[Genetic basis for skeletal disease. Molecular advances in sclerosing bone disorders].

Sclerosing bone disorders are caused by impaired osteoclastic bone resorption or increased bone formation. Osteopetrosis, a representative disease caused by impaired bone resorption, is a heterogeneous disease, and various molecules have been recently identified to be responsible. In infantile malignant osteopetrosis, there are osteoclast-rich and osteoclast-poor forms, which are caused by dysfunction of osteoclasts and impaired osteoclastogenesis, respectively. As to the sclerosing bone diseases related to the increased bone formation, molecular analyses of these disorders uncovered the invol

Biomechanical analysis of functional adaptation of metatarsal bones in statically deformed feet.

We analysed the functional adaptation of the first and second metatarsal bones to altered strain in flexible flatfoot. Fifty consecutive women (20-40 years of age) were enrolled: 31 patients with a flexible flatfoot and metatarsalgia (59 feet) and 19 controls with asymptomatic feet (37 feet). They were compared for cortical thickness (medial, lateral, dorsal and plantar) of the two bones. The null hypothesis of no overall difference between the deformed and healthy feet with regard to cortical thicknesses of the two bones was rejected in a multivariate test (p = 0.046). The groups differed sig

A proposed imaging scoring system to differentiate dural-based metastasis from meningioma using MR and CT images.

BACKGROUND AND PURPOSE: The differentiation between dural-based metastasis and meningioma, which is the most common benign extra axial tumor … edema, cystic changes, dural tail, leptomeningeal enhancement, calcifications, bone destruction and hyperostosis, were evaluated. The efficacy of the proposed imaging method for meningioma and its benign findings was evaluated. RESULTS: There was a significant difference in most of the imaging features between patients with meningiomas and those with dural-ba …

Perioperative risk stratification of spine trauma patients with ankylosing spinal disorders: a comparison of 3 quantitative indices.

OBJECTIVE: Patients with ankylosing spinal disorders (ASDs), including ankylosing spondylitis (AS) and diffuse idiopathic skeletal hyperostosis (DISH), have been shown to experience significantly increased rates of postoperative complications. Despite this, very few risk stratification tools have been validated for this population. As such, the purpose of this study was to identify predictors of adverse events and mortality in ASD patients undergoing surgery for 3-column fractures. METHODS: All adult patients with a documented history of AS or DISH who underwent surgery for a traumatic 3-colu…

Instability of the spine is a complex clinical entity that exists on a wide spectrum encompassing many aspects of spinal pathology including … uled out. Certain clinical entities, such as ankylosing spondylitis and diffuse idiopathic skeletal hyperostosis, are at high risk for instability particularly in the traumatic setting. In these situations, the spine should be considered unstable until proven otherwise. Ultimately, the determination of spinal stability, and subsequent need for surgical treatme …

A 66-year-old Japanese woman who was diagnosed with synovitis-acne-pustulosis-hyperostosis-osteitis (SAPHO) syndrome presented with bilateral blurred vision 4 months prior to visiting our hospital. She had visited a local ophthalmology clinic first. She was diagnosed with conjunctivitis and was prescribed antibacterial eye drops. The symptoms persisted in spite of treatment. She was then referred to our hospital. At her initial visit, the visual acuities were 0.6 in both eyes. A slit-lamp examination revealed bilateral shallow anterior chamber, and intraocular pressures of 18 mm Hg in the righ…

In cases of exposed bone and/or mucosal stripping, hyperostosis at the recipient graft site was avoided. All the patients had complete healing at the donor site and the recipient site, with minimal morbidity at 5 and 12 weeks, and no evidence of recurrent CSF leak. Some patients may have overlap with osteopoikilosis or Buschke-Ollendorff syndrome. Analysis of the Literature on Cervical Spine Fractures in Ankylosing Spinal Disorders.

STUDY DESIGN: Narrative literature review. OBJECTIVE: The numbers of low-energy cervical fractures seen in patients suffering from ankylosing spondylitis (also known as Bechterew disease) or diffuse idiopathic skeletal hyperostosis (also known as Forestier disease) have greatly increased over recent decades. These fractures tend to be particularly overlooked, leading to delayed diagnosis and secondary neurological deterioration. The aim of the present evaluation was to summarize current knowledge on cervical fractures in patients with ankylosing spinal disorders (ASDs). METHODS: The literatu…

From misdiagnosis to definitive diagnosis of SAPHO syndrome during a routine health check-up: a case report and literature review.

BACKGROUND: SAPHO syndrome is a rare chronic autoinflammatory disease with heterogeneous clinical manifestations. Because there are no specific diagnostic biomarkers, the condition is often diagnosed late or misdiagnosed. CASE PRESENTATION: A 51-year-old man presented with unexplained chest pain and low back pain. He had been evaluated at several hospitals and had received different diagnoses related to bone disease. During a routine health check-up, imaging revealed multiple osteolytic lesions in the sternum, together with sclerosis and hyperostosis involving the sternoclavicular and sternoc…

Skeletal abnormalities caused by a Connexin43R239Q mutation in a mouse model for autosomal recessive craniometaphyseal dysplasia.

Craniometaphyseal dysplasia (CMD), a rare craniotubular disorder, occurs in an autosomal dominant (AD) or autosomal recessive (AR) form. CMD is characterized by hyperostosis of craniofacial bones and metaphyseal flaring of long bones. Many patients with CMD suffer from neurological symptoms. The pathogenesis of CMD is not fully understood. Treatment is limited to craniofacial surgery. Here, we report a knock in (KI) mouse model for AR CMD carrying a Cx43R239Q mutation. Cx43KI/KI mice replicate typical features of AR CMD, including thickening of craniofacial bones, club-shaped femurs, and widen…

Biomechanical comparison of pedicle screw fixation strength between traditional trajectory and the single and double endplates penetrating screw trajectories in cadaver spine.

The purpose of this study is to compare the peak insertion torque (PIT) and pullout force (POF) of traditional trajectory (TT) and SEPST/DEPST using cadaveric spine. METHODS: The eighth thoracic vertebra to the fifth lumbar vertebra from six formalin-fixed adult human cadavers (two males, four females; age at death …
