Needle Cap External Fixator for Proximal Phalanx Fracture - A Case Report.

INTRODUCTION: Among all the phalanges, most fractured is the proximal phalanx. Frequently encountered complications are malunion, stiffness, and soft-tissue injury which invariably increase the disability. The aim of fracture reduction, therefore, comprises acceptable alignment and the gliding of the flexor and extensor tendons are maintained. Factors affecting management are fracture location, type of fracture, soft-tissue injury, and fracture stability. CASE REPORT: A 26-year-gentlemen, right hand dominant, a clerk by occupation, came to emergency with right-hand index finger pain, swelling

Robust metal ion-chelated polymer interfacial layer for ultraflexible non-fullerene organic solar cells.

Achieving high power conversion efficiency and good mechanical robustness is still challenging for the ultraflexible organic solar cells. In … ompression ratio of 45%. At the end, the ultraflexible cell is demonstrated to be attached onto the finger joint and displays reversible current output during the finger bending-spreading.

Bilateral hand malformations with absence of carpal bones with fusion of proximal metacarpals.

A 14-year-old boy presented with a deformity of his left hand since birth. His parents were nonconsanguineous and his early history was uneventful. The mother was not exposed to any teratogenic drugs during pregnancy. His other siblings are normal. The parents noticed the hand deformity at birth but he had not come to medical attention until he complained of restricted movement of the left hand at the wrist. On examination he did not have any pain on movement of the wrist nor tenderness over the wrist joint. Palmar flexion was restricted completely and active dorsiflexion was restricted in the

Split notochord syndrome with congenital unilateral Horner's sign.

A 2-year-old boy exhibited congenital right Horner's sign and right finger, wrist, and elbow flexion arthrogryposis. He had dyspnea and feeding difficulty 12 hours after birth. Radiologic examination revealed a thoracoabdominal intestinal tube and mediastinal cystic lesion at the right side, with vertebral anomaly at the cervical level. Histopathologically, the intestinal tube was diagnosed as bowel duplication. Because the mediastinal lesion could not be resected surgically, no histopathological diagnosis was made. Embryologically, the combination of transdiaphragmatic duplication, mediastina

[Dupuytren's disease. State of the art and therapeutic perspectives].

Dupuytren's disease is a benign fibroproliferative disorder characterised by the sclerosis and the retractile thickening of the medial palmar aponeurosis and digital aponeurosis, which can lead to the progressive and irreversible finger flexion. Genetic, metabolic and environmental factors are involved in the pathogenesis of Dupuytren's disease, but their role is not clearly established. Different medical therapies have been proposed for Dupuytren's disease treatment, but their real effectiveness has not been shown. Surgical therapy (fasciectomy) is the most used treatment, whose purpose is to

A report of two cases of volar and dorsal fractures of the middle phalanx caused by a basketball.

We present two rare cases of volar and dorsal fractures of the middle phalanx at the proximal interphalangeal joint (PIPJ). The first case was a 16-year-old girl. She hurt her middle finger while playing basketball. A lateral radiograph revealed volar and dorsal fractures of the middle phalanx. She underwent external immobilization of the PIPJ in 30 degrees of flexion with a splint for 3 weeks. The fractures healed 7 weeks after the injury with neither tenderness nor limitation of range of motion. The second case was a 16-year-old boy. He injured his index finger while playing basketball, and

[Reconstruction of first web space of hand: follow-up analysis of 40 cases].

The follow-up outcome of 40 cases after reconstruction of the first web space of hand was reported. Of them, the hand function was improved in 31 cases, not improved in 6 cases, and worsened in 3 cases. The authors thought that the pedicled ilio-inguinal axial flap for treatment of first web space defect is one of the reconstructive methods for choice in spite of its bulky appearance. Though the operative method of the dorsal local skin flap of hand is simple, it may interfere with the flexion of MP joint of the index finger and the reconstructed first web space is very deep and not plumpy. Analysis of distribution and severity of inflammation in patients with osteoarthitis compared to rheumatoid arthritis by ICG-enhanced fluorescence optical imaging and musculoskeletal ultrasound: a pilot study.

BACKGROUND: In rheumatoid arthritis (RA), hand synovitis appears especially in wrist, metacarpophalangeal (MCP) and proximal interphalangeal (PIP) joints. In hand osteoarthritis (OA), potential inflammatory changes are mainly present in PIP and distal interphalangeal (DIP) joints. Joint inflammation can be visualised by fluorescence optical imaging (FOI) and musculoskeletal ultrasound (US). OBJECTIVE: Comparison of the amount and distribution of inflammatory signs in wrist and finger joints of the clinically dominant hand in patients with OA and RA by FOI and gray-scale (GSUS) and power Doppl…

Limited joint mobility in the diabetic foot: relationship to neuropathic ulceration.

Limited joint mobility in the hand is a common manifestation of diabetes with the reported prevalence in insulin-dependent diabetes varying between 8 and 43%. Sixty-two subjects were studied in three groups (controls, diabetic patients without foot problems, and diabetic patients with neuropathic ulceration) to determine whether similar changes occur in the joints of the foot and to examine any possible relationship with neuropathic ulceration. There was a significant impairment of mobility in the range of motion of the sub-talar joint in diabetic patients with ulcers when compared with contro

Severe granulomatous arthritis due to spinous injury by a "sea mouse" annelid worm.

