Brain imaging in pediatric obsessive-compulsive disorder.

OBJECTIVE: To review progress in understanding pediatric obsessive-compulsive disorder (OCD). The focus is on the frontal-striatal-thalamic model of OCD, neurobiological and genetic studies of the disorder, and their influence on recent advances in treatment. METHOD: Computerized literature searches were conducted with the key words "obsessive-compulsive disorder" in conjunction with "pediatric," "genetics," and "imaging." RESULTS: Neuroimaging studies find evidence to support the frontal-striatal-thalamic model. Genetic and neurochemical studies also implicate glutamate in the pathological

Right ventromedial prefrontal cortex: a neuroanatomical correlate of impulse control in boys.

Emerging data on the neural mechanisms of impulse control highlight brain regions involved in emotion and decision making, including the ventromedial prefrontal cortex (vmPFC), anterior cingulate cortex (ACC) and amygdala. Variation in the development of these regions may influence one's propensity for impulsivity and, by extension, one's vulnerability to disorders involving low impulse control (e.g. substance abuse). Here we test the hypothesis that lower impulse control is associated with structural differences in these regions, particularly on the right side, in 61 normal healthy boys aged

Significant differences in central imaging of histamine-induced itch between atopic dermatitis and healthy subjects.

BACKGROUND/AIM: This is the first investigation of the central processing of itch in the brain in 8 subjects with atopic dermatitis (AD) in comparison to 6 healthy controls (HC), comparing histamine-induced itch related activations in the frontal, prefrontal, parietal, cingulate cortex, thalamus, basal ganglia and cerebellum. METHODS: We employed 1% histamine-dihydrochlorid-iontophoresis of the left hand, recorded H2(15)O-PET-scans and perception of itch intensity on a numeric rating scale. RESULTS: There was no significant difference in perceived itch intensity between AD and HC. Significan

A magnetic resonance imaging study of mood stabilizer- and neuroleptic-naïve first-episode mania.

OBJECTIVES: Patients with bipolar disorder have changes in brain structures but it is unclear if these are present at disease onset and thus predispose subjects to develop the disorder, or whether they develop during the course of the disorder, either due to the effects of multiple episodes or as a consequence of treatment with psychotropic agents. Studies in first-episode (FE) manic patients have the potential to provide answers to these questions. METHODS: Voxel-based morphometry (VBM) was used to assess magnetic resonance imaging scans of 15 FE manic patients and 15 matched healthy control

Retrosplenial cortex lesions impair acquisition of active avoidance while sparing fear-based emotional memory.

There is strong evidence that the rat retrosplenial cortex (RC) is implicated in spatial navigation and in learning of both aversive and reward-based discrimination tasks. However, its involvement in other functions subserved by the limbic system to which it belongs has not yet been documented. We compared the performance of rats with bilateral excitotoxic damage to RC with that of control rats in a battery of conventional tests, including an open field, plus maze, fear conditioning, step-through passive avoidance, and two-way active avoidance techniques. In the open field, RC-lesioned rats sh

Impaired wakefulness is associated with reduced anterior cingulate CBF in patients with normal pressure hydrocephalus.

OBJECTIVE: To evaluate if impaired wakefulness (IW) in normal pressure hydrocephalus (NPH) is associated with reduced blood flow in regions associated with the brain arousal system. METHOD: NPH (n = 28) patients were studied before and after surgery. Wakefulness was assessed using a new developed scale. Relative regional cerebral blood flow (rrCBF) was quantified using SPECT and rectangular regions of interest analysis. RESULTS: Sixteen patients presented with IW at baseline and in 14 of these, IW vanished after surgery. Patients presenting with IW had reduced rrCBF in the anterior cingulate

Changes in emotion after circumscribed surgical lesions of the orbitofrontal and cingulate cortices.

To analyse the functions of different parts of the prefrontal cortex in emotion, patients with different prefrontal surgical excisions were compared on four measures of emotion: voice and face emotional expression identification, social behaviour, and the subjective experience of emotion. Some patients with bilateral lesions of the orbitofrontal cortex (OFC) had deficits in voice and face expression identification, and the group had impairments in social behaviour and significant changes in their subjective emotional state. Some patients with unilateral damage restricted to the OFC also had de

A fMRI study of brain activations during non-noxious and noxious electrical stimulation of the sciatic nerve of rats.

An acute pain animal model for fMRI study would provide useful spatial and temporal information for studying the supraspinal nociceptive neuronal responses. The aim of the present study was to investigate whether the nociceptive responses in different brain areas can be differentiated by using functional magnetic resonance imaging (fMRI) in anesthetized rats. Functional changes in brain regions activated by noxious or non-noxious stimuli of the sciatic nerve were investigated using fMRI in a 4.7 T MR system in alpha-chloralose anaesthetized rats. To determine the electrical intensity for noxio

Cerebral correlates of psychotic symptoms in Alzheimer's disease.

BACKGROUND: Psychotic symptoms are produced by distributed neuronal dysfunction. Abnormalities of reality testing and false inference implicate frontal lobe abnormalities. OBJECTIVES: To identify the functional imaging profile of patients with Alzheimer's disease manifesting psychotic symptoms as measured by single photon emission computed tomography (SPECT). METHODS: Twenty patients with Alzheimer's disease who had SPECT and clinical evaluations were divided into two equal groups with similar mini mental status examination (MMSE), age, sex, and the range of behaviours documented by the neur

Photosensitive epilepsy studied by functional magnetic resonance imaging and magnetic resonance spectroscopy.

