BACKGROUND: The guidelines for colonoscopy present withdrawal time (WT) and adenoma detection rate (ADR) as the quality indicator. The purpose of this retrospective study is to analyze the predicting factors with polyp detection rate (PDR) as a surrogate for ADR by using comprehensive health checkup data, and assess the correlation between PDR per each colonic segment and WT, and factors influencing WT.

Chylous ascites is the accumulation of a milk-like peritoneal fluid rich in triglycerides and it is an unusual complication following surgical treatment of colorectal cancer. Conservative management is usually sufficient in patients with chylous ascites after surgery. However, we describe a patient with intractable chylous ascites after laparoscopic anterior resection for sigmoid colon cancer who failed initial conservative treatment. This patient was successfully managed by surgery.

Every complication of the sigmoid stoma means a restriction of patients' quality of life. The complications of 164 patients were analyzed retrospectively (1/1996-5/1999). Indications for creation of a sigmoid stoma were malignomas of the rectum (n = 109). Hartmann's operation with or without sigmoid resection was performed in 55 patients for different reasons. The blind-pouch-syndrome may display a variety of symptoms. But it rarely causes intestinal bleeding. In a patient suffering from anemia of chronic intestinal hemorrhage, a blind-pouch was diagnosed through the search for colon polyps. A hemicolectomy with a side-to-side ileocolic anastomosis was performed 30 years ago, but there were no symptoms of maldigestion or malabsorption. The explorative laparotomy revealed an anastomotic stricture with a lot of coproliths in the ileum-blind-pouch. The histological examination of the resected anastomosis showed an unspecific inflammation of the mucosa.

To evaluate the assumption that inflammatory changes in the rectal wall after treatment of colorectal adenoma with photocoagulation may simulate malignant infiltration, the findings were reviewed in 23 follow-up examinations performed with transrectal ultrasound (TRUS) in 17 patients with benign adenoma. Under conditions of disturbed blood supply, irrespective of the method of anastomosing, the trophicity of tissues in the zone of suture is sharply disturbed. These alterations are especially pronounced after using multi-row suture when healing develops very unfavorably. In hypoxia of tissues a one-row everting suture made by the author's method with a removal of mucosa and preservation of submucosa provides more favorable conditions for healing of the wound in the place of anastomosis. Stercoral obstruction in a young woman with disseminated phaeochromocytoma was diagnosed and treated successfully using an enema of isosmolar iohexol (Omnipaque) and 1% polysorbate 80 (Tween 80) without complication. Surgical intervention was thus avoided. A low osmolality water-soluble contrast medium (iohexol 150 mg I/ml) with a wetting agent (1% Tween 80) was used because a barium suspension would have inspissated, exacerbating the constipation and a hyperosmolar contrast medium might have precipitated a hypertensive crisis and destabilized her critical salt and water balance.