A case of destructive arthritis and soft tissue granulomatous inflammation occurred in a 25 year old man who had injured his right index finger while snorkelling in the Mediterranean. It was initially thought that he had fallen on a sea-urchin. He removed some spines at the time of injury but the finger became stiff, swollen, and painful, and after eight months with no symptomatic improvement amputation through the proximal phalanx was performed. Examination showed an exuberant granulomatous and foreign body type inflammation in the dermis and subcutaneous tissues and affecting the bone, with

Joint mobility in adult patients with groin hernias.

The basic mechanisms of hernia formation remain mostly unknown, but several studies suggest that a connective-tissue pathology, affecting mainly the collagen metabolism, could play a role in the genesis of groin hernias. It would be interesting to know if this pathology can express some clinical signs other than the hernia. Our study focused on the joint mobility and the diagnostic criteria for benign joint hypermobility syndrome. Sixty male adult patients with inguinal hernias and 62 control subjects without hernias, age-matched, were compared, taking into account anamnestic criteria (family

Ligament stability of the metacarpophalangeal joint: a biomechanical study.

Capsuloligamentous contributions to metacarpophalangeal joint stability were obtained from load-displacement tests on the joints of 15 human cadavers. The contribution of each ligament to joint stability was obtained by measuring the reduction of load that induced a given joint displacement after sequential ligamentous sectioning. Four modes of joint displacement were examined: distal distraction, dorsopalmar dislocation, abduction-adduction rotations, and supination-pronation rotations. This study demonstrated that both radial and ulnar collateral ligaments play primary roles in stabilizing t

Assessing ulnar instability of the metacarpophalangeal joint of the thumb.

Seven hundred and fifty normal thumbs were examined to determine the normal range of radioulnar mobility of the metacarpophalangeal joint with the joint in full extension, 15 degrees of flexion, and full flexion. Full flexion was the position of greatest stability. Twenty-five adult unembalmed cadaver thumbs were examined to determine the optimal position for testing the metacarpophalangeal joint for ulnar collateral ligament stability. When radial stress was applied to the metacarpophalangeal joint of 20 specimens after cutting the adductor aponeurosis and capsule, minimal ulnar instability w

Congenital Insensitivity to Pain Syndrome With Hidrosis in a 17-Year-Old Female.

Congenital insensitivity to pain syndrome (CIP) is a rare hereditary sensory and autonomic neuropathy (HSAN) characterized by impaired pain perception, recurrent injuries, chronic infections, and varying degrees of autonomic involvement. Among its subtypes, HSAN-V is distinguished by preserved cognition and sweating function but with a complete absence of pain sensation and progressive joint degeneration. We present the case of a 17-year-old female with suspected HSAN-V who developed orthopedic complications, including a chronic foot ulcer and an infected finger wound. Imaging revealed soft ti

[Treatment of recurrent Dupuytren's disease].

BACKGROUND: The tendency of recurrence or progression is a frequent problem in Dupuytren's disease. The management of recurrence is adapted to the individual situation and the patient's needs. In mild cases a non-operative approach is recommended. Revision surgery is reserved for disabling situations with acceptable circulation and sensation in absence of dystrophy. It is complicated by a combined formation of scar tissue and new cords. This increases the risk of soft tissue loss and injuries to the neurovascular bundles, which impair sensation and circulation and may result in loss of the fin

Technical considerations in two-stage functioning free muscle transplantation reconstruction of both flexor and extensor functions of the forearm.

We report on two patients with severe injuries of the forearm who were reconstructed using functioning free muscle transplantation (FFMT) for individual replacement of flexor and extensor function. In both cases a two-stage procedure was performed: The extensor reconstruction preceded the flexor reconstruction by 4-6 months. The extensor digitorum communis and flexor digitorum profundus were successfully reconstructed in both cases using bilateral gracilis FFMT. In one case the flexor pollicis longus and extensor pollicis longus were also reconstructed using the adductor longus in addition to

Hypertrophic osteoarthropathy secondary to bronchopulmonary cancer (our experience).

During 5 years (1986-1990), 81 of the patients under our care showed bronchopulmonary cancer diagnosed by clinical, x-ray and bronchoscopic examinations. Of these, 69 (85.2) were men and 12 (14.8%) women, with a mean age of 62 years; 72 patients (90%) were moderate or heavy smokers. Four of them (4.9%) presented the classical hypertrophic osteoarthropathy (HOA) described by Bamberger and Pierre Marie, i.e., finger clubbing, ossifying periostitis of the long bones, joint pains and swelling. Other 3 patients showed only finger clubbing, while 7 of 30 cases studied by systematic x-ray examination

Ligamentous sports injuries of the hand and wrist.

Athletic injuries of the hand and wrist continue to increase as athletes get bigger, faster, and the weekend warrior continues to be active throughout his or her lifespan. The management of hand and wrist injuries in these athletes depends on the sport, position, timing during the season the injury occurs, and how many years the athlete has left to play in competition. In the past, these injuries were thought to be trivial; but if not ideally managed, complications can occur which may keep the skilled athlete or the weekend warrior out for a prolonged period of time. This article concentrates
