Aspergillus and lung transplantation.

BACKGROUND: Aspergillus infection is a known complication of transplantation. METHODS: We describe our experience with 37 patients who received lung transplants over 2 years at Loyola University Medical Center. All patients who had evidence of aspergillus on culture of clinical specimens or had biopsies with hyphal forms consistent with aspergillus were categorized according to the clinical manifestations. Important risk factors were analyzed in comparison with other lung transplant recipients during the same period. RESULTS: The incidence of invasive aspergillosis was high (16%). No patient…

Abdominal multivisceral transplantation.

Under FK506-based immunosuppression, 13 abdominal multivisceral transplantations were performed in 6 children and 7 adults. Of the 13 recipients, 7 (53.8%) are alive and well with functioning grafts after 9 to 31 months. Six recipients died: three from PTLD, one from rejection, one from sepsis, and one from respiratory failure. In addition to rejection, postoperative complications occurring in more than isolated cases included PTLD (n = 6), abdominal abscess formation (n = 5), pancreatitis (n = 3), and ampullary dysfunction (n = 2). In addition, infection by enteric microorganisms was common d

Cardiac transplantation: a review.

Cardiac transplantation is currently a highly successful treatment for selected patients with end-stage cardiac failure. The long-term results are limited by the development of coronary artery vasculopathy, infection and malignancy. The activity of transplantation programmes worldwide is severely limited by the availability of donor organs. Further refinements of immunosuppressive agents are likely to result in improved prevention of both acute and chronic rejection. The donor pool is unlikely to be significantly extended as a result of measures to increase donor organ supply. Alternative meth

Small bowel transplantation.

The small bowel, the largest lymphoid organ in the body and host to a myriad of foreign antigens has presented a formidable challenge for transplantation. In the past 5 years, small bowel transplantation has become a viable clinical entity. Successful transplantation of the small bowel has been achieved as part of multivisceral grafting, combined small bowel and liver grafts and even as isolated small bowel grafts. Allograft rejection remains an important post-transplantation problem, although graft versus host disease has been less of a clinical problem than initially feared. Markers that may

[Liver transplantation in children].

The need for paediatric liver transplantation, which in most paediatric series is the remedy for biliary atresia after Kasai's operation has failed, is not sufficiently covered by organ retrieval at the present time. In most cases, survival after liver transplantation in children is approximately 80%. Mortality is still high due to intra-operative complications in most cases. Morbidity is related to vascular complications as well as to different types of infections which occur in nearly all the children. After the initial period, growth in most children returns to normal with normal physical a

Intestinal and multivisceral transplantation.

Intestinal transplantation for treatment of irreversible intestinal failure is the newest of the transplantation operations to be developed for clinical use. Because the bowel is more vulnerable to rejection than heart, lung, kidney, liver, or pancreas, practical clinical intestinal transplantation has been relatively slow to develop and is still in its infancy. Imaging studies play an important role in the initial evaluation of the residual native gastrointestinal tract for patients who are potential candidates for enteric implant. After transplantation, gastrointestinal contrast studies help

Liver transplantation and haemophilia A.

Liver transplantation has become the standard treatment for a variety of inherited metabolic disorders. We report on two patients who underwent successful transplantation for posthepatitis viral cirrhosis, which developed following blood factor replacement for haemophilia A. The second patient was transplanted before the occurrence of major complications of either his liver or haemophilic disease. We propose early liver transplantation to achieve metabolic cure of haemophilia.

[Lung transplantation in COPD].

Pulmonary transplantation can be performed as a treatment for panlobular emphysema. Young patients can benefit from pulmonary transplantation: double lung transplantation for patients aged less than 55 years and single lung transplantation for patients aged less than 60 years. Only patients with major functional impairment should be considered for transplantation. It is assessed by pulmonary function tests, but also by evaluating the intensity of dyspnea and the decreased quality of life. In emphysema, transplantation is the treatment of the respiratory handicap without decreased life expectan

Neuropathology of liver transplantation.

We reviewed retrospectively the clinical records, autopsy protocols and central nervous system tissue sections of 50 patients who underwent orthotopic liver transplantation for end-stage liver disease between 12/83 and 8/93. The postoperative survival period ranged from hours (6), weeks (17), months (17), to years (10). All patients received immunosuppressive drugs from the immediate postoperative period to the time of their death (cyclosporine, steroids; occasionally azathioprine, OKT3, FK506). Nineteen patients had neurological manifestations (hepatic encephalopathy) prior to surgery. Post-t

Pediatric renal transplantation.

1. Approximately half of the pediatric patients received grafts from their parents and half from cadaver donors. Pediatric recipients accounted for only 5% of cadaver-donor transplants. 2. Pediatric patients differed from adults in the incidence of primary disease, principally in the occurrence of diabetes (almost no juvenile diabetics had progressed to renal failure compared with 30% among adults). Obstructive uropathy and dysplasia were the primary diseases for about 37% of pediatric patients compared with 3% in adults. 3. Pediatric patients under the age of one had a markedly lower graft su…

[Nephrectomy of the transplanted kidney].

