Teaching undergraduates using Tracy Kidder's Rough Sleepers: students' perceptions of homelessness as a public health issue.

BACKGROUND: Homelessness is a pressing public health issue that is associated with adverse physical and mental health outcomes. Negative perceptions of individuals experiencing homelessness, including stereotyping and stigma, exacerbate health inequalities and impede access to care. Calls to address these issues include the incorporation of culturally competent education for future healthcare providers. This study explored the use of Tracy Kidder's Rough Sleepers as a teaching tool in an undergraduate health sciences course to deepen students' understanding of homelessness as a multifaceted pu

Perceived community environment and depressive symptoms among older adults in China: a cross-sectional analysis of physical activity as a moderator.

INTRODUCTION: Perceived community environment is an important contextual factor related to depressive symptoms in later life. However, few studies have examined whether physical activity (PA), a modifiable behavioral factor, moderates this association, particularly across specific aspects of perceived community environment and different types of PA. METHODS: This study used data from the 2023 China Longitudinal Aging Social Survey, including 9,410 older adults. Linear regression models were used to examine the association between perceived community environment and depressive symptoms and to

'I've been here before, but never looked at the ceiling': Narratives of medical trainees who took illness-based leaves.

INTRODUCTION: Medical training is commonly imagined as a linear, uninterrupted progression, yet many trainees experience illness that necessitates a leave of absence (LOA). Illness-based leaves may be especially fraught given stigma, opaque processes and training cultures that prioritise endurance. Little is known about how trainees experience illness and illness-based leaves. METHODS: We conducted a narrative inquiry using Riessman's narrative analysis framework. Canadian physicians who had taken an illness-based LOA during medical school or residency were recruited nationwide. Semi-structur

A community and clinical collaboration to optimize HIV treatment and care for people with HIV in Australia-Solutions from the Beyond Undetectable symposium.

INTRODUCTION: Australia has made strong progress toward the UNAIDS 95:95:95 targets (UNAIDS, 2025). As of 2024, an estimated 92% of people with Human Immunodeficiency Virus (HIV) in Australia had been diagnosed, 97% of those diagnosed were receiving antiretroviral therapy (ART), and 98% of those on treatment had achieved viral suppression (Kirby Institute, 2024). While these outcomes reflect a well-functioning HIV care continuum, diagnosis gaps persist, particularly among people born overseas, heterosexual men, and Aboriginal and Torres Strait Islander peoples (Kirby Institute, 2024). METHODS

Statin Initiation and Risk of Incident Alzheimer Disease and Cognitive Decline in Genetically Susceptible Older Adults.

BACKGROUND AND OBJECTIVES: The association of statin initiation with incident Alzheimer disease (AD) dementia and cognitive decline by the APOE ε4 allele is unknown. Our objective was to examine whether the association of statin initiation with incident AD dementia and cognitive decline differs by the APOE ε4 allele. METHODS: This population-based longitudinal cohort study was conducted in 4 urban communities in Chicago, IL, United States, consisting of 4,807 participants. Statin initiation is based on the inspection of medications during home assessments. Clinical diagnosis for incident AD u

Incidence and contributing factors of dementia among people living with HIV in British Columbia, Canada, from 2002 to 2016: a retrospective cohort study.

INTRODUCTION: Dementia is a progressive and debilitating disease, and people living with HIV (PLWH) often develop dementia much earlier than those not living with HIV. We estimated the incidence and prevalence of dementia and identified its key risk factors in a cohort of PLWH in British Columbia, Canada. METHODS: This retrospective cohort study used data from the Seek and Treat for Optimal Prevention of HIV/AIDS study. Eligible individuals were diagnosed with HIV, ≥40 years of age, naïve to antiretroviral therapy (ART), had no dementia at the index date and were followed for ≥1 year during 2

Sleep Misperception and Associated Factors in Patients With Anxiety-Related Disorders and Complaint of Insomnia: A Retrospective Study.

PURPOSE: Data on sleep parameters by polysomnography (PSG) in patients with anxiety-related disorders are limited. Although the disturbance and risk factors of sleep misperception have been implicated in psychopathology, its role in anxiety-related disorders remains unclear. This retrospective study aimed to explore the characteristics and sleep parameters in patients with anxiety-related disorders and different sleep perception types, and to explore the associated factors for sleep misperception. METHODS: Patients with anxiety-related disorders who had complaint of insomnia for more than 3 m

Cognitive Function, Access to Kidney Transplantation, and Waitlist Mortality Among Kidney Transplant Candidates With or Without Diabetes.

RATIONALE & OBJECTIVE: Intact cognition is generally a prerequisite for navigating through and completing evaluation for kidney transplantation. Despite kidney transplantation being contraindicated for those with severe dementia, screening for more mild forms of cognitive impairment before referral is rare. Candidates may have unrecognized cognitive impairment, which may prolong evaluation, elevate mortality risk, and hinder access to kidney transplantation. We estimated the burden of cognitive impairment and its association with access to kidney transplantation and waitlist mortality. STUDY

Exposure therapy for PTSD during pregnancy: a feasibility, acceptability, and case series study of Narrative Exposure Therapy (NET).

