Increased risk of high-grade dermatologic toxicities with radiation plus epidermal growth factor receptor inhibitor therapy.

BACKGROUND: The addition of epidermal growth factor receptor (EGFR) inhibitors to radiotherapy has produced increased locoregional control and has reduced mortality from various solid tumors with few additional toxicities. Although anecdotal reports have suggested increased radiation dermatitis, the overall effect of these regimens on dermatologic toxicities has not been ascertained. METHODS: Dermatologic toxicity data were analyzed from abstracts presented at the annual meetings of the American Society of Clinical Oncology, the American Society of Therapeutic Radiology and Oncology, Cochrane

[Radiotherapy of basal cell carcinoma of the face and head: Importance of low dose per fraction on long-term outcome].

BACKGROUND: Radiotherapy plays an important role in the treatment of basal cell carcinoma of the face and head and achieves cure rates of 92-96 %. Different fractionation concepts of radiotherapy have been described. This study investigates the efficacy, as well as acute and chronic toxicity, of a slightly hypo-fractionated radiotherapy schedule. PATIENTS AND METHODS: 85 patients with 104 tumors underwent radiotherapy for basal cell carcinoma of the face and head. The radiotherapy schedule was 5 x 3 Gray/week up to a total dose of 57 Gray in 95 % of patients. Acute and late radiotherapy toxic

[Analysis of variation in delivered dose in diagnostic and therapeutic cerebral angiography].

OBJECTIVES: Analysis of dose-area product (DAP) in cerebral angiography, according to the type of pathology and technique used. MATERIALS AND METHODS: Use of dosimetric and clinical data resulting from 84 diagnostic cerebral angiograms and 32 cerebral embolizations. RESULTS: The diagnostic angiography data were divided into three categories: A/ When a secondarily embolized aneurysm was diagnosed: n=17. B/ When the final diagnosis was an arteriovenous malformation (AVM): n=10. C/ In the case where the diagnosis was neither AVM, nor aneurysm to embolize: n=57. The cerebral embolizations were c

Epidermal localization and protective effects of topically applied superoxide dismutase.

Data from the literature, as well as our previous work, indicate a protective effect of superoxide dismutase (SOD) in topical application against UV-induced cutaneous damage. In the present article we show that pre-treatment of the skin with SOD protects against PUVA-induced inflammatory reactions not only in murine, but also in human skin. Using fluorescently labelled Cu,Zn SOD, epifluorescence microscopy and digital image processing, we demonstrate that the FITC fluorescence localizes in the stratum corneum and upper granulosa, as well as in the epidermal cell layer surrounding the lumina of

[Plastic-surgical therapy of extensive radiogenic recurrence of pharyngo-esophagostoma].

Reconstruction of extended post-irradiation pharyngo-esophagostomy is still a formidable challenge in head and neck surgery. Early rehabilitation of speech, swallowing and chewing and prevention of severe hemorrhage of the carotid artery by soft tissue coverage is mandatory. If previous attempts with local lipocutaneous and myocutaneous flaps have failed, a two stage reconstruction by microsurgical transfer of a jejunal patch in combination with a myocutaneous latissimus dorsi flap will lead to sufficient lining and a permanent closure of the defect. The indication and the technical procedures

From clinic to couch: a pilot study of home-use photobiomodulation for radiation-induced oral mucositis and dermatitis.

PURPOSE: Radiation-induced oral mucositis (OM) and dermatitis (RD) are common debilitating toxicities of radiotherapy (RT) for head and neck cancer (HNC) patients. While photobiomodulation (PBM) has shown promise in reducing severity of these adverse effects, in-clinic treatments pose logistic and financial challenges. This study assessed the feasibility and compliance of a home-use, self-applied near-infrared PBM device for the prevention and treatment of RT-induced OM and RD. METHODS: This prospective, single-arm trial enrolled 20 HNC patients scheduled for intensity-modulated RT (IMRT) wit

Non-invasive quantitative measures of qualitative grading effectiveness as the indices of acute radiation dermatitis in breast cancer patients.

BACKGROUND: Recent improvement of machinery evaluation for the skin changes in various therapies enabled us to evaluate fine changes quantitatively. In this study, we performed evaluation of the changes in radiation dermatitis (RD) using quantitative and qualitative methods, and verified the validity of the conventional qualitative assessment for clinical use. METHODS: Forty-three breast cancer patients received conventional fractionated radiotherapy to whole breast after breast-conserving surgery. Erythema, pigmentation and skin dryness were evaluated qualitatively, and biophysical parameter

Feasibility and acute toxicity of hypofractionated radiation in large-breasted patients.

PURPOSE: To determine the feasibility of and acute toxicity associated with hypofractionated whole breast radiation (HypoRT) after breast-conserving surgery in patients excluded from or underrepresented in randomized trials comparing HypoRT with conventional fractionation schedules. METHODS AND MATERIALS: A review was conducted of all patients consecutively treated with HypoRT at the University of Chicago. All patients were treated to 42.56 Gy in 2.66 Gy daily fractions in either the prone or supine position. Planning was performed in most cases using wedges and large segments or a "field-in-

Acute and chronic radiation injury.

INTRODUCTION: Although all areas of the body are susceptible to radiation injury, different tissues have varying tolerances for radiation exposure. The goal of this summary is to introduce basic concepts of radiation biology and discuss the effects of radiation on various tissues. METHODS: Reference texts and literature were reviewed to summarize key points in radiation biology and the direct and indirect cell damage caused by radiation. RESULTS: The most prevalent factor for injury is long exposure time, which can be an issue in lengthy peripheral vascular or aortic interventions. Several k

Isomorphic cutaneous graft-versus-host disease reaction after ultraviolet exposure: clinical, histological and direct immunofluorescence studies of four allo-transplanted patients.

BACKGROUND: Acute and chronic graft-versus-host disease (GVHD) continues to be a major limitation to successful haematopoietic stem cell transplantation. If experimental studies and clinical observations could partially elucidate the pathophysiology of acute GVHD, the biology of chronic GVHD is still much less well understood. OBJECTIVES: The aim of this study is to describe a peculiar photoinduced rash which triggered acute and then chronic lesions of GVHD in four allogenic haematopoietic-transplanted patients and discuss the possible aetiology and treatment. PATIENTS/METHODS: Four patients
