The triangular fibrocartilage complex: an important component of the pulley for the ulnar wrist extensor.

The extensor carpi ulnaris (ECU) tendon is the only wrist motor tendon that broadly connects with the triangular fibrocartilage complex (TFCC) of the wrist. The goal of this study was to determine the biomechanical effect of the TFCC on the function of the ECU. The effect of avulsion of the TFCC on the changes in mechanics of the ulnar wrist extensor tendon was investigated in 8 fresh-frozen cadaver forearms. Excursion of the ECU tendon was continuously recorded over the functional range of wrist extension and ulnar deviation in intact wrists, wrists with ulnar styloid fractures, wrists with T

Influence of psychomotor skills and innervation patterns on results of latissimus dorsi tendon transfer for irreparable rotator cuff tears.

This investigation was performed to analyze the influence of innervation and psychomotor skills on the outcome of latissimus dorsi transfer. Patients with the 10 best and 10 worst results after latissimus dorsi transfer for irreparable rotator cuff tears were selected. All patients meeting the inclusion criteria (n = 12) were subject to a psychomotor test battery (Motorische Leistungsserie) and electromyographic innervation assessment. There was no statistical difference between the 2 groups preoperatively in terms of the commonly tested factors known to influence the results of this procedure

"Tennis leg": gastrocnemius injury is a far more common cause than plantaris rupture.

We report a typical case of "tennis leg", in which the main finding was a fluid collection between the medial head of the gastrocnemius and soleus muscles. Since the first clinical description of this entity in 1883, the injury has been attributed to rupture of the plantaris tendon. However, recent studies of this condition with sonography and magnetic resonance imaging have shown that most of these cases are actually due to injury to the gastrocnemius and/or soleus muscles, and up to 10% are due to deep venous thrombosis masquerading as muscle injury. The plantaris muscle and tendon are only

Ring finger lumbrical origin strain: a case series with imaging findings.

Lumbrical strain is a relatively unknown hand injury. We report four cases of lumbrical origin strain involving the ring finger flexor tendons. Three patients sustained the injury during rock climbing and one while working with a jackhammer. In all cases, circumferential fluid around the flexor tendons of the ring finger was demonstrated on MRI and/or ultrasound at the distal palmar level at the "bare area," which is normally devoid of a synovial sheath. There is a paucity of information in the literature regarding this injury and its specific imaging features. Lack of awareness of this injury

MRI of the tibioastragalus anticus of Gruber muscle: a rare accessory muscle and normal anatomical variant.

We present the case of a 31-year-old man who sustained a hyperplantar flexion injury of his right ankle, and was evaluated using computed tomography and MRI to assess for osseous and ligamentous injury. The MRI and CT studies demonstrated a tibioastragalus anticus of Gruber (TAAG) muscle in the lower limb's anterior compartment. To our knowledge, the imaging of this muscle has not been previously described. The TAAG muscle arises from the lower third of the anterolateral tibia and the interosseous membrane. Its tendon passes laterally, deep to the tibialis anterior and extensor hallucis longus

Bilateral Chronic Quadriceps Tendon Ruptures Treated with Hamstring Autograft Reconstruction: A Case Report.

CASE: A 30-year-old semiprofessional football player presented with chronic bilateral quadriceps tendon ruptures. Both quadriceps tendon ruptures were unsuitable for isolated primary repair because of tendon retraction and immobility. A novel reconstruction technique using semitendinosus and gracilis tendon autografts was performed to restore both lower extremities' disrupted extensor mechanisms. At the final follow-up, the patient regained excellent motion of both knees and returned to high-intensity activity. CONCLUSION: Chronic quadriceps tendon ruptures present challenges related to tendo

Floating patella: Ipsilateral and simultaneous rupture of the quadriceps and patellar tendons in a young individual (a surgical case report).

