Effect of Tai Chi on cardiac function in patients with myocardial infarction: A protocol for a randomized controlled trial.

BACKGROUND: Myocardial infarction is 1 of the most serious cardiovascular diseases. Early interventional therapy preserves the cardiac function of patients with myocardial infarction to the greatest extent, but it is far from meeting people's need only limited to cardiac revascularization. It is also necessary to help patients improve their quality of life, exercise tolerance, and reduce the incidence of acute cardiac recurrence as much as possible. All these depend on cardiac rehabilitation (CR) are based on exercise. Early and correct CR helps to improve the patient's heart function and impr…

Simple exercise recovery index for sympathetic overactivity is linked to insulin resistance.

UNLABELLED: Exercise HR recovery (HRR) has proven an effective clinical means to assess parasympathetic dysfunction linked to all-cause mortality, but an analogous functional assessment for sympathetic dysfunction has not been developed. PURPOSE: We investigated whether exercise recovery provides additional cardiorespiratory information, beyond the initial HRR period, to index sympathetic overactivity associated with insulin resistance. METHODS: Young people (N = 20) with diverse percent body fat (9%-52%) were studied using fasting, oral glucose tolerance test (OGTT), and high-carbohydrate m

A 65-Year-Old Man With Weight Loss, Peripheral Neuropathy, and Lower Extremity Swelling.

A 65-year-old man with no past medical history sought treatment at the hospital with lower extremity swelling, pain, tingling in a stocking-glove distribution, and syncope. He reported a 23-pound unintentional weight loss. He felt unsteady walking with a couple of falls, and his exercise tolerance was limited to several hundred feet. He did not report vision changes, dysphagia, bowel or bladder problems, tremor, orthopnea, lightheadedness, or chest pain. He did not report any history of substance misuse, high-risk sexual behavior, or concerning exposures. The patient was admitted for further w…

Active and Passive Bioimplants for Vocal Fold Paralysis.

Vocal fold paralysis is one of the diseases that particularly affect quality of life. While unilateral paralysis leads to glottis closure insufficiency and hoarseness, bilateral paralysis compromises respiration and limits the exercise tolerance. Bioimplants have been used to treat persistent paralysis for over 100 years. The spectrum ranges from autologous tissue transfer and resorbable or permanent injection materials to composite thyroplasty implants and active electrical implants for neurostimulation of the larynx. If bioimplants are used in accordance with the recommendations, the quality…

Clinical application of ghrelin for chronic respiratory diseases.

The discovery of ghrelin has resulted in the development of potential therapeutics for cachexia caused by multiple underlying diseases. When chronic respiratory diseases progress to their advanced stages, cachexia often occurs, thereby worsening the patient's prognosis. A small clinical trial that enrolled cachectic patients with chronic respiratory disease revealed that administration of ghrelin improved their nutritional status and exercise tolerance. Short-term administration of ghrelin was found to be safe and tolerated with adverse events, including suffusion, sleepiness, peristalsis, hun…

[Anginal States provoked by psychogenic stimuli. To the problem of psychosomatic reactions in patients with ischemic heart disease].

Two types of vulnerability of coronary vessels damaged by atherosclerosis were proposed basing on results of a study of 94 patients with psychosomatic reactions accompanied with ischemia or myocardial infarction. First variant (35 patients) was characterized by stable course of the disease (high effort angina without coronary catastrophes, episodes of unstable angina, etc.) and selective sensitivity to definite key affect of high personal value. Second variant (49 patients) was distinguished by pronounced clinical manifestations (effort angina with progressive lowering of exercise tolerance). …

Risk factors for non-insulin-dependent diabetes mellitus.

