BACKGROUND: Patients diagnosed as having somatization disorder (SD) who present with a lifetime history of multiple, medically unexplained physical symptoms represent a significant challenge to health care providers. To date, no psychotherapeutic or pharmacologic intervention has been found to produce clinically meaningful improvement in symptoms or functioning of patients with SD. We examined the efficacy of cognitive-behavioral therapy (CBT) for SD.
METHODS: Eighty-four participants meeting criteria for SD were randomly assigned to 1 of 2 conditions: (1) standard medical care augmented by a

Somatic delusional disorder is an uncommon psychiatric disorder that has been typically reported in elderly women. This article reviews the literature and describes a Saudi female patient who presented with novel somatic delusion of turabosis--the conviction that she was continually being covered by sand from sandstorms. Although the systematized delusional convictions remained unshakable, her anxiety and mood symptoms responded to a regimen of tricyclic antidepressants and her delusional beliefs resolved within eight weeks of treatment with risperidone. Sociocultural themes and therapeutic ap

This study assessed the relative strength of the association between abuse, negative parenting style, and somatization in irritable bowel syndrome (IBS) patients. Drawing from preclinical stress physiology and abuse research identifying the family social climate as a frequently stronger and independent determinant of long-term health effects than abuse-specific variables, we predicted that negative parenting behaviors would more strongly correlate with somatization than abuse. Subjects were 81 consecutively evaluated patients, who at baseline underwent psychological testing, measuring perceive

Burning Mouth Syndrome (BMS) or glossodynia is a chronic pain syndrome that mainly affects middle-aged/elderly women. This condition is probably of multifactorial origin, often idiopathic, and its etiopathogenesis remains largely enigmatic. Visible pathologic lesions of the oral mucosa or processes are usually not evident. Etiologic factors that have been reported include several disorders (e. g. hematologic disorders, denture factors, the climacteric, infections, endocrinological, neurological or psychiatric disorders). On the other hand psychological factors, such as anxiety, depression and

1. A 78-year-old female with DDST and pain disorder was treated by clomipramine 20-100 mg/day. The hypochondriacal delusion was completely resolved, while the pains were partially resolved. 2. The SPECT using Xe-133 taken at the early stage of clomipramine treatment, when she still had hypochondriacal delusions, showed markedly reduced rCBF in the temporal and parietal lobes, with predominance on the left hemisphere. Meanwhile, the SPECT taken after resolution of the hypochondriacal delusions showed a marked improvement in the reduced rCBF. 3. This report suggests that DDST has some associatio

With the introduction of the erection-supporting oral drug sildenafil (Viagra) the development of somatic treatments of male sexual dysfunction has reached a temporary peak. The availability of a medicament that is easy to take will result in an increase of the incidence and possibly also the prevalence of erectile disorder. It may even lead to the myth that only a perfect erection is normal. The question is where to draw the line between a normal inconvenience of life and a serious disease. The erection pill will probably be preferred to all other treatments. Therefore, most patients will con

This work presents results of a retrospective and prospective research of 15 persons with a double diagnosis: body dysmorphic disorder, and delusional disorder, somatic type. It was established that these disorders starting in adolescence, occurred in persons who earlier had traits of avoidant and obsessive-compulsive personality or social phobia. In most subjects with dysmorphophobia there was comorbidity of dysthymia or major depression. An improvement in the state of these patients, most often after implementation of complex therapeutic measures: psychotherapy as well as pharmacotherapy wit

This study was designed to investigate the menstrual, psychosexual, psychological and somatic sequelae in a group of women who may be more prone to express regret following postpartum sterilization. The follow-up was conducted by questionnaire at six months and five years following the procedure. Data were available from 242 patient; 76.8% were below the age of 30. Of all patients, 21.9% expressed regrets. About one third had various menstrual disturbances. Patients rated their sex life as generally more enjoyable in many aspects. The most common psychological symptoms were irritability, nervo

