New Olympic Sports: Surfing and Skateboarding.

Surfing and skateboarding were primarily recreational sports before being thrust in the limelight of competition that culminated in their inclusion in the 2020 Tokyo Olympics. With their immense growth came increased propensity for musculoskeletal damage due to the performance of often aggressive airborne maneuvers. Direct trauma is the most common mechanism of injury in both sports. In surfing, the lower limbs are more often injured than the upper limbs, preferentially the rear extremities. Knee injuries like sprains, dislocations, and meniscal tears predominate. Contrary to surfing, skateboa

Anterior Tibial Tendon Side-to-Side Tenorrhaphy after Posterior Tibial Tendon Transfer: A Technique to Improve Reliability in Drop Foot after Common Peroneal Nerve Injury.

Common peroneal nerve injury is present in 40% of knee dislocations, and foot drop is the principal complication. Posterior tibial tendon transfer is a viable solution to replace the function of the anterior tibial tendon (ATT) in the mid-foot. Several techniques for posterior tibial tendon transfer exist today, with variable results reported. However, adding augmentation with side-to-side tenorrhaphy of ATT to the transferred posterior tibial tendon (PTT) enhances anterior tissue balance and load sharing stress between native ATT enthesis and PTT tenodesis, allowing early rehabilitation and i…

Anesthetic Management of a Patient With Ehlers-Danlos Syndrome.

We describe the case of a 37-year-old woman who had been diagnosed with Ehlers-Danlos syndrome (EDS) 4 years earlier and was scheduled to undergo removal of synovial chondromatosis in the temporomandibular joint. EDS is a heritable connective tissue disorder and has 6 types. In this case, the patient was classified into EDS hypermobility type. The major clinical feature of this type is joint hypermobility. The patient had sprain or subluxation of the elbows and ankles and dislocation of the knees. Anticipated problems during general anesthesia would be affected by the disease type. For this pa

Distal femoral varus osteotomy: problems associated with the lateral open-wedge technique.

INTRODUCTION: Varisation osteotomies on the distal femur are an established treatment method for valgus osteoarthritis of the knee in younger patients. Osteotomy can be done in a lateral open-wedge or medial closed-wedge manner. METHOD: We retrospectively studied 14 patients treated by the lateral open-wedge technique, fixed with the Tomofix plate, with a mean duration of follow-up of 45 ± 3.4 months. RESULTS: We observed often delayed osteotomy healing after 3, 6 and 12 months, no secondary dislocations, and frequent troublesome irritation due to the plate being on the iliotibial band. Howe

Clinical results of isolated reconstruction of the medial patellofemoral ligament for recurrent dislocation and subluxation of the patella.

Complications and results of medial patellofemoral ligament (MPFL) reconstruction, performed between 2000 and 2003, were studied retrospectively in 24 randomised patients (24 knees). All procedures were performed for recurrent dislocation or subluxation of the patella, using an autogenous semitendinosus tendon. Two different methods of anchoring of the transplant were compared. After a 2-year follow-up, patellar stability was found to be similar, when the transplant was sutured to the fibrous tissue and periosteum overlying the patella or when it was fixed in a bone tunnel through the patella.

Differing injury patterns in snowboarding and alpine skiing.

This is a study of all people presenting to ski patrollers with ski and snowboard injuries during the 1995 season. There were 476 casualties of whom 396 were skiers and 57 were snowboarders. Information was collected retrospectively on the mechanism, region and injury, and also personal details of the casualties. The information was compared with the numbers of ski and snowboard injuries for the 1994 season. Injuries due to snowboarding increased in frequency, from 4 per cent in 1994 to 11 per cent in 1995. Snowboarders were younger, predominantly male, and were more often beginners than were

Mobility aids for children with high-level myelomeningocele: parapodium versus wheelchair.

The optimal method for providing mobility for children with myelomeningocele remains controversial. 39 children using a parapodium were compared with 29 children in wheelchairs. There were no significant differences between the two groups for medical complications, use of health-care services or activities of daily living, although there were differences in the patterns of complications. Children using the parapodium were more likely to develop lesions of the lower extremities, to have dislocated hips, to be more obese and to watch more television; children using wheelchairs were more likely t

Congenital and acquired orthopedic abnormalities in patients with myelomeningocele.

This article presents a radiologic review of the spectrum of acquired and congenital orthopedic abnormalities found in patients with myelomeningocele. These abnormalities are caused predominantly by muscle imbalance, paralysis, and decreased sensation in the lower extremity. Iatrogenic injury, such as a postoperative tethered cord, may also cause bone abnormalities. Selected images were obtained from more than 800 children. Important entities presented include spinal curvatures such as kyphosis, scoliosis, and lordosis; subluxation and dislocation of the hip, coxa valga, contractures of the hi

Patellofemoral joint abnormalities in athletes: evaluation by kinematic magnetic resonance imaging.

The patellofemoral joint is one of the most common sources of painful symptoms encountered by athletes and is one of the most frequently injured joints. Patellar incongruency is the primary pathologic condition that affects the patellofemoral joint and has been reported to be associated with patellar subluxation, patellar dislocation, chondromalacia, and arthrosis. The diagnosis of patellofemoral incongruency by physical examination alone is extremely difficult because the clinical signs may stimulate other types of internal derangements of the knee, and there is a high incidence of combined a

[Comparative study of a series of normal knees and a series of knees with patellar instability].

In studying the long-term follow-up of 39 cases of patellofemoral instability operated in our department for recurrent dislocation, we also analyzed their morphological appearance on X-ray. We compared the data given by Maldague and Malghem, Dejour and Walch for the pathological conditions of our cases with the same number of normal knees. This comparative analysis showed that measuring the bulging of the upper part of the trochlea and the depth of its groove on a lateral view is a reliable method to evaluate patellar instability. The classical axial view gives less useful information to quant

Popliteal artery trauma: update and recent advances in management.

