Usher's syndrome. Ophthalmic and neuro-otologic findings suggesting genetic heterogeneity.

The conditions of 70 patients with Usher's syndrome were studied by ophthalmic and neuro-otologic examinations. Two distinct clinical and presumed genetic types were discernible on the basis of differences in hearing impairment, vestibular sensitivity, and, to a lesser extent, deterioration in retinal photoreceptor function. Distinguishing these two types has relevance for both diagnosis and genetic counseling of patients with Usher's syndrome.

Follow-up vestibular examination in Menière's disease.

A retrospective study was carried out on repeated audio-vestibular examinations in 53 patients with unilateral Meniere's disease, between the vertiginous spells. Most patients showed no change in the caloric response reduction of the afflicted ear in the subsequent tests compared with the first one. These patients had a comparatively long illness duration, a small percentage of fluctuations in the pure tone audiogram and mostly flat or falling type curves. A minority of patients showed an increase in or a fluctuating caloric test reduction. This group consists of patients with a short disease

Skull vibration-induced nystagmus test in unilateral superior canal dehiscence and otosclerosis: a vestibular Weber test.

CONCLUSIONS: The skull vibration-induced nystagmus test (SVINT) acts as a vestibular Weber test and reveals a vibration-induced nystagmus (VIN), elicited mainly on the vertex location, with a horizontal or torsional component beating more often toward the side of the lesion in superior canal dehiscence (SCD) than in otosclerosis (OS). In SCD, the VIN vertical component is most often up-beating. These results suggest more a global vestibular contribution than the sole stimulation of the superior semicircular canal. OBJECTIVES: This study aimed to evaluate the possible occurrence of nystagmus d

Mobile posturography: posturographic analysis of daily-life mobility.

OBJECTIVE: Mobility is crucial to maintain a sufficient quality of life. Posturography should be therefore focused on the investigation of daily-life activities ("mobile posturography"). Nowadays, postural control is often estimated by stance tasks on a force plate under different sensorimotor conditions. This technique applies an indirect approximation of the center of body mass and is not related to tasks required for mobility. An alternative approach would be the direct measurement of body sway during daily-life conditions close to the center of body mass. The present study was aimed at inv

Chronic subjective dizziness.

CONCLUSION: Chronic subjective dizziness (CSD) is frequent and affects twice as many women as men. Anxiety is a strong predisposing factor. The pathophysiologic concept of this disorder assumes that balance function and emotion share common neurologic pathways, which might explain that the balance disorder can provoke fear and vice versa, giving rise to a problem in perception of space and motion. In anxious patients this can turn into a space and motion phobia, with avoidance behaviour. OBJECTIVE: CSD is a diagnosis based on the hypothesis of an interaction between the vestibular system and

Perilymph fistula--a diagnostic dilemma.

A retrospective series is presented of 51 cases operated on for suspected perilymph fistula. In 26 ears a fistula was identified at surgery. A positive fistula test was found to strongly indicate a perilymph fistula but was more often negative than positive in surgically demonstrated fistula ears. Other vestibular tests were found to be of little value in the pre-operative diagnosis. Ears with a surgically demonstrated fistula and sensorineural hearing loss had either flat or downward-sloping audiograms. Difficulties in diagnosing a perilymph fistula at tympanotomy are discussed. At follow-up,

Contralesional High-Acceleration Vestibulo-Ocular Reflex Function in Vestibular Schwannoma.

UNLABELLED: Impairment of ipsilesional vestibulo-ocular reflex (VOR) function is well described in vestibular schwannoma (VS) and a correlation between gain of the VOR and tumor size has been suggested. Bilateral VOR impairment may also occur in VS patients, but its mechanisms are poorly understood. We sought to explore the effect of unilateral VS on ipsilesional and contralesional high-acceleration VOR function using video head impulse testing, and evaluate potential factors responsible for contralesional VOR impairment. MATERIALS AND METHODS: Chart review in tertiary referral center of pati

The influence of sensory function on walking stability in aging and falls through a head-movement perturbation model.

Age-related sensory decline impairs responses to balance disturbances, increasing fall risk. Evaluating gait and balance under sensory challenges can help reveal how this decline affects instability in older adults. This study (Clinicaltrials.gov NCT06345625) incorporates head movements to evaluate the effect of sensory function - cervical sensorimotor control, vestibular function, and visual acuity - on walking stability in aging and its relationship with falls through a novel augmented-reality perturbation model. Sixty-one healthy, community-dwelling older adults (76.2 years; 40 females) wit

Horizontal VOR function shows frequency dynamics in vestibular schwannoma.

