Long-term effects of ospemifene on densitometric and bone metabolism biomarkers in postmenopausal women reporting Vulvar and Vaginal Atrophy (VVA).

Ospemifene (OSP), a Selective Estrogen Receptor Modulator (SERM), is recommended for treating vulvovaginal atrophy (VVA) in postmenopausal women (PMW). Previous 12 mm data suggested that OSP therapy had beneficial effects on bone mineral and biochemical markers in a similar VVA population. Objective This real-life study sought to evaluate the long-term effects (24 mm) of Ospemifene therapy on bone metabolism and mineral parameters compared with a control group. Methods PMW aged 40–64 years with VVA symptoms and a baseline bone health assessment were included in the study. A total of 72 subjects

Extended therapy with letrozole as adjuvant treatment of postmenopausal patients with early-stage breast cancer: a multicentre, open-label, randomised, phase 3 trial.

BACKGROUND: The benefit of extending aromatase inhibitor therapy beyond 5 years in the context of previous aromatase inhibitors remains controversial. We aimed to compare extended therapy with letrozole for 5 years versus the standard duration of 2-3 years of letrozole in postmenopausal patients with breast cancer who have already received 2-3 years of tamoxifen. METHODS: This multicentre, open-label, randomised, phase 3 trial was done at 69 hospitals in Italy. Women were eligible if they were postmenopausal at the time of study entry, had stage I-III histologically proven and operable invasi

Selective inhibition by aspirin and naproxen of mainstream cigarette smoke-induced genotoxicity and lung tumors in female mice.

The role of nonsteroidal anti-inflammatory drugs (NSAIDs) in smoke-related lung carcinogenesis is still controversial. We have developed and validated a murine model for evaluating the tumorigenicity of mainstream cigarette smoke (MCS) and its modulation by chemopreventive agents. In the present study, the protective effects of the nonselective cyclooxygenase inhibitors aspirin and naproxen were investigated by using a total of 277 Swiss H neonatal mice of both genders. Groups of mice were exposed whole-body to MCS during the first 4 months of life, followed by an additional 3.5 months in filt

Impact of Fertility Concerns on Tamoxifen Initiation and Persistence.

BACKGROUND: Adjuvant tamoxifen reduces breast cancer recurrence risk and mortality; however, initiation and treatment persistence are poor for younger patients. We hypothesized that a unique set of factors, including fertility concerns, would contribute to the poor tamoxifen use among premenopausal patients. METHODS: From 2007 to 2012, 515 premenopausal patients younger than age 45 years, with stage 0 to III hormone receptor-positive breast cancer, for whom tamoxifen was recommended, were identified. Clinical and pathologic tumor characteristics, treatment regimens, and fertility concerns wer

Hormone modulation: a novel therapeutic approach for women with severe mental illness.

OBJECTIVES: Accumulating evidence describes the effects of oestrogen and other gonadal hormones on the central nervous system and, in particular, on the mental state of women. Evidence supporting the psychotherapeutic effects of exogenous oestrogen has started to emerge only over the past two decades. The purpose of the present paper was to provide an overview of different applications of adjunctive hormones, as treatments for symptoms of severe mental illness in women. METHODS: Three case reports are presented: in each case the woman selected had participated in large, double-blind, randomiz

Risk-reduction mastectomy: clinical issues and research needs.

Risk-reduction mastectomy (RRM), also known as bilateral prophylactic mastectomy, is a controversial clinical option for women who are at increased risk of breast cancer. High-risk women, including women with a strong family history of breast cancer and BRCA1/2 mutation carriers, have several clinical options: risk-reduction surgery (bilateral mastectomy and bilateral oophorectomy), surveillance (mammography, clinical breast examination, and breast self-examination), and chemoprevention (tamoxifen). We review research in a number of areas central to our understanding of RRM, including recent d

A prospective single-arm pilot study evaluating [F-18]fluoroestradiol dedicated breast PET in invasive lobular carcinoma after neoadjuvant endocrine therapy.