BACKGROUND: Prenatal posttraumatic stress disorder (PTSD) is a significant complication of pregnancy linked to increased risk of adverse perinatal outcomes. Although 1 in 5 pregnant trauma-exposed individuals have PTSD, most PTSD treatment trials exclude participants who are pregnant, and none focus on treatment specifically during pregnancy. Moreover, access to mental health treatment is particularly challenging in low-resource settings with high rates of trauma. This study examined implementation of Narrative Exposure Therapy (NET), a short-term evidence-based PTSD treatment, in an urban pre

Trauma-focused psychodynamic therapy and STAIR Narrative Therapy of post-traumatic stress disorder related to childhood maltreatment: trial protocol of a multicentre randomised controlled trial assessing psychological, neurobiological and health economic outcomes (ENHANCE).

INTRODUCTION: Success rates of psychotherapy in post-traumatic stress disorder related to childhood maltreatment (PTSD-CM) are limited. METHODS AND ANALYSIS: Observer-blind multicentre randomised clinical trial (A-1) of 4-year duration comparing enhanced methods of STAIR Narrative Therapy (SNT) and of trauma-focused psychodynamic therapy (TF-PDT) each of up to 24 sessions with each other and a minimal attention waiting list in PTSD-CM. Primary outcome is severity of PTSD (Clinician-Administered PTSD Scale for DSM-5 total) assessed by masked raters. For SNT and TF-PDT, both superiority and non

Jail-to-community treatment continuum for adults with co-occurring substance use and mental disorders: study protocol for a pilot randomized controlled trial.

BACKGROUND: Adults with co-occurring mental and substance use disorders (CODs) are overrepresented in jails. In-custody barriers to treatment, including a lack of evidence-based treatment options and the often short periods of incarceration, and limited communication between jails and community-based treatment agencies that can hinder immediate enrollment into community care once released have contributed to a cycle of limited treatment engagement, unaddressed criminogenic risks, and (re)arrest among this vulnerable and high-risk population. This paper describes a study that will develop resea

Should essays and other "open-ended"-type questions retain a place in written summative assessment in clinical medicine?

BACKGROUND: Written assessments fall into two classes: constructed-response or open-ended questions, such as the essay and a number of variants of the short-answer question, and selected-response or closed-ended questions; typically in the form of multiple-choice. It is widely believed that constructed response written questions test higher order cognitive processes in a manner that multiple-choice questions cannot, and consequently have higher validity. DISCUSSION: An extensive review of the literature suggests that in summative assessment neither premise is evidence-based. Well-structured o

Percutaneous internal fixation of proximal fifth metatarsal jones fractures (Zones II and III) with Charlotte Carolina screw and bone marrow aspirate concentrate: an outcome study in athletes.

BACKGROUND: Internal fixation is a popular first-line treatment method for proximal fifth metatarsal Jones fractures in athletes; however, nonunions and screw breakage can occur, in part because of nonspecific fixation hardware and poor blood supply. PURPOSE: To report the results from 26 patients who underwent percutaneous internal fixation with a specialized screw system of a proximal fifth metatarsal Jones fracture (zones II and III) and bone marrow aspirate concentrate. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: Percutaneous internal fixation for a proximal fifth metatars

Psychological aspects of traumatic injury in children and adolescents.

Each year millions of children are exposed to some form of extreme traumatic stressor. These traumatic events include natural disasters (e.g., tornadoes, floods, hurricanes), motor vehicle accidents, life-threatening illnesses and associated painful medical procedures (e.g., severe burns, cancer, limb amputations), physical abuse, sexual assault, witnessing domestic or community violence, kidnapping, and sudden death of a parent. During times of war, violent and nonviolent trauma (e.g., lack of fuel and food) may have terrible effects on children's adjustment. The events of September 11, 2001

Oral ivermectin in the treatment of scabies.

One hundred and twenty scabietic patients attending the outpatient clinic of the Department of Dermatology, Mansoura University Hospital, voluntarily participated in this uncontrolled, open label study to evaluate ivermectin 20 microg/kg as a scabietic after they had given their consent. The scabietic subjects included in this study were otherwise healthy, mentally competent, aged more than 18 years, and used no topical antiscabietic treatment in the week before ivermectin treatment, or during the 4-week study period. Patients were also required to show clinical evidence of scabies, and the mi

Psychological and physical effects of short-term discontinuation of feminizing gender-affirming hormone therapy among older transgender women: a within-subject clinical trial.

STUDY QUESTION: What are the psychological and physical effects of short-term discontinuation and subsequent reinitiation of feminizing gender-affirming hormone therapy (fGAHT) in older transgender women receiving long-term therapy? SUMMARY ANSWER: Short-term fGAHT discontinuation increased estradiol deprivation-related symptoms while reinitiation showed opposite effects, and psychological well-being was not affected. WHAT IS KNOWN ALREADY: With growing numbers of older transgender individuals, questions arise regarding the necessity of long-term continuation of fGAHT, particularly since cis

Exploring parents' physical activity motivation during the COVID-19 pandemic: a mixed-methods study from a self-determination theory perspective.

BACKGROUND AND OBJECTIVES: The COVID-19 lockdowns impacted physical activity for all, but especially parents, because they had to balance home, work and leisure activities. Motivation for exercise is consistently shown to be associated with physical activity levels. Self-determination theory provides a framework through which the motivation for exercise and its social-contextual antecedents can be explored. The purpose of this study is to explore the role of motivation in determining physical activity in parents and carers of English primary school children before, during and after the COVID-1