A total of 118 allograft nephrectomies (TX) were performed after 474 renal transplants (TR) (24.9%). 49.1% of the patients were immunosuppressed with azathioprine-prednisone (AZA-PRED) and 50.9% with cyclosporine-prednisone (CSA-PRED). Mean time to TX after returning to hemodialysis was 36.2 +/- 6.4 days (0-372). Acute rejection (33.1%) was the first cause of TX, followed by chronic rejection (26.3%), vascular complications (25.5%), recurrent renal disease (5.9%), non-functioning allograft (4.2%) and not clearly established cause (1.7%). The surgical technique was extracapsular in 70.3% of the…

Pancreas transplantation and dysuria.

Bladder drainage of the pancreatic graft remains the most common technique in pancreatic transplantation. Graft and patient survival is superior to duct injection and bowel drainage techniques. However, several urological complications develop as a result of bladder drainage. We report 6 cases of the dysuria syndrome after combined kidney/pancreas transplantation. Retrograde urethrography or cystoscopy showed extravasation from the bulbous urethra in all cases, which was most likely caused by activated proteolytic exocrine enzymes from the transplanted pancreas. Minor urethral trauma due to Fo

The kidney in liver transplantation.

Renal dysfunction often complicates the course of liver transplant recipients. Preoperative renal dysfunction, including hepatorenal syndrome (HRS) may be present. Assessment of renal function in the pretransplant patient with end-stage liver disease is fraught with pitfalls. Direct measurement of GFR by a method other than creatinine clearance is recommended wherever possible. Preoperative renal biopsy should also be considered in those patients with renal dysfunction in whom the diagnosis of HRS is not definite. With the routine use of veno venous bypass, renal perfusion is maintained and in

The transplantation of gastrointestinal organs.

Over a period of 33 years, it has become possible to successfully transplant individual intra-abdominal viscera or combinations of these organs. The consequences have been, first, new information about the metabolic interrelations that the visceral organs have in disease or health; second, the addition of several procedures to the treatment armamentarium of gastrointestinal diseases; and third, a more profound understanding of the means by which all whole organ grafts are accepted.

[Indications for heart transplantation].

Heart transplantation may have a considerable impact on the fate of the patient in the end stage of heart failure. A successful procedure requires the existence of a sophisticated programme combining the expertise of top departments of cardiology and cardiac surgery working in conjunction with the transplant centre directly responsible for removing the organ. One of the factors playing an important role in a favourable outcome is the correct indication for the procedure. The information on patient selection and criteria for the heart donor, presented in this paper, become increasingly importan

Coccidioidomycosis and heart transplantation.

Eleven episodes of Coccidioides immits infection (cocci) were documented in nine of 199 heart transplant recipients. Cocci was confirmed by fungal cultures and treated with 1 gram of intravenous amphotericin B, after which all patients were put on ketoconazole 200 mg orally twice a day. Two recurrent cocci episodes were noted, both occurring when ketoconazole maintenance therapy was discontinued. When compared with the group of patients without cocci, actuarial survival at 1 and 5 years was identical. We conclude that cocci does not seem to affect the early and late clinical outcome and that p

Cardiac transplantation at UCLA.

In the last decade, the number of patients undergoing heart transplantation has steadily increased as a result of expanding indications for cardiac transplantation. The limitation on the number of transplants performed has been the scarcity of donor organs. At UCLA, 320 heart transplant procedures were performed in adults through the end of 1992; 173 patients (54%) underwent transplantation for ischemic cardiomyopathy; 104 (33%) for dilated cardiomyopathy, and 13 (4%) for severe rejection or cardiac allograft vasculopathy. The mean number of rejection episodes and infections per patient per ye…

Cardiac transplantation: a review.

Cardiac transplantation is no longer an experimental procedure, but rather a definitive therapeutic option for patients with end-stage cardiac failure. The current 5-year and 10-year survival rates following cardiac transplantation are 67% and 52%, respectively. Rejection and infection still result in significant morbidity and mortality, and the development of graft coronary occlusive disease limits even longer-term survival. This review summarises donor and recipient selection criteria, the operative procedure, the current immunosuppressive protocols, as well as newer immunosuppressive agents

A decade of lung transplantation.

OBJECTIVE: The experience accrued at the University of Pittsburgh between March 1982 and December 1992 in the various forms of lung transplantation, including heart-lung, double lung, and single lung, is discussed. SUMMARY BACKGROUND DATA: Heart-lung (n = 97) was the most commonly performed operation followed by double lung (n = 80) and single lung (n = 68). Major indications included primary pulmonary hypertension (n = 76), obstructive lung disease (n = 57), Eisenmenger's syndrome (n = 42), cystic fibrosis (n = 32), and retransplantation (n = 13). Since May 1991, 115 procedures have been per

Neoplastic complications of transplantation.

Cancer incidence in transplant patients ranges from 4% to 18% (average, 6%). We present data on 7668 types of malignancy that occurred in 7192 patients. The predominant tumors are skin/lip carcinomas, lymphomas, vulvar/perineal carcinomas, in situ uterine cervical carcinomas, Kaposi's sarcoma (KS), renal carcinomas, hepatobiliary tumors, and soft tissue/visceral sarcomas. Tumors appear relatively early after transplantation. The earliest is KS, appearing an average of 22 months post-transplantation whereas the latest are vulva/perineal carcinomas, presenting an average of 113 months posttransp
