Efficacy and safety of monotherapy with the novel sodium/glucose cotransporter-2 inhibitor tofogliflozin in Japanese patients with type 2 diabetes mellitus: a combined Phase 2 and 3 randomized, placebo-controlled, double-blind, parallel-group comparative study.

BACKGROUND: In recent years, several oral antidiabetic drugs with new mechanisms of action have become available, expanding the number of treatment options. Sodium/glucose cotransporter-2 (SGLT2) inhibitors are a new class of oral antidiabetic drugs with an insulin-independent mechanism promoting urinary glucose excretion. We report the results of a combined Phase 2 and 3 clinical study (Japic CTI-101349) of the SGLT2 inhibitor tofogliflozin (CSG452, RG7201) in Japanese patients with type 2 diabetes mellitus. METHODS: The efficacy and safety of tofogliflozin were assessed in this multicenter,

Blood gas parameters during cardio-pulmonary resuscitation in a post hoc analysis-porcine study with data pooled from two studies: increased acidosis with repeated adrenaline injections compared to a control group during prolonged advanced life support.

BACKGROUND: Cardiopulmonary resuscitation (CPR) guidelines recommend an injection of 1 mg of adrenaline every 3-5 min in a cardiac arrest situation. Adrenaline increases return of spontaneous circulation (ROSC) and induces a more acidotic state in cardiac arrest situations. Blood gas analysis is a useful tool to assess the metabolic state during CPR.Most experimental studies have been conducted with a single injection of adrenaline. However, little is known about how blood gas parameters are affected by repeated adrenaline injections during prolonged CPR. METHODS: This was a pooled post-hoc a

Effectiveness of a mindfulness-based intervention + standard diet treatment compared to standard diet treatment alone on food cravings in overweight/obese individuals with mild to moderate anxiety/depression: a study protocol for an open-labeled, parallel-group randomized controlled trial.

BACKGROUND: Overweight/obesity is highly prevalent worldwide. Dropping out of weight loss diets is one of the main reasons for treatment failure in obese individuals. Evidence indicates a significant relationship between anxiety/depression and overweight/obesity. Up to 11% of excess food consumption and the resulting overweight have been attributed to food cravings. Mindfulness-based interventions (MBIs) have recently become a topic of interest in the treatment of obesity-related eating behaviors. The use of recorded audio files has shown promising results in different studies. However, these

Effects of Teneligliptin on the Progressive Left Ventricular Diastolic Dysfunction in Patients with Type 2 Diabetes Mellitus in Open-Label, Marker-Stratified Randomized, Parallel-Group Comparison, Standard Treatment-Controlled Multicenter Trial (TOPLEVEL Study): Rationale and Study Design.

BACKGROUND AND AIMS: Diabetes mellitus (DM) can cause left ventricular (LV) diastolic dysfunction, leading to heart failure with preserved ejection fraction (HFpEF). Dipeptidyl peptidase IV (DPP-IV) inhibitors have failed to reduce hospitalization due to HF in type 2 DM (T2D) patients in a large-scale clinical trial, despite their cardiovascular protective effects. Therefore, it is important to investigate whether DPP-IV inhibitors can improve LV diastolic dysfunction in T2D patients. The aim of the study was to evaluate whether teneligliptin, the strongest of the DPP-IV inhibitors, improves L

Effect of vitamin D3-fortified fruit juice supplementation of 4000 IU daily on the recovery of iron status in childbearing-aged women with marginally low iron stores: Protocol for an 8-week, parallel group, double-blind randomized controlled trial.

BACKGROUND: In recent years, emerging evidence has highlighted the role of vitamin D as an iron absorption enhancer by suppressing hepcidin concentration, albeit with an unclear underlying mechanism. Dietary-based approach in improving iron status has been widely practised, however, there is a scarcity in randomized controlled trials (RCT) to elucidate the effect of vitamin D-fortified juice on iron status recovery. Therefore, this study aims to investigate the effect of an 8-week vitamin D3-fortified fruit juice supplementation on iron status indicators in childbearing-aged women with margina

Phase-IIa randomized, double-blind, sham-controlled, parallel group trial on anodal transcranial direct current stimulation (tDCS) over the left and right tempo-parietal junction in autism spectrum disorder-StimAT: study protocol for a clinical trial.

BACKGROUND: Autism spectrum disorder (ASD) is characterized by impaired social communication and interaction, and stereotyped, repetitive behaviour and sensory interests. To date, there is no effective medication that can improve social communication and interaction in ASD, and effect sizes of behaviour-based psychotherapy remain in the low to medium range. Consequently, there is a clear need for new treatment options. ASD is associated with altered activation and connectivity patterns in brain areas which process social information. Transcranial direct current stimulation (tDCS) is a techniqu

Comparison of efficacy of ultrasound-guided obturator nerve block versus pericapsular nerve group block in preventing adductor jerk in patients undergoing transurethral resection of postero-lateral bladder mass under spinal anaesthesia: A randomised, single-blinded, controlled study.

BACKGROUND AND AIMS: Adductor jerk during transurethral resection of bladder tumour (TURBT) is a common complication caused by obturator nerve stimulation, particularly in tumours located on the lateral or posterolateral bladder wall. Obturator nerve block (ONB) is widely used to prevent this complication. The pericapsular nerve group (PENG) block is a newer regional technique that may also reduce adductor jerk by blocking branches of the obturator and accessory obturator nerves. This study compared the efficacy of ultrasound-guided PENG block and ONB in preventing adductor jerk during TURBT.

The clinical and cost effectiveness of a conservative treatment pathway compared to appendicectomy in children with uncomplicated acute appendicitis: study protocol for a randomised controlled multi-centre open-label parallel-group non-inferiority trial (CONTRACT 2 trial) in the United Kingdom (UK).

BACKGROUND: Currently, the most frequently used treatment for acute appendicitis in children in the United Kingdom (UK) is an appendicectomy. However, there is increasing scientific and patient interest and research into non-operative treatment of appendicitis. Despite a number of non-randomised studies in children and randomised studies in adults, comparative outcomes of non-operative treatment and appendicectomy in comparable groups of children remain unknown. Following the successful completion of a feasibility study, we now aim to perform a UK-based multi-centre open-label randomised contr

Study protocol of a randomised, double-blind, placebo-controlled, two-arm parallel-group, multi-centre phase 3 pivotal trial to investigate the efficacy and safety of recombinant human alkaline phosphatase for treatment of patients with sepsis-associated acute kidney injury.

INTRODUCTION: Sepsis, the leading cause of acute kidney injury (AKI), is associated with a high morbidity and mortality. Alkaline phosphatase (ALP) is an endogenous detoxifying enzyme. A recombinant human ALP compound, ilofotase alfa, showed no safety or tolerability concerns in a phase 2 trial. Renal function improvement over 28 days was significantly greater in the ilofotase alfa group. Moreover, a significant relative reduction in 28-day all-cause mortality of >40% was observed. A follow-up trial has been designed to confirm these findings. METHODS AND ANALYSIS: This is a phase 3, global,