INTRODUCTION AND IMPORTANCE: The phenomenon of a floating patella or ipsilateral bifocal rupture of the extensor apparatus is an extremely rare injury, with few cases reported in the literature. The aim of this case study is to report an unprecedented event: the simultaneous trans-tendinous rupture of both quadriceps and patellar tendons in a young adult without predisposing factors, emphasizing the need for awareness in diagnosis and management, and suggesting a new avenue for research in prevention and rehabilitation. CASE PRESENTATION: We report the case of a 35-year-old patient with no si

Spontaneous Rupture of the Extensor Pollicis Longus Tendon: A Systematic Review.

BACKGROUND: Extensor pollicis longus (EPL) rupture and tenosynovitis of the third dorsal compartment is often described in association with a history of rheumatoid arthritis or in the setting of a distal radius fracture. However, the literature suggests multiple other potential factors that may lead to a seemingly spontaneous rupture. METHODS: We performed a systematic review following guidelines set forth by the Preferred Reporting Items for Systematic Reviews and Meta-Analyses statement. The search consisted of headings and keywords related to tendon injuries, tendinopathy, hand surgery, te…

Complications of Supine Surgical Achilles Tendon Repair.

BACKGROUND: Open Achilles tendon surgery with the patient in the supine position potentially avoids the complications of the prone position, but the safety and viability of the supine position for this procedure are not known. The aim of this study was to test the hypothesis that supine positioning for open repair of acute Achilles tendon ruptures would be safe, with low wound and neurologic complication rates. METHODS: Supine position safety in acute Achilles tendon repair was investigated. Consecutive cases of supine Achilles tendon surgical repair performed by one surgeon from 2010 to 2015

Anatomy of the sural nerve in a computer-assisted model: implications for surgical minimal-invasive Achilles tendon repair.

BACKGROUND: Sural nerve injuries are an evident risk especially of minimal-invasive surgical Achilles tendon repair. However, detailed anatomical studies focusing on the relationship of the sural nerve with the Achilles tendon at various levels are scarce, even pending in two planes. AIM: To determine the position and course of the sural nerve in relation to the Achilles tendon in two planes after trans-section and computer-assisted determination. METHODS: The exact course of the sural nerve was determined in 10 cadavers (55.3 years, 19-89 years), using a computer-assisted method in two plan

Low to moderate risk of nerve damage during peroneus longus tendon autograft harvest.

PURPOSE: This study aims to evaluate the proximity of the tendon stripper to both the peroneal and sural nerves during peroneus longus tendon (PLT) autograft harvesting. METHODS: Ten fresh-frozen human cadaveric lower extremities were used to harvest a full-thickness PLT autograft using a standard closed blunt-ended tendon stripper. The distance to the sural nerve from the PLT (at 0, 1, 2 and 3 cm proximal to lateral malleolus (LM), and the distance to the peroneal nerve and its branches from the end of the tendon stripper were measured by two separate observers using ImageJ software. [Transposition of scapular muscles in the treatment of injuries of the rotator cuff of the shoulder (RCS)].

An experience of 13 cases of short rotator muscles advancement is reported. These procedures were found to be available in treatment of rotator cuff avulsions with retraction, when anatomical reinsertion or shoe-lace suture after Mclaughlin appeared to be impossible. The authors had performed advancement of the supraspinatus alone in 10 cases and simultaneous advancement of supra-, infraspinatus and teres minor in 3 cases. Only one poor result was obtained that can't but confirm a most high efficacy of the procedures described.

Irreducible dislocation of the distal radioulnar joint.

Although dislocation of the distal radioulnar joint is commonplace in association with fractures of the radial shaft, irreducible dislocation has previously been considered to be rare. In the only three previously reported cases the tendon of the extensor carpi ulnaris blocked reduction of the distal radioulnar joint in Galeazzi injuries. The cases presented in this report show that other tendons may be involved, and that the injury may occur even when the ulna is broken. It is likely that the injury described is more common than is realized and is usually overlooked.
