All forms of supraventricular tachycardia (SVT) are now potentially curable by surgery and we believe that patients should be offered surgery as an initial therapeutic option. At Westmead Hospital, 311 patients have undergone surgery for SVT, 13 having AV node ablation, a procedure now rarely performed, and 298 have had attempts at curative surgery. Whether or not the atrium plays an essential role in initiating and/or sustaining atrioventricular (A-V) nodal re-entrant tachycardia was evaluated in eight patients. Ventricular tachycardias originating from the Purkinje system are the most common type of idiopathic left ventricular tachycardia. The effects of isoetharine on the His bundle electrogram were studied in 10 patients with heart disease. Recordings were made at varied heart rates using atrial pacing. Cardiac resynchronisation therapy (CRT) for atrial fibrillation (AF) with intraventricular conduction and low cardiac output may not allow high-frequency biventricular pacing (BiVP). To this end, drugs and ablation treatments are commonly used. Ventricular sense response pacing (VSRP) senses self-pulse and can produce a state comparable with BiVP. If found to be effective for cardiac resynchronisation, the benefit of CRT with VSRP is expected to extend to patients with AF. Herein, we describe the case of a man in his early 60s with low cardiac output caused by AF and left bundle branch block.