PURPOSE: Invasive lobular carcinoma (ILC) of the breast presents challenges in monitoring response to neoadjuvant endocrine therapy (NET). Dedicated breast positron emission tomography with 18F-fluoroestradiol (FES-dbPET), a molecular imaging technique targeting the estrogen receptor (ER), may offer insight into treatment response. This study evaluated whether changes in FES-dbPET uptake before and after NET in patients with ILC correlate with treatment response indicators, and whether these associations differ by endocrine therapy type. METHODS: This prospective pilot study included patients

In vitro effects of 2-hydroxyestradiol-17β on ovarian follicular steroid secretion in the catfish Heteropneustes fossilis and identification of the receptor and signaling mechanisms.

Ovarian pieces containing postvitellogenic follicles were incubated in vitro with different concentrations of the catecholestrogen 2-hydroxyestradiol-17β (2-OHE(2)) to evaluate its effects on steroid production and germinal vesicle breakdown (GVBD) in the catfish Heteropneustes fossilis. The incubation with 2-OHE(2) induced a shift in steroidogenic pattern: the C(19) and C(18) steroids testosterone (T) and estradiol-17β (E(2)), respectively were significantly decreased with a concomitant significant increase in the C(21) steroids progesterone (P(4)), 17-hydroxyprogesterone (17-OHP), 17,20β-dih

Reduced incidence of invasive breast cancer with raloxifene among women at increased coronary risk.

BACKGROUND: In the Raloxifene Use for The Heart trial, 10 101 postmenopausal women with coronary heart disease (CHD) or multiple CHD risk factors were randomly assigned to 60 mg/d raloxifene or to placebo and followed for a median of 5.6 years. Raloxifene, a selective estrogen receptor modulator, was found to reduce the risk of invasive breast cancer and vertebral fractures but not the risk of cardiovascular events. Here, we provide further details about breast cancer incidence by tumor characteristics, duration of treatment, and subgroup. METHODS: Reported breast cancer was adjudicated by an

Exploring the Therapeutic Potential of Ocimum sanctum and Phanera variegata in Breast Cancer Treatment: A Promising Natural Approach.

Breast cancer is one of the most common malignancies affecting women worldwide. It is a complex, heterogeneous disease, classified into several subtypes, including hormone receptor-positive and triple-negative breast cancer (TNBC), each with distinct therapeutic challenges. TNBC, in particular, is characterized by its aggressive nature and lack of targeted therapies due to the absence of estrogen, progesterone, and HER2 receptors. This review explores the potential of natural plant-based compounds, especially focusing on Clove Basil (Ocimum sanctum) and Phanera variegata, in combating breast c

Genistein and the immune system: experimental evidence, key challenges, and future perspectives.

Genistein (GEN), a soy-derived isoflavonoid and phytoestrogen, exhibits diverse biological activities against allergic and hypersensitivity reactions, autoimmune disorders, infectious diseases, and cancer. Its immunomodulatory properties are crucial to these effects; however, clinical translation remains limited due to the complexity and pronounced context dependence of its immune actions. This review critically summarizes experimental evidence on GEN-mediated immunomodulation across innate and adaptive immune systems, including its effects on cytokine regulation and key molecular signaling pa

A Potential Role and Contribution of Androgens in Placental Development and Pregnancy.

Successful pregnancy requires the establishment of a highly regulated maternal-fetal environment. This is achieved through the harmonious regulation of steroid hormones, which modulate both maternal and fetal physiology, and are critical for pregnancy maintenance. Defects in steroidogenesis and steroid signaling can lead to pregnancy disorders or even fetal loss. The placenta is a multifunctional, transitory organ which develops at the maternal-fetal interface, and supports fetal development through endocrine signaling, the transport of nutrients and gas exchange. The placenta has the ability

Gonadal hormones and the control of reactive gliosis.

Astrocytes and microglia respond to central nervous system (CNS) injury with changes in morphology, proliferation, migration and expression of inflammatory regulators. This phenomenon is known as reactive gliosis. Activation of astrocytes and microglia after acute neural insults, such as stroke or traumatic CNS injury, is considered to be an adaptive response that contributes to minimize neuronal damage. However, reactive gliosis may amplify CNS damage under chronic neurodegenerative conditions. Progesterone, estradiol and testosterone have been shown to control reactive gliosis in different m