The objective of this retrospective study was to investigate the horizontal vestibulo-ocular reflex (hVOR) pathway with caloric test (low-frequency hVOR) and video head impulse test (vHIT) (high-frequency hVOR) in patients with sporadic vestibular schwannoma (69 patients, 27-86 years, mean age 58.1 years) and to compare both test methods in terms of their sensitivity and specificity to detect a retrocochlear lesion. Test results with a unilateral weakness (UWCaloric) >25 % (caloric test) or a Mean-GainvHIT <0.79/asymmetry ratio of Gain (AR-GainvHIT) >8.5 % and accompanied refixation saccades (

Classification of human rotation test results using parametric modeling and multivariate statistics.

The usefulness of vestibular testing is directly related to the accuracy of the test interpretations. Two factors, subjective analysis of large test data sets and failure to make appropriate age corrections, tend to reduce test accuracy. Correction of these problems can be accomplished by application of physiologically based models of vestibular function and multivariate classification techniques to the test data, thereby creating a more objective test interpretation procedure. Herein we report our results on the use of this strategy for analysis of sinusoidal harmonic acceleration (SHA) test

Neuropsychological Assessment Following Concussion: an Evidence-Based Review of the Role of Neuropsychological Assessment Pre- and Post-Concussion.

Neuropsychological evaluation is one component of a comprehensive and multifaceted assessment following concussion. Although some neuropsychologists use a "hybrid" assessment approach integrating computerized neurocognitive testing batteries with traditional paper and pencil tests, computerized neurocognitive test batteries are the predominant testing modality for assessment of athletes from the youth to professional level. This review summarizes the most recent research supporting the utility of neuropsychological evaluation and highlights the strengths and weaknesses of both computerized and

[Hearing classification in patients with vestibular schwannoma using German-language test procedures].

BACKGROUND: Hearing function in patients with vestibular schwannoma is often classified according to the Gardner and Robertson (1988) or the American Academy of Otolaryngology-Head and Neck Surgery (AAO-HNS, 1995) systems. These classification systems are based on English-language test procedures, there is no German-language equivalent. The aim of the study was to investigate the influence of various target parameters on hearing classification and to derive a recommendation for the use of German-language test methods. MATERIALS AND METHODS: The rules for speech audiometry based on English-lan

Loss of peripheral vestibular input alters the statistics of head movement experienced during natural self-motion.

KEY POINTS: Sensory systems are adapted to the statistical structure of natural stimuli, thereby optimizing neural coding. Head motion during natural activities is first sensed and then processed by central vestibulo-motor pathways to influence subsequent behaviour, thereby establishing a feedback loop. To investigate the role of this vestibular feedback on the statistical structure of the head movements, we compared head movements in patients with unilateral vestibular loss and healthy controls. We show that the loss of vestibular feedback substantially alters the statistical structure of hea

Silent neurovestibular frailty: a conceptual model integrating vestibular dysfunction, dual-task mobility and cognitive load as candidate contributors to falls, delirium and functional decline in older adults.

Falls, delirium and functional decline are commonly treated as distinct geriatric outcomes despite sharing vulnerabilities in sensory integration, gait regulation, attention and physiological reserve. We propose silent neurovestibular frailty as a conceptual model describing a latent ageing phenotype in which vestibular dysfunction, dual-task gait deterioration and sensitivity to cognitive load interact before overt disability becomes apparent. This construct is presented as a hypothesis-generating framework rather than a validated diagnostic entity, intended to integrate currently separate li

Clinical Heterogeneity in Vestibular Migraine Diagnosed Using Bárány Society Criteria: Differences According to Caloric-Test-Based Evidence of Peripheral Vestibular Dysfunction.

BACKGROUND AND PURPOSE: Vestibular migraine (VM) is a common cause of episodic vertigo with heterogeneous presentations. Some VM patients show caloric abnormalities suggestive of peripheral vestibular dysfunction (PVD). This study compared clinical symptoms and vestibulo-ocular function between VM patients with caloric canal paresis (CP) >25% (VM-CP+), which was used as an operational definition of evidence of PVD, and those without caloric CP >25% (VM-CP-). METHODS: We enrolled 181 patients diagnosed with VM according to the Bárány Society criteria: 47 in the VM-CP+ group and 134 in the VM-C