Brief dynamic psychotherapy, carried out by an resident in training, can bring significant improvement to a patient who does not necessarily meet ideal selection criteria. S. had experienced early childhood deprivation resulting in adult psychosomatic manifestations of feeling hypoglycemic under pressure of responsibility or during fear of abandonment. He soothed himself with the aid of a bottle of "protein extract," which he carried at all times on his belt. The 21-week therapy resulted in his increased ability to differentiate the sources of his anxiety and allowed him to find new and more m

Somatizing patients present a history of vague, unexplained medical symptoms. This study compared somatizing patients with pulmonary control subjects by using the Diagnostic Interview Schedule (DIS-III-R), the Illness Attitude Scales (IAS), and the Minnesota Multiphasic Personality Inventory (MMPI-2). The groups differed in the number of somatization symptoms reported and in the frequency of somatization disorder diagnoses when the screening criteria were used. The somatizing group obtained higher scores on the bodily preoccupation and hypochondriacal beliefs subscales of the IAS; no differenc

The effect of cognitive therapy (CT) on resistant burning mouth syndrome (BMS) was studied. Thirty patients with resistant BMS after odontological and medical treatment were randomly divided into two equal groups; a therapy group (TG) was treated with CT and an attention/placebo group (APG) served as a control group. The intensity of BMS, which was estimated by the use of a visual analogue scale, was significantly reduced in the TG directly after CT was completed and was further reduced in a 6-month follow-up. The APG did not show any decrease in intensity of BMS. The results of this study ind

A 30 days therapeutical trial with trazodone has been performed in 47 patients suffering from different types of tremor. No significant improvement was detected in patients suffering from parkinsonian syndromes, multiple sclerosis, psycogenic tremor and hyperthyroidism. After the double-blind trial with trazodone, a significant improvement (p less than 0.025) was, instead, evident in 5 out of 6 patients with essential tremor, while the score did not change in the 3 patients with essential tremor who were treated with placebo.

Recurrent abdominal pain in childhood can be caused structurally, functionally, metabolically or psychosomatically. In the neonatal period there occur malformations, in infancy chronic inflammations of bowel as well as obstructions due to adhesions following laparotomies or chronic intussusceptions or volvulus. In pre-school and school-age symptoms of appendicitis, lymphadenitis, Crohn's disease or Colitis ulcerosa occur. But every 8th to 9th child of school-age suffers from functional abdominal pain without structural origin, probably caused by an "irritable colon".

Depressive illness was a common clinical syndrome found in general practice in Hong Kong. The phenomenon of somatization among Chinese depressives in general practice was investigated. Patients clinically identified as depressed were compared with organic patients in terms of their presenting complaints and responses to a symptom checklist. Sleep disturbances, general malaise, pains and aches, dizziness, and menopausal symptoms were the most frequent distrubances that prompted Chinese depressives to seek help in general practice. When directly asked, these patients admitted having various psyc

Although the relationship between psychological trauma and medically unexplained symptoms (MUS) is well established, this relationship is less well understood in people with medically unexplained neurological symptoms. In the present study, we set out to compare people with functional neurological disorders, and organic neurological disorders, in terms of childhood and adulthood traumatic events, traumatic stress, emotional dysregulation and symptoms of depression and anxiety. We have hypothesised that those with functional neurological disorders would be more likely to report childhood and ad

The Lifetime and 6 month DSM-III prevalence rates of mental disorders from an adult general population sample of former West Germany are reported. The most frequent mental disorders (lifetime) from the Munich Follow-up Study were anxiety disorders (13.87%), followed by substance (13.51%) and affective (12.90%) disorders. Within anxiety disorders, simple and social phobia (8.01%) were the most common, followed by agoraphobia (5.47%) and panic disorder (2.39%). Females had about twice the rates of males for affective (18.68% versus 6.42%), anxiety (18.13% versus 9.07%), and somatization disorder