Management of vascular trauma has been standardized in the past two decades with a significant increase in limb salvage, but trauma to the popliteal artery still remains a challenge. A seven-year experience at San Bernardino County Medical Center was comprised of 20 popliteal artery injuries in 19 patients. Thirteen injuries (65%) were from blunt trauma, four injuries (20%) were from gunshot wounds, two injuries (10%) were from close-range shotgun blasts, and one (5%) was from a stab wound. Limb salvage was 100%. Liberal use of vein interposition grafts, routine intraoperative postreconstructi

Anterior approach to the knee with osteotomy of the tibial tubercle for bicondylar tibial fractures.

Eight patients--six who had a bicondylar fracture of the tibia and two who had a complex fracture-dislocation--were treated by open reduction and internal fixation that was achieved through an anterior approach to the knee. The approach included elevation of the tibial tubercle, proximal retraction of the extensor mechanism (patellar tendon, retropatellar fat pad, and patella), and transection and detachment of the anterior horn of one or both menisci. The extent of the approach depended on the specific need for exposure. The quality of reduction was better and the rate of complications was lo

Association Between Preoperative Patellar Jumping Sign and Graft Failure After Medial Patellofemoral Ligament Reconstruction for Recurrent Patellar Dislocation.

BACKGROUND: Two recent studies have shown that patients with jumping sign exhibit more pronounced lower limb torsional abnormalities and trochlear bump in recurrent patellar dislocation (RPD), suggesting that medial patellofemoral ligament (MPFL) reconstruction may not fully correct the patellofemoral relationship in patients with a jumping sign. PURPOSE: To determine whether there was an association between the preoperative jumping sign and MPFL graft failure. STUDY DESIGN: Case-control study; Level of evidence, 3. METHODS: Patients who underwent primary surgery and follow-up evaluation du

Knee osteochondral fractures in skeletally immature patients: French multicenter study.

INTRODUCTION: Femoral or patellar osteochondral fractures complicating patellar dislocation are more frequently observed in adolescents than in adults. These incidental lesions of vulnerable joint cartilage are often neglected in the initial phase, which is regrettable given their good capacity for healing after repositioning. The objective of this study was to investigate the characteristics and analyze the results of repositioning these fractures in skeletally immature patients. MATERIAL AND METHODS: This retrospective multicenter study grouped 14 patients and knees (seven females). The mea

[Osteochondral fractures in childhood and adolescence].

BACKGROUND: Osteochondral fractures (OCF) are traumatic shearing injuries to portions of cartilage and bone. The most common cause is patellar dislocation, with the main localisation being the medial patellar facet and the lateral femoral condyle. They can occur in all age groups. DIAGNOSIS: Clinically, there is a painful knee joint effusion (haemarthrosis) with a "dancing patella". This is usually accompanied by restricted movement and/or a locking phenomenon. In addition to the standard X‑ray of the knee in three planes (lateral, anterior-posterior and tangential patella), an MRI should be

Exploring the Epidemiology of Injuries in Athletes of the Olympic Winter Games: A Systematic Review and Meta-Analysis.

This study sought to provide a comprehensive assessment of the incidence of sports injuries among athletes participating in the Olympic Winter Games and to investigate contributing factors. We gathered injury data from athletes participating in the recent four Olympic Winter Games, incorporating details on the sports event, sex, injury location, and type. Through a meta-analysis, we calculated the injury incidence rates for each sport and examined the influence of sex and the type of sport on these incidence rates. Out of 11,197 registered athletes, we documented 1,304 sports injuries. The spo

[Unusual location of osteochondral lesions in adolescent's knees].

Osteochondral knee lesions in adolescents are primarily located in loading areas at the level of the femoral condyles. Lesions located in the trochlea are exceptional and account for less than 1%. The etiology of juvenile osteochondritis dissecans (JOCD) is still unknown. Osteochondral lesions may be caused by direct frontal trauma of the femoral condyles and by direct trauma of the patella on the trochlea in dislocations of the latter. At this level both mechanisms may cause both chondral and osteochondral lesions. We present herein two cases with the same characteristics that include one pat

MR diagnosis of complete tears of the anterior cruciate ligament of the knee: importance of anterior subluxation of the tibia.

OBJECTIVE: The primary role of the anterior cruciate ligament is to resist anterior subluxation of the tibia. When the ligament is torn, the tibia is free to sublux anteriorly when the leg is pulled forward. In addition to visualizing the anterior cruciate ligament directly, MR imaging can show joint alignment. The purpose of this study was to test the efficacy of MR images of anterior subluxation of the tibia for diagnosing complete tears of the anterior cruciate ligament. MATERIALS AND METHODS: We evaluated the records of 120 consecutive patients who underwent MR imaging of the knee and sub

Normal Pressure Hydrocephalus Is Associated With Increased Risks of Postoperative Complications Following Total Knee and Total Hip Arthroplasty.

BACKGROUND: Normal pressure hydrocephalus (NPH) has not been studied as a potential risk factor for postoperative complications after primary total knee (TKA) and total hip arthroplasty (THA). METHODS: Nearly 2000 patients with a diagnosis of NPH who underwent TKA or THA from 2005 to 2014 were identified in a national insurance database and compared to 10:1 matched controls using a logistic regression analysis. RESULTS: NPH was associated with an increased risk of hospital readmission, emergency room visit, and infection following TKA (odds ratio 1.48-2.70, all P < .01). NPH was associated w
