoms in persons with no history of s

Social, financial and psychological stress during an emerging pandemic: observations from a population survey in the acute phase of COVID-19.

INTRODUCTION: The negative impacts of COVID-19 have rippled through every facet of society. Understanding the multidimensional impacts of this pandemic is crucial to identify the most critical needs and to inform targeted interventions. This population survey study aimed to investigate the acute phase of the COVID-19 outbreak in terms of perceived threats and concerns, occupational and financial impacts, social impacts and stress between 3 April and 15 May 2020. METHODS: 6040 participants are included in this report. A multivariate linear regression model was used to identify factors associat

Automated classification of exposure and encourage events in speech data from pediatric OCD treatment.

OBJECTIVE: To develop and evaluate an automated classification system for labeling Exposure Process Coding System (EPCS) quality codes-specifically exposure and encourage events-during in-person exposure therapy sessions using automatic speech recognition (ASR) and natural language processing techniques. MATERIALS AND METHODS: The system was trained and tested on 360 manually labeled pediatric Obsessive-Compulsive Disorder (OCD) therapy sessions from 3 clinical trials. Audio recordings were transcribed using ASR tools (OpenAI's Whisper and Google Speech-to-Text). Transcription accuracy was ev

Psychopathological features of obsessive-compulsive disorder in an Italian family with Gilles de la Tourette syndrome not linked to the SLITRK1 gene.

We report the psychopathological features in a large Italian family with Gilles de la Tourette syndrome not linked to the SLITRK1 gene. Of the 23 living family members, 14 were evaluated using the Structured Clinical Interview for DSM-IV, the Yale-Brown Obsessive-Compulsive Scale, the Clinical Global Impression and the Global Assessment of Functioning scale. Ten patients were found to have obsessive-compulsive disorder in which tic-like compulsions predominated. The distinctive feature of this family is the high frequency of obsessive-compulsive disorder with various clinical phenotypes.

Building Primary-School Children's Resilience through a Web-Based Interactive Learning Environment: Quasi-Experimental Pre-Post Study.

BACKGROUND: Resilience is a person's mental ability to deal with challenging situations adaptively and is a crucial stress management skill. Psychological resilience and finding ways to cope in crises is a highly relevant topic considering the COVID-19 pandemic, which enforced quarantine, social distancing measures, and school closures worldwide. Parents and children are currently living with increased stress due to COVID-19. We need to respond with immediate ways to strengthen children's resilience. Internet-based cognitive behavioral therapy interventions for children's stress management ove

Age-specific determinants of psychiatric outcomes after the first COVID-19 wave: baseline findings from a Canadian online cohort study.

BACKGROUND: Canadians endured unprecedented mental health (MH) and support access challenges during the first COVID-19 wave. Identifying groups of individuals who remain at risk beyond the acute pandemic phase is key to guiding systemic intervention efforts and policy. We hypothesized that determinants of three complementary, clinically actionable psychiatric outcomes would differ across Canadian age groups. METHODS: The Personal Impacts of COVID-19 Survey (PICS) was iteratively developed with stakeholder feedback, incorporating validated, age-appropriate measures. Baseline, cross-sectional o

Should Reward Deficiency Syndrome (RDS) Be Considered an Umbrella Disorder for Mental Illness and Associated Genetic and Epigenetic Induced Dysregulation of Brain Reward Circuitry?

Reward Deficiency Syndrome (RDS) is defined as a breakdown of reward neurotransmission that results in a wide range of addictive, compulsive, and impulsive behaviors. RDS is caused by a combination of environmental (epigenetic) influences and DNA-based (genetic) neurotransmission deficits that interfere with the normal satisfaction of human physiological drives (i.e., food, water, and sex). An essential feature of RDS is the lack of integration between perception, cognition, and emotions that occurs because of (1) significant dopaminergic surges in motivation, reward, and learning centers caus

Effectiveness of youth psychotherapy delivered remotely: A meta-analysis.

Reports on remote psychotherapies for youth (e.g., technology-based treatment) suggest it is acceptable, feasible, and useful in overcoming logistical barriers to treatment. But how effective is remote care? To find out, PsycINFO and PubMed were searched from 1960 through 2020, supplemented by journal searches and reference trails, to identify randomized controlled trials of youth psychotherapy for anxiety (including obsessive-compulsive disorder and trauma), depression, attention-deficit/hyperactivity disorder (ADHD), or conduct problems, in which all therapeutic contact occurred remotely. Cognitive Behavioral Therapy Reduces Symptom Severity and Normalizes Neurophysiological and Attentional Reactivity in Anorexia Nervosa: A Randomized Controlled Trial.

BACKGROUND: Anorexia nervosa (AN) is a severe psychiatric disorder marked by restrictive eating, distorted body image, and high relapse rates. While cognitive-behavioral therapy (CBT) is a widely used treatment, its mechanisms of action in AN remain incompletely understood, particularly beyond self-reported symptom change. This study investigated the effects of a 12-week CBT intervention on both clinical and multimodal laboratory-based outcomes in women with restrictive-type AN. METHODS: In a two-arm, pre-post randomized controlled trial (ClinicalTrials.gov: NCT07037017), 59 women with restri
