PURPOSE: The primary purpose of this study was to measure the in vitro scaphoid and lunate motion during 9 different variations of a wrist dart throw motion. Another goal was to determine the specific dart throw motion that minimized scaphoid and lunate motion.
METHODS: Scaphoid and lunate motion were recorded in 7 cadaver forearms during various combinations of wrist dart throw motions caused by a wrist joint motion simulator.
RESULTS: During wrist flexion and extension the scaphoid and lunate motions follow the wrist motion. During wrist radial and ulnar deviation the scaphoid and lunate c

Assessment of the cost-effectiveness of early magnetic resonance imaging (MRI) for suspected radiographically occult scaphoid fractures. Methods Compare costs of patients presenting acutely with suspected scaphoid injuries (managed either with traditional follow-up radiographs and plasters) versus early MRI to exclude a fracture. Results The average medical cost for the control group was NZ470 dollars versus NZ533 dollars in the MRI group. The cost to exclude a fracture was NZ437 dollars with MRI versus NZ459 dollars for the traditional protocol. Weekly compensation costs were comparable. Conc

Isolated dislocation of the carpal scaphoid is an extremely rare injury, which is usually accompanied by significant ligamento-capsular lesions. A review of the literature found only 21 reported cases. The aim of this report is to present a single case of isolated radio-palmar dislocation of the scaphoid treated conservatively, which provided a good functional result at 10 years follow up.

BACKGROUND: The popularity of rollerblading and skateboarding activities in Norway is reflected in the number of injuries seen at our casualty centres and hospitals.
MATERIAL AND METHODS: During 1998, we made a prospective registration of 7,041 new injuries involving children below the age of 16 treated at the Accident and Emergency Department and at Haukeland University Hospital in Bergen.
RESULTS: Rollerblading and skateboarding injuries accounted for 113 cases, representing 1.6% of all injuries. Three quarters were boys, nine out of ten were 10 to 15-year-olds, and arms/hands were injured

BACKGROUND: The purpose of this study was to investigate the frequency and distribution of associated carpal bone fractures (CBFs) in distal radial fractures (DRFs).
METHODS: Three hundred and thirteen patients who underwent surgical treatment for DRFs between March 2007 and January 2010 were reviewed retrospectively. In this study, 223 patients who had preoperative computed tomography (CT) were included. We investigated the frequency and distribution of associated CBFs on CT scans. The relationship between the frequency of associated CBFs and patient factors such as age, gender, body mass in

The gross examination of 50 minium injected specimens showed us the various patterns of the posterior arterial network of the wrist. This is chiefly provided by the radial artery which gives off a constant dorsal carpal branch over the posterior aspect of the trapezium. This vessel crosses the carpus transversely to the inner border of the hand and is joined in half of the cases by the interosseous artery; contribution of the posterior carpal branch of the ulnar artery was more rarely found than usually described (25%). From the arch thus formed are given off short branches which run upwards a

Scapholunate dissociation (SLD) is the commonest cause of carpal instability and wrist osteoarthrosis. The value of early diagnosis and treatment of this injury is well established in the literature. When a partial or total rupture of the scapholunate ligament is treated with early anatomic reduction and repair, functional results may be good to excellent. However, if this ligament is not addressed acutely then an overall carpal malalignment may seem progressively as a result of failure of the secondary scaphoid stabilizers. Chronic SLD will lead to scapholunate advanced collapse and progressi

PURPOSE: A high incidence of nonunion and relatively poor outcomes with prior fixation techniques has precluded scapholunate (SL) arthrodesis as a standard treatment for SL instability. Our purpose was to determine the impact on range of motion (ROM) of simulated SL arthrodesis via headless screw fixation.
METHODS: We performed baseline wrist ROM for 10 cadaveric wrists using a standardized mounting-and-weights system. Extension, flexion, radial deviation, ulnar deviation, dart-thrower's extension, and dart-thrower's flexion were assessed. Two 3.0-mm headless compression screws were inserted

PURPOSE: We studied 2 methods used for screw placement through a volar approach for fixation of scaphoid fractures.
METHODS: We performed measurements on 20 computed tomography scans of unfractured scaphoids. A central virtual guidewire was computed in 10 scaphoids with the wrist in neutral or in extension and ulnar deviation. Second, we compared the central guidewire and a guidewire representing a volar approach to the scaphoid avoiding the trapezium.
RESULTS: The central guidewire passed through the trapezium in all cases with the wrist either in neutral or in extension and ulnar deviation

