INTRODUCTION: Orthostatic tremor is an infrequent movement disorder characterized by a high-frequency tremor manifesting primarily in the standing position. This condition can lead to relevant restrictions of mobility in everyday life and adversely affect the quality of life. The etiology has not been conclusively clarified. To date, there are few therapy studies of sufficient quality.
AIM: The aim of the present literature analysis is to systematically evaluate the existing evidence of pharmacological therapies for orthostatic tremor.
MATERIALS AND METHODS: This study searched for publicati

BACKGROUND: Exercise has been proven to significantly enhance balance performance in healthy older adults, particularly those living independently in community settings. However, it remains unclear which specific types of exercise are the most effective and which modality yields the best intervention outcomes. We aimed to evaluate the effects of different exercise modalities on four balance-related outcomes-Berg Balance Scale (BBS), Timed Up and Go Test (TUG), gait speed (GS), and one-leg stance with eyes open (OLSEO)-in healthy older adults, using a systematic review and network meta-analysis

Double-blind, randomized, controlled trials (DB-RCT), if designed and conducted well, are widely considered the gold standard in medical research for purposes of establishing causal efficacy. Their logic is compelling: by balancing out all confounding variables through the research design, DB-RCTs are thought to reveal whether a proposed treatment-by virtue of its characteristic constituents-causes therapeutic effects. Many studies on psychedelic-assisted therapy (PAT) follow this ostensible gold standard and use a DB-RCT design. But several authors have already noted that conducting psychedel

BACKGROUND: Nasotracheal intubation is associated with a risk of epistaxis. Decongestion of the nasal mucosa reduces the risk of epistaxis, and different vasoconstrictors may be used. Cocaine has both decongestive and analgesic properties, but it also has side effects. In this systematic review, we aimed to evaluate if cocaine decreases the occurrence and severity of epistaxis when administered topically to the nasal mucosa before nasotracheal intubation.
METHODS: We conducted a systematic review and meta-analysis following the PRISMA guidelines based on a predefined protocol. We included ran

Despite substantial advancements in the management of cardiogenic shock, mortality rates remain greater than 40%. Trials have shown that increasing survival rates in cardiogenic shock is challenging. Even the most successful trials show 5-15% reductions in mortality, and gains have been restricted to acute myocardial infarction cardiogenic shock, representing approximately 5% of the population with cardiogenic shock. Trials studying pharmacological strategies in all populations with cardiogenic shock have been consistently neutral or negative. The reasons are complex and include heterogeneity

Postpartum hemorrhage (PPH) remains the leading cause of preventable maternal mortality despite standard interventions. Recent fibrinogen trials failed to improve outcomes, prompting interest in coagulation factor XIII (FXIII). FXIII functions as "molecular cement," cross-linking fibrin and stabilizing clots. During pregnancy, FXIII activity decreases 20%-30%, with further depletion during PPH. Observational studies show low antepartum FXIII predicts bleeding risk, while ex vivo supplementation restores clot firmness. The SWIFT trial (NCT06481995) represents the first randomized controlled tri

The protocol of a randomised trial is the foundation for study planning, conduct, reporting, and external review. However, trial protocols vary in their completeness and often do not address key elements of design and conduct. The SPIRIT (Standard Protocol Items: Recommendations for Interventional Trials) statement was first published in 2013 as guidance to improve the completeness of trial protocols. Periodic updates incorporating the latest evidence and best practices are needed to ensure that the guidance remains relevant to users. We aimed to systematically update the SPIRIT recommendation

Prosthetic joint infections (PJI) remain among the most devastating complications in orthopedic surgery, with increasing incidence paralleling the growth in arthroplasty procedures worldwide. While treatment protocols are well-established, evidence supporting current approaches is lacking, and outcomes remain suboptimal, highlighting the need for improved therapeutic strategies. The objective of this study is to review ongoing and recently completed randomized controlled trials (RCTs) investigating novel approaches to PJI prevention and treatment. We searched PubMed and ClinicalTrials.gov data

Randomized prospective studies demonstrate clear benefits for operative versus nonoperative management of symptomatic lumbar spondylolisthesis, but, despite the current randomized controlled trial (RCT) evidence, there remains no universally accepted surgical treatment. The challenge inherent to an RCT is the impact nonadherence presents to evaluating a treatment effect in spinal procedures. Prospective registry data reflect the current practice of spine surgery. While not randomized, the registry data are continuous and available potentially in orders of magnitude greater than RCT.
