Longitudinal association between sleep and Alzheimer's pathology.

INTRODUCTION: Since sleep disturbance is a modifiable risk factor for Alzheimer's disease (AD), we tested associations between sleep and AD pathology in cognitively unimpaired (CU) persons. METHODS: We included 223 participants from the PREVENT-AD cohort with self-reported measures of sleep, objective actigraphy measures of sleep, and positron emission tomography (PET) scans for AD pathology quantification. Repeated PET scans (mean follow-up: 4.31 ± 0.55 years) were available for 103 participants. We conducted robust linear models (RLM) for cross-sectional analyses and RLMs using the annual c…

Digital pathology in urology: the new frontier?

Systems of digital pathology are used to enhance diagnostic consistency and reduce workload. They cannot replace pathologists but can serve as tireless and intelligent assistants. Artificial intelligence (AI) excels at high-volume and repetitive tasks, freeing pathologists to focus on complex cases and final diagnostic decisions. The other advantages of digitization include reducing interobserver variability, use as second reader tools and for triage negative biopsies. Digital images can be used for remote consultation, external quality assessment, and education. Despite these favorable result…

Extracellular Vesicles in Reproductive Physiology and Pathology.

Extracellular vesicles (EVs) are lipid-bound nanostructures that play important roles in reproduction as universal mediators of bidirectional cell-to-cell communication. EVs transfer diverse cargoes between reproductive cells, influencing fundamental reproductive processes such as gametogenesis and pregnancy. In the male reproductive tract, specialized EVs such as epididymosomes and prostasomes regulate sperm activation, motility, and capacitation, exerting some of these effects within the female reproductive tract as well. In the female reproductive tract, EVs found in follicular fluid, ovidu

[Pathology and Assessment of Peripheral Autonomic Neuropathy].

The peripheral autonomic nervous system predominantly comprises unmyelinated fibers, which are difficult to evaluate using conventional light microscopy. Historically, morphological assessment of unmyelinated fibers relied on the electron microscopy of sural nerve biopsy specimens, which is a technically demanding approach with limited accessibility to autonomic structures. Skin biopsy with immunohistochemical staining for the pan-axonal marker PGP 9.5 has enabled minimally invasive and quantitative evaluation of somatic small (intraepidermal nerve fiber density) and autonomic fibers innervati

Digital twin manifesto for the pathology laboratory.

This manuscript presents a manifesto developed by a multifaceted board of stakeholders aimed at guiding the implementation of Digital Twin (DT) technology in pathology laboratories. DTs, already transformative in other sectors, hold substantial promise for enhancing operational efficiency, diagnostic accuracy, and quality of care in pathology. We provide a comparative analysis of traditional versus DT-enhanced workflows across critical steps including accessioning, grossing, processing, embedding, cutting, staining, scanning, diagnosis, and archiving. The framework highlights measurable gains …

Calcified Nodules: Pathology, Mechanisms, and Clinical Challenges.

PURPOSE OF REVIEW: Calcified nodules (CNs) account for ~ 5% of acute coronary syndrome cases. Although the progression of calcification generally leads to plaque stability, CNs ─ an advanced form of calcified plaques ─ can trigger coronary thrombosis and sudden cardiac death. RECENT FINDINGS: CNs are histologically defined as lesions with fibrous cap disruption and luminal thrombus, associated with dense, eruptive calcific nodules protruding into the lumen. They are linked to poor prognosis both post percutaneous coronary intervention and in non-culprit lesions. Interventionalists are explori

Understanding Duchenne muscular dystrophy-associated brain pathology.

The most common neuromuscular disorder, Duchenne muscular dystrophy (DMD), is caused by mutations in the DMD gene, resulting in a lack of dystrophin. In addition to severe and progressive muscle wasting, a subset of individuals with DMD experience, to largely varying extents, behavioural and cognitive deficits, including a lower IQ, and neurological comorbidities, such as autism spectrum disorder, obsessive compulsive disorder and attention deficit hyperactivity disorder. Neuroimaging studies in individuals with DMD have identified widespread pathology, including structural, physiological and …

Imaging of the Globe: Anatomy and Pathology.

Advancements in computed tomography and MR imaging have greatly improved the detection of ocular abnormalities, necessitating a systematic diagnostic approach. Understanding globe anatomy and recognizing imaging features of both traumatic and non-traumatic conditions are essential for accurate diagnosis and appropriate referral. Awareness of incidental findings, such as degenerative changes or implants, helps avoid unnecessary investigations. This article highlights common pathologic processes affecting the various anatomic layers of the globe, illustrated through case examples, and aims to fa

New insights into diabetes-induced cardiac pathology.

Individuals with diabetes have an elevated risk of heart disease, and there is a significant clinical need for evidence-based treatments. Heart disease in diabetes manifests as a distinct cardiopathology, with cardiac structural and functional remodeling underlying increased susceptibility to cardiac dysfunction and arrhythmias. An understanding of the mechanisms associated with cardiac vulnerability in diabetes is incomplete, but recent studies have advanced new insights into the roles of metabolic disturbances, gene dysregulation and epicardial adipose influence. This perspective article hig…

[Fundamentals in the pathology of testicular tumours].

The most common group of testicular tumours comprises germ cell tumours. Other primary testicular tumours are rare, but it is important to be aware of the wide variety of other, much rarer testicular tumours for the differential diagnosis. These tumours include sex cord stromal tumours and testicular adnexal tumours, which must be distinguished from metastases or somatic-type malignancies in germ cell tumours. Immunohistochemical markers and molecular alterations can help to correctly diagnose these tumours.

Pondering the problem of peri-implant pathology.

DATA SOURCES: Randomised control trials were identified via searches of PubMed, Embase, Web of Science and The Cochrane Library. STUDY SELECTION: Only randomised control trials involving adults with at least a single dental implant which had experienced peri-implant mucositis or peri-implantitis were considered. Within studies, one cohort must have been treated with a combination of mechanical debridement (MD) and non-surgical strategies, and the other solely by mechanical debridement. Outcomes were measured using periodontal indices such as bleeding on probing, and each group needed to compr

Advances in semantic dementia: Neuropsychology, pathology & neuroimaging.

Semantic dementia is a kind of neurodegenerative disorder, characterized by prominent semantic impairments and anterior temporal lobe atrophy. Since 2010, more studies have devoted to this rare disorder, revealing that it is more complex than we think. Clinical advances include more specific findings of semantic impairments and other higher order cognitive deficits. Neuroimaging techniques can help revealing the different brain networks affected (both structurally and functionally) in this condition. Pathological and genetic studies have also found more complex situations of semantic dementia,

[Fluorescence confocal microscopy-complete digitization of pathology].

BACKGROUND: Fluorescence-based confocal microscopy (FCM) can be used to create virtual H&E sections in real time. So far, FCM has been used in dermato-, uro-, and gynecopathology. FCM allows the creation of a completely digitized frozen section, which could potentially replace conventional frozen sections in the future. OBJECTIVE: The aim of the current work is to implement FCM technology as a component of fully digitized processes in the pathological workflow. For this purpose, the current use of FCM in liver transplant pathology will be extended to other disciplines such as urology and otor…