UNLABELLED: RISK FACTORS AND PREDICTORS: The risk factors for non-insulin-dependent diabetes mellitus (NIDDM) include age, overall adiposity, an adverse body fat distribution, adverse glucose and insulin concentrations, insulin resistance and lack of physical exercise. The strongest predictors of NIDDM appear to be metabolic factors, including impaired glucose tolerance and insulin resistance. TREATMENT: Pharmacological agents that improve insulin sensitivity are being tested in multicenter clinical trials. In patients with a high risk of NIDDM, it seems prudent to avoid the use of pharmacolo

[Mitochondrial encephalopathies with late disclosure and predominant involvement of central nervous system].

We report three cases of mitochondrial encephalomyopathy affecting predominantly the central nervous system; two patients had the MELAS syndrome and one had "ophthalmoplegia plus". Histoenzymatic analysis of muscle biopsy and biochemical studies of muscle mitochondria demonstrated myopathy associated with partial deficiency of complex I of the electron transfer chain in three cases, complex IV in two cases and complex III in one case. Molecular analysis of mtDNA in the first case did not revealed any abnormality. Coenzyme Q10 therapy improved exercise tolerance but not the central nervous sign…

The use of internal and external vascular conduits for correction of D-transposition of the great arteries and double-outlet right ventricle with pulmonary atresia.

Between 1967 and 1978, an 11-year-old boy had undergone 3 separate systemic-pulmonary artery shunt operations. Two of the 3 shunt operations failed. The patient was restudied because of progressive cyanosis and was diagnosed as having D-transposition of the great arteries and double-outlet right ventricle with pulmonary atresia. Repair was successfully accomplished with the use of an internal vascular conduit and an external vascular conduit with a glutaraldehyde-treated porcine valve. Four months after the corrective procedure, the patient's exercise tolerance had improved dramatically.

Long-term results of surgical treatment for pulmonary valve stenosis.

109 children who survived surgical treatment for isolated pulmonary valve stenosis were followed for up to 17 years. In all the postoperative status was assessed as satisfactory. Cardiac catheterization repeated in 43 gave a resting valve gradient below 40 mmHg. The 22 children whose pulmonary valves had been excised were as healthy as the 87 who had undergone pulmonary valvotomy. Consideration was given to the desirable length of postoperative review. Except for the few children with symptoms before operation, a postoperative increase in exercise tolerance was not a feature.

Effects of physical activity and exercise interventions in health-related variables in Hodgkin's and non-Hodgkin's lymphoma patients during clinical treatment: a systematic review and single-arm meta-analysis.

INTRODUCTION: Cancer-related fatigue and reduced health-related quality of life (HRQOL) are common among lymphoma patients undergoing treatment. Exercise may be safe and feasible for individuals with hematological malignancies, with potential benefits in mitigating treatment-related toxicity and enhancing treatment tolerance. This systematic review and meta-analysis aimed to evaluate the effects of exercise interventions on health-related outcomes such as HRQOL, muscle strength, cardiorespiratory fitness (CRF), fatigue, and body composition in this population. EVIDENCE ACQUISITION: Electronic

Rapamycin Does Not Compromise Exercise-Induced Muscular Adaptations in Female Mice.

An increasing number of physically active adults are taking the mTOR inhibitor rapamycin off label with the goal of extending healthspan. However, frequent rapamycin dosing disrupts metabolic health during sedentary conditions and abates the anabolic response to exercise. Intermittent once-weekly rapamycin dosing minimizes many negative metabolic side effects of frequent rapamycin in sedentary mice. However, it remains unknown how different rapamycin dosing schedules impact metabolic, physical, and skeletal muscle adaptations to voluntary exercise training. Therefore, we tested the hypothesis

Results of surgical revascularization in ischaemic heart failure without angina.

Seventeen patients with severe ischaemic heart failure without angina were studied prospectively to determine the effects of surgical revascularization on exercise tolerance, peak oxygen consumption and left ventricular function at rest and during inotropic stimulation at 3 months after surgery. Suitability for surgery was assessed by the presence of ischaemia identified by thallium scintigraphy and stress electrocardiographic (ECG) testing and the left ventricular response to dobutamine measured by radionuclide ventriculography. One patient died awaiting surgery and one required cardiac trans…
