Thirty-two consecutive patients who had routine x-ray films were examined 12 days after wrist injury because of persistent signs and symptoms of wrist derangement. All patients had the injured and contralateral uninjured wrist examined by fluoroscopy and a scapholunate dissociation was observed in three patients. This study shows that the diagnosis of scapholunate dissociation cannot always be established by routine x-ray examination. The diagnosis should not be based solely on the size of the scapholunate gap in the injured wrist, but on the difference in the size of the gaps between the inju

BACKGROUND: Intraosseous ganglion is a cystic lesion that contains gelatinous material, most often occurs in middle-aged patients, and is regarded as similar to soft-tissue ganglion. The etiology is unknown, but association with degenerative joint disease has been considered.
MATERIALS AND METHODS: At a single institute, 17 patients (8 men, 9 women) with a mean age of 48.9 years (22-72 years) were surgically treated for an intraosseous ganglion. The lesions were located in 9 long bones (5 tibiae, 2 humeri, 1 ulna, and 1 femur); 4 flat bones (2 scapulae, 2 ilia); and 4 small bones (2 scaphoid,

PURPOSE: Percutaneous scaphoid fixation (PSF) is growing in popularity as a treatment option for non-displaced fractures. Success of this procedure demands high-precision screw placement, which can be difficult to achieve with standard 2D imaging. This study aimed to develop and test a system for computer-assisted navigation using volume slicing of 3D cone-beam computed tomography (CBCT).
METHODS: The navigated technique involved a distinctive workflow in which a 3D CBCT imager was calibrated preoperatively, circumventing the need for intraoperative patient-based registration. Intraoperativel

Thus Four-dimensional computed tomography (4DCT), dynamic imaging, is a promising innovative imaging technique that can improve the capability for diagnosis of the complex wrist joint. It extends conventional three-dimensional CT by incorporating the temporal dimension; this allows visualization of the moving wrist in just a few seconds, resulting in a fast, non-invasive and affordable method to assess and quantify kinematics. The current challenge to optimize the 4DCT workflow include improvements in scan acquisition, reconstruction, postprocessing and image analysis, and the development of a

BACKGROUND: Pediatric scaphoid fractures can be challenging to diagnose on plain radiograph. Rates of missed scaphoid fractures can be as high as 30% to 37% on initial imaging and overall sensitivity ranging from 21% to 97%. Few studies, however, have examined the reliability of radiographs in the diagnosis of scaphoid fractures, and none are specific to the pediatric population. Reliability, both between different specialists and for individual raters, may elucidate some of the diagnostic challenges.
METHODS: We conducted a 2-iteration survey of pediatric orthopedic surgeons, plastic surgeon

BACKGROUND: An irreparable scaphoid proximal pole is challenging to treat because of the fragment size and limited blood supply. Salvage surgery, such as partial wrist fusion or proximal row carpectomy, may be performed but is not ideal for young patients. There are few reports of proximal scaphoid reconstruction using rib osteochondral grafts.
METHODS: Four patients were treated with rib osteochondral graft for reconstruction of the scaphoid proximal pole. The patients had a mean postoperative follow-up of 24 months. The mean age at the time of surgery was 30 years. Outcome measurements incl

BACKGROUND: Despite surgical fixation, the scaphoid nonunion rate remains at 3% to 5%. Recent biomechanical studies have demonstrated increased stability with 2-screw constructs. The objective of our study is to determine the preliminary union rate and anatomic feasibility of 2-screw surgical fixation for scaphoid fractures.
METHODS: This study is a retrospective case series of 25 patients (average age 32 years) with scaphoid fractures treated with 2 parallel headless compression screws (HCS). Postoperative evaluation included Mayo Wrist Score (MWS), range of motion, time to union, and return

BACKGROUND: Abnormal scaphoid alignment after fracture is used as an indication for fixation. Acceptable alignment after reduction and fixation of scaphoid fractures is not well defined. We systematically reviewed the literature to identify how scaphoid malunion is currently defined and by what parameters.
METHODS: A systematic review was performed using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. Multiple databases were searched for studies published in the English language that reported on outcomes after scaphoid malunion and included measurements to d

PURPOSE: In nonsurgical treatment for acute undisplaced or minimally displaced scaphoid waist fractures, immobilization of both wrist and thumb can be an option. However, in vivo scaphoid motion during forearm and thumb motion with the wrist immobilized in a cast has not been measured. Therefore, we examined the in vivo kinematics of the scaphoid during forearm and thumb motion with cast immobilization.
METHODS: Ten healthy right wrists of 10 male volunteers were included. These wrists were immobilized in a short-arm spica cast with the thumb in a position of volar abduction and then were sca

Given the limitations of conventional radiography in the diagnosis of suspected scaphoid fractures on presentation, advanced imaging, particularly magnetic resonance imaging, is a useful additional investigation. We carried out a systematic review of the economic evidence for the use of advanced imaging in the management of suspected scaphoid fractures. Fifteen articles were included in the review. Owing to the heterogeneity of study designs, the type and timing of interventions and the economic analyses performed, direct comparisons between the 15 studies were difficult. From a health care pe

In sports, wrist and hand injuries are commonplace. Too often, injuries to these areas can be under-treated and left for further complications to arise. While some injuries to the wrist and hand can be treated conservatively with immediate return to play, others require a more in-depth assessment prior to return to play. This article describes the most common wrist and hand injuries in sport, and provides information related to current treatment approaches.