PURPOSE: To study metabolic and hemodynamic correlates of photic stimulation-triggered discharges. METHODS: Simultaneous EEG, functional MRI (tMRI) and magnetic resonance spectroscopy (MRS) were performed in nine patients with photosensitive epilepsy and in 12 normal subjects. RESULTS: Prominent visual cortex activation was seen in all normal subjects and patients, and no tMRI-registered hemodynamic abnormalities were correlated with the brief photoparoxysmal spike-wave activity evoked in the photosensitive patients. However, irrespective of the presence of a spike-wave response to the photi

PET imaging of serotonin type 2A receptors in late-life neuropsychiatric disorders.

OBJECTIVE: To determine whether there are abnormalities in the in vivo status of the serotonin type 2A (5-HT2A) receptor in late-life depression and Alzheimer's disease, the authors used positron emission tomography (PET) to assess patients with these two conditions and healthy subjects. METHOD: PET was performed by using [18F]altanserin to evaluate 5-HT2A receptor binding in 11 elderly patients with depression (four men, seven women; mean age = 65.0 years, SD = 5.5); nine Alzheimer's disease patients, including three with concurrent depression (two men, seven women; mean age = 69.7 years, SD

p-Chlorophenylalanine and fluoxetine inhibit D-fenfluramine-induced Fos expression in the paraventricular nucleus, cingulate cortex and frontal cortex but not in other forebrain and brainstem regions.

D-Fenfluramine, a putative serotonin releaser and reuptake inhibitor, is commonly prescribed for the treatment of obesity. Brain sites activated by D-fenfluramine have been mapped via the expression of the immediate early gene Fos. However, it is not clear that serotonin release in the brain mediates the effects of D-fenfluramine on Fos expression. The present study determined whether D-fenfluramine induces the expression of Fos in the brain through the release of serotonin. Rats were pretreated either with the serotonin depleting drug p-chlorophenylalanine (PCPA) or with the serotonin reuptak

Brain activation induced by the perceptual maze test: a PET study of cognitive performance.

We investigated with PET the cerebral activation pattern elicited by the perceptual maze test (PMT), a neuropsychological test used to evaluate organic brain injury. The PMT examines visuospatial skill, general intelligence, visually guided motor planning, and the ability to obey rules. Eight right-handed volunteers were examined with PET using the tracer [15O]butanol. Three paradigms containing the PMT, a motor control (SHAM), and a rest condition were examined twice in a randomized order. Solving the PMT caused extensive bilateral activations in the occipital lobe extending rostrally into th

Standardized depth-resolved mapping of anterior cingulate cortex synaptic markers reveals inhibitory marker alterations following prenatal valproic acid exposure.

Proper coordination of excitatory and inhibitory synaptic inputs is essential for stable neural circuit activity and flexible behavior. Although excitation-inhibition (E/I) imbalance has been implicated in neurodevelopmental disorders, quantitative histological approaches for relating cortical synaptic marker organization to behavioral variation remain limited. Here, using male C57BL/6J mice, we established a standardized depth-aligned histological framework to quantify anterior cingulate cortex (ACC) synaptic marker organization and its relationship to behavioral variation. Using postsynaptic

Compensatory neurochemical adaptation in first-episode, drug-naïve depression: Integrating cortical and systemic metabolism using multivoxel 1H-MRS.

BACKGROUND: Major depressive disorder (MDD) involves both central neurotransmission and systemic metabolic disturbances. This study investigated how body mass index (BMI) modulates prefrontal neurometabolic profiles and their relationship with depressive severity in depression. METHODS: 66 first-episode, drug-naïve Han Chinese patients with MDD and 79 Han Chinese healthy controls were recruited. Using multi-voxel proton magnetic resonance spectroscopy, we quantified tissue-corrected metabolite-to-creatine ratios in the anterior cingulate cortex (ACC) and bilateral dorsolateral prefrontal cort

Not just the medial temporal lobe: Precuneus and posterior cingulate volumes relate to plasma biomarkers and cognition in a sub-Saharan African cohort.

INTRODUCTION: The precuneus and posterior cingulate-key regions in early Alzheimer's disease (AD)-undergo atrophy before clinical symptoms emerge. Limited research in sub-Saharan Africa (SSA) has explored how blood-based biomarkers relate to brain changes and cognition. This study examined these associations in older adults with dementia in Kinshasa, Democratic Republic of Congo (DRC). METHODS: A total of 117 adults (≥ 65 years; five dementia, and 58 controls) completed neuropsychological testing and blood draws, with 75 completing neuroimaging. Plasma amyloid-beta (Aβ) 40, Aβ42, pTau181, pTa

Fronto-temporal brain activity and connectivity track implicit attention to positive and negative social words in a novel socio-emotional Stroop task.

Previous inconsistencies on the effects of implicitly processing positively - vs. negatively - connotated emotional words might reflect the influence of uncontrolled psycholinguistic dimensions, and/or social facets inherent in putative "emotional" stimuli. Based on the relevance of social features in semantic cognition, we developed a socio-emotional Stroop task to assess the influence of social vs. individual (non-social) emotional content, besides negative vs. positive valence, on implicit word processing. The effect of these variables was evaluated in terms of performance and RTs, alongsid

Stereotactic MRI-guided laser interstitial thermal therapy for extratemporal lobe epilepsy.

OBJECTIVE: Magnetic resonance imaging (MRI)-guided laser interstitial thermal therapy (MRg-LITT) is an alternative to open epilepsy surgery. We assess safety and effectiveness of MRg-LITT for extratemporal lobe epilepsy (ETLE) in patients who are considered less favorable for open resection. METHODS: We retrospectively reviewed sequential cases of patients with focal ETLE who underwent MRg-LITT between 2012 and 2019. Epileptogenic zones were determined from standard clinical and imaging data ± stereoelectroencephalography (SEEG). Standard stereotactic techniques, MRI thermometry, and a commer
