[Cannulation of the Portal Vein during Endoscopic Retrograde Cholangiopancreatography in a Patient with Choledocholithiasis].

Cannulation of the portal vein is a rare complication of ERCP. This paper reports a case of portal vein catheterization during ERCP in a patient with choledocholithiasis. A 62-year-old man was admitted to the Presbyterian Medical Center with right upper quadrant pain and jaundice. ERCP was performed under the suspicion of obstructive jaundice caused by a radiolucent stone. Bile duct cannulation using a pull-type papillotome was attempted, but it failed. After needle-knife fistulotomy, wire-guided cannulation was performed successfully, and 10 mL contrast was injected. On the other hand, the fl

Is endoscopic papillary balloon dilatation really a risk factor for post-ERCP pancreatitis?

Endoscopic papillary balloon dilatation (EPBD) is useful for decreasing early complications of endoscopic retrograde cholangio-pancreatograp … eviewing past studies. We reviewed thirteen randomised control trials comparing EPBD and endoscopic sphincterotomy (EST) and ten studies comparing direct EPLBD and EST. Three randomized controlled trials of EPBD showed significantly higher incidence of PEP than EST, but no study of EPLBD did. Careful analysis of these studies suggested that longer and higher-pre …

[Treatment of calculous cholecystitis and its complications].

Results of treatment of 10,724 patients with different forms of acute and chronic cholecystitis are analyzed. Surgical treatment was performed in 7819 (72.9%) patients. Variants of treatment of acute and chronic cholecystitis are presented. Typical cholecystectomy is the basic surgery in patients with acute calculous cholecystitis (63% procedures). Laparoscopic cholecystectomy (LCE) was performed in 37% patients. Two-stage surgeries with previous microcholecystostomy (MCS) and endoscopic papilloshincterotomy (EPST) are indicated in late hospitalization of patients with intoxication and severe

[Endoscopic methods in the treatment of the obturation of the papilla magna duodeni].

Endoscopic methods of treatment were applied in 320 patients with the duodenal papilla magna (DPM) obturation. After performance of endosurgical correction of the bile outflow the pain disappeared in 54%, subsided significantly in 38% patients. In 48 patients endoscopic suprapapillar choledochoduodenostomy was performed, cryodestruction of tumor--in 17 for cancer affection of DPM. In 197 patients with wedged ampullar DPM calculus the atypical papillosphincterotomy was performed successfully. Three (0.9%) patients died.

Pancreatic duct stenting in benign pancreatic disease.

The ability to place endoscopic stents into the pancreatic duct has led to a dramatic increase in stent therapy for benign pancreatic diseases, particularly chronic pancreatitis and pancreas divisum. The overall effectiveness of this therapy remains unknown. This article critically reviews the available literature with a focus on patient selection, efficacy, and risk. The risk of pancreatic duct stenting is only now beginning to be appreciated, and clinicians must understand not only the potential effectiveness of pancreatic duct stenting but also the magnitude of the potential risk when consi

Endoscopic therapy of chronic and recurrent pancreatitis.

Endoscopic pancreatic therapy is emerging as an exciting nonoperative alternative in the management of chronic pancreatitis, in particular in cases where pancreatic duct obstruction due to stones, protein plugs, or strictures is present. Although the preliminary results available are promising, prospective randomized trials comparing endoscopic and surgical techniques are much needed. Furthermore, the published data come from large endoscopic centers with very high levels of technical skill and experience, and, therefore, its wide-spread clinical use is limited. Time and well-designed prospect

Endoscopic treatment in chronic pancreatitis.

Endoscopic treatment of chronic pancreatitis has drawn benefits from endoscopic procedures previously described for the main bile duct. Endotherapy is developing throughout the world. Cyst drainage procedures certainly represent the largest step forward in that non-surgical approach, whatever it is either direct (through the stomach or the duodenum) or indirect through the papilla in the duodenum. This procedure gives similar results to surgery with a lower morbidity. Pancreatic duct drainage associated with stone clearance is feasible and provides good results which have not yet been compared

Common bile duct stones: management strategies in the laparoscopic era.

We reviewed our experience with the treatment of common bile duct (CBD) stones in 70 patients by sequential endoscopic-laparoscopic manageme … patients with dilated CBD, because insufficient opening of the ampulla of Vater made by endoscopic sphincterotomy (EST) may lead to stasis and reflux-related complications such as cholangitis and recurrent stones. We conclude that the most rational management of CBD stones should be decided according to the size of the CBD, which depends on the size, number, an …

[Endoscopic treatment methods in patients with the postcholecystectomy syndrome].

458 patients with postcholecystectomy syndrome have been examined. In 289 (63.1%) of them the reason of complication and location of the lesion in biliopancreatic-duodenal zone have been specified. In 212 (73.4%) patients the endoscopic treatment was used, including papillosphincterotomy with removal of concrements; suprapapillary choledochoduodenostomy; nasobiliary draining, endoprosthesis. In 181 (85.4%) of cases these methods appeared to be efficient and final; in 31 (14.6%) they promoted stabilization of clinical status of patients and performing surgery in more favorable conditions. Compl…

Pancreatitis and pancreas divisum: aetiological and surgical considerations.

It has been suggested that acute pancreatitis occurs more commonly in patients with pancreas divisum and that these patients may respond to surgery aimed at improving pancreatic ductal drainage. We have studied the incidence of pancreas divisum in patients referred for endoscopic retrograde cholangio pancreatography (ERCP) and the results of surgical sphincteroplasty in a separate series of such patients. Twenty-three patients with pancreas divisum were identified among 336 successful pancreatograms (Group A), an incidence of 6.8%. The incidence of pancreas divisum in patients having ERCP for

Vascular anomaly as a cause of late bleeding after endoscopic retrograde cholangiopancreatography: A case report.

BACKGROUND: Post-endoscopic retrograde cholangiopancreatography (ERCP) bleeding is a serious complication, and related case reports have described various bleeding events following ERCP, including injury to the right gastroepiploic artery, bleeding from biliary varices, retroperitoneal hematoma in liver transplant recipients and subcapsular liver hematoma after ERCP. CASE SUMMARY: We present a case of a 55-year-old male patient who developed post-ERCP hemorrhage one month after undergoing ERCP, endoscopic sphincterotomy, and bile duct stone removal for acute biliary pancreatitis. The patient …
