INTRODUCTION: Both the proximal humerus fracture and the acromioclavicular (AC) joint injury are commonly found in shoulder girdle injuries but there are no reports of them presenting together. The mechanism of the AC joint injury is similar to that of the proximal humerus fracture, a lateral impact to the shoulder girdle, but the arm positions are different, as the AC injury usually involves an adducted arm while the proximal humerus fracture normally occurs with the arm in a neutral position. Herein we report, to our knowledge, the first case of a combined proximal humerus fracture and AC jo

Injury to the axillary artery following anterior shoulder dislocation is a very rare occurrence. This review serves to illustrate the now classical case of an elderly gentleman with a recurrent dislocation, transection of the axillary artery and its invariable association with a severe brachial plexus lesion, which is the most important determinant of long-term disability. It also highlights the pathognomic triad of anterior shoulder dislocation, expanding axillary haematoma and diminished peripheral pulse, to highlight awareness of this important injury. The literature on this injury has been

The shoulder achieves a wide spectrum of motion, and in a subset of patients, including those who use manual wheelchairs and upper extremity walking aids, the shoulder also serves as the primary weight-bearing joint. Because the weight-bearing shoulder is subject to considerable joint reaction forces and overuse, a broad spectrum of pathology can affect the joint. The combination of muscle imbalance and repetitive trauma presents most commonly as subacromial impingement syndrome but can progress to other pathology. Patients with high-level spinal cord injury, leading to quadriplegia and motor

Proximal fractures of the humerus resulting in intrathoracic displacement of the humeral head are rare, but cause serious pain and can lead to pulmonary and glenohumeral complications. Many treatment options exist, but there is still no literature regarding mortality and morbidity associated with rib fixation in concordance with treatment of the proximal humerus. This case describes a patient who was treated using a multi-team approach that involved shoulder arthroplasty with rib fixation, and which resulted in decreased pain and decreased narcotic use while improving the patient's pulmonary f

Most obstetric brachial plexus palsies are due to rupture of the upper roots in babies whose delivery was complicated by shoulder dystocia. If treated by early exploration and grafting, they have a favourable prognosis. We reviewed 36 babies who had had an obstetric brachial plexus palsy after a breech delivery and found that they had a different pattern of injury; 81% had avulsion of the upper roots. This injury cannot be treated satisfactorily by exploration and microsurgical grafting and carries a considerably worse prognosis for shoulder function.

We present a case of traumatic L4-5 bilateral facet dislocation, without neurological deficit, in a 32-year-old female patient, as an unusual seatbelt injury caused by positioning the shoulder harness improperly under her armpit. Open reduction, posterior interbody fusion, and posterior segmental instrumentation were carried out. The aim of this report is to describe this rarely encountered condition and speculate regarding automotive shoulder harness misuse as a potential cause of bilateral locked facet at L4-5, and to emphasize the importance of multidirectional X-ray on first examination. Posterosuperior glenoid internal impingement (PGII) is an impingement syndrome of the shoulder that is most commonly seen in the throwing or overhead athlete. The supraspinatus can be normally compressed or impinged between the greater tuberosity and the posterosuperior labrum in the abduction and external rotation position. However, repetitive throwing and biomechanical abnormalities may lead to the intensification of this contact and to the clinical and pathological picture of PGII. The injured athlete usually complains of poor throwing performance and pain located in the posterosuperior asp

BACKGROUND: With the advent of arthroscopy, more partial subscapularis tears are being recognized. The biomechanical effects of partial subscapularis tears are unknown, and there is no consensus as to their treatment. Therefore, the objective of this study was to evaluate and to quantify the changes in range of motion and glenohumeral kinematics for isolated subscapularis partial tears, combined subscapularis and supraspinatus tears, supraspinatus repair, and combined supraspinatus and subscapularis repair.
METHODS: Six cadaveric shoulders were tested in the scapular plane with 0°, 30°, and 6

In the detection of SLAP (superior labral anterior to posterior) lesions of the shoulder MR arthrography shows a significantly higher sensitivity compared with conventional MR techniques and therefore, represents the method of choice in diagnostic imaging of the superior labral-bicipital complex. On the basis of morphological criteria it mostly allows distinction of traumatic lesions and anatomic variants of the superior labrum and the biceps anchor as well as classification of detected SLAP lesions (types 1-4 according to Snyder). However, the differentiation of a SLAP type 2 lesion and a sub

OBJECTIVE: To evaluate the ability of ultrasonography to detect labral lesions in patients with anterior shoulder dislocation.
METHODS: We examined 29 patients with anterior shoulder dislocation with ultrasonography prior to arthroscopy.
RESULTS: Twenty-six patients had labral lesions diagnosed by arthroscopy. These lesions were detected by ultrasonography with a sensitivity of 88% and a specificity of 67%. All lesions affected the anterior labrum, while six extended to the posterior labrum. The latter were not visualized by ultrasonography.
CONCLUSION: Ultrasonography is valuable in the de

CONTEXT: Athletic trainers use clinical pain and range of motion (ROM) to gauge recovery after musculoskeletal injury. Limited evidence to date suggests which shoulder ROM measures can predict symptomatic relief and functional recovery after delayed-onset muscle soreness (DOMS).
OBJECTIVE: To determine whether shoulder passive internal rotation, passive external rotation, active abduction, and active flexion and evoked pain with abduction are associated with resting pain experienced after exercise-induced DOMS.
DESIGN: Descriptive laboratory study.
SETTING: Controlled research laboratory.

CONTEXT: Proper scapular function during humeral elevation, such as upward rotation, external rotation, and posterior tilting of the scapula, is necessary to prevent shoulder injury. However, the appropriate intensity of rehabilitation exercise for the periscapular muscles has yet to be clarified.
OBJECTIVE: To identify the serratus anterior, lower trapezius, infraspinatus, and posterior deltoid muscle activities during 2 free-motion exercises using 3 intensities and to compare these muscle activities with isometric contractions during quadruped shoulder flexion and external rotation and abdu

Obstetric brachial plexus injury, also known as neonatal brachial plexus injury, is not unusual in newborns. Given the lack of a comprehensive synthesis of the available data on the effectiveness of botulinum neurotoxin (BoNT) in treating children with obstetric brachial plexus injury, we conducted a systematic review and meta-analysis. We searched PubMed, Embase, Web of Science, and Cochrane databases from inception to 25 November 2022. Outcomes were function of the shoulder and elbow joints, muscle power of the deltoid, biceps brachii and triceps brachii, and the recurrence rate of subluxati

The subacromial and subdeltoid bursa commonly communicate and, for all intents and purposes, function as a single bursa. The subacromial bursa lies between the supraspinatus and the acromion process, while the subdeltoid bursa is the lateral extension of the subacromial bursa. These bursae function to limit frictional forces between the various structures. These bursae can become irritated and inflamed when they are impinged between the greater tubercle of the humerus and the acromion and/or the coracoacromial ligament when one lifts the shoulder into elevation and internal rotation. It is dif

OBJECTIVES: One of the most common reasons for shoulder instability is recurrent shoulder instability. Arthroscopic repair is an option in the surgical treatment of Bankart lesions, and it is indicated if certain criteria are met. Recent evidence suggests that neurobehavioral traits may predispose individuals to orthopedic injuries. Therefore, we aimed to investigate the correlation between psychiatric parameters and recurrent dislocations in patients treated with arthroscopic Bankart repair.
METHODS: A total of 83 patients were included in the study, including 42 patients who underwent arthr