One method of Herbert screw insertion is to mobilize the scaphotrapezial joint and insert the screw through the articular surface of the distal scaphoid. Because of concern that this might predispose to osteoarthritis in the scaphotrapezial joint, we have investigated joint space width and the presence of osteophytes adjacent to the scaphotrapezial joint using high definition macroradiography.Twenty-eight patients with symptomatic primary osteoarthritis of their hands, 23 with a scaphoid fracture successfully treated conservatively and 18 treated with a Herbert screw were studied. The patients

The aim of our study was to assess the anatomical relationships between wrist arthroscopic portals and the periscaphoid vascular network. Fourteen fresh human cadaver wrists were perfused. Needles were placed in the 3–4, 6R, midcarpal radial (MCR), midcarpal radial scaphoid (MCRS), and scaphotrapezium-trapezoid (STT) portals. Contrast-enhanced CT scans were performed to analyze the constant arteries, their anatomical relationships, and their distance from the needles. Scaphoid vascularization mainly consisted of a dorsal branch along the dorsal ridge and a palmar branch opposite the waist. Our

CASE: A 55-year-old professional flutist developed a scaphoid waist nonunion that did not receive initial treatment. Six months later, she sustained an ipsilateral distal radius fracture for which cast immobilization was applied as conservative treatment. This approach facilitated healing of the distal radius fracture and serendipitously led to spontaneous healing of the scaphoid nonunion.
CONCLUSION: This case illustrates an unusual healing response in a scaphoid nonunion, which typically requires surgical intervention. The spontaneous healing observed in this case following conservative tre

PURPOSE: The aim of the study is to perform a systematic review and meta-analysis comparing the diagnostic performance of artificial intelligence (AI) and human readers in the detection of wrist fractures.
METHOD: This study conducted a systematic review following PRISMA guidelines. Medline and Embase databases were searched for relevant articles published up to August 14, 2023. All included studies reported the diagnostic performance of AI to detect wrist fractures, with or without comparison to human readers. A meta-analysis was performed to calculate the pooled sensitivity and specificity

BACKGROUND: Dynamic radiocarpal instability is one of the causes of post-trauma radial-sided wrist pain. It is not easy to diagnose and may possibly be overlooked. The key ligaments responsible for dynamic radiocarpal instability are the radioscaphocapitate (RSC) and long radiolunate (LRL) ligaments. Tensioning of these 2 ligaments could be a method of treatment for dynamic carpal instability. We proposed a method for arthroscopic thermal shrinkage of these 2 ligaments, and for setting a landmark arthroscopically to facilitate identification of these 2 ligaments during the combined open suture

Vascularized osteochondral flaps have been described for use in reconstruction of the wrist, providing the benefits of osteochondral grafts in addition to the benefit of osseous and subchondral perfusion via the microvascular pedicle. Various harvest sites have been described including the medial and lateral femoral trochlea and the proximal third metatarsal. The reconstructed surfaces described include the cartilage surfaces of the scaphoid, lunate, capitate and radius. The ability to transfer analogous osteochondral surfaces poses the possibility for new motion-preserving alternatives for di

This case report describes a patient who presented with a complete rupture of the flexor pollicis longus (FPL) tendon and partial rupture of the flexor digitorum profundus of the index finger, secondary to scaphoid nonunion. This is a rare, late complication that deserves to be described because of the potential diagnostic confusion with anterior interosseous nerve palsy. No case has been reported since 1999 in the literature. The mechanism was an attrition rupture due to sharp osteophytes. The scaphoid osteophytes were removed and the FPL was repaired by tendon transfer. The results were sati

Idiopathic avascular necrosis of the scaphoid, also called Preiser's disease, has been diagnosed in only 150 cases. Its clinical and radiological description is well established, but there is no consensus regarding its treatment. We report the case of a 53-year-old woman, with an advanced disease; treated arthroscopically by partial resection of the scaphoid, and insertion of an adapted pyrocarbon implant. Preliminary results at 6 months were satisfying in terms of pain. However, there was little improvement in range of motion, with a final range of 150°.

The authors report 2 cases of concomitant scaphoid and distal radial fractures, a rare combination of lesions in children. The first case was in a 13-year-old boy who presented a transverse midscaphoid fracture associated with a Salter type II distal radial fracture. The second concerned a 10-year-old boy who presented a bilateral wrist injury. On the left side, a distal forearm fracture with anterior displacement was associated with a transverse midscaphoid fracture. On the right side, there was only a distal forearm fracture. Scaphoid fracture associated with distal radial fracture seems to
