Experimental pancreatic transplantation was performed in dogs using different surgical techniques. Ten dogs underwent whole organ graft with diversion of exocrine secretion by anastomosis of duodenal segment to bladder (six animals) and of duodenum to a jejunal loop (four animals). Segmental pancreatic grafts were performed in 10 dogs with ductal filling with polymer in six animals and duct ligation in four dogs. It is concluded that segmental pancreatic graft with ductal filling with polymer is preferable to the other surgical techniques.

Organ transplantation in children has to consider the development of the children. One limitation is the organ size of orthotopic transplanted organs as liver, heart and lung. The activation of unspecific immune response in the pediatric recipients is a risk of rejection episodes especially in infants. The experiences of the Medical School Hannover with 203 pediatric renal recipients and with 117 pediatric liver recipients are reported. Congenital disorders with chronic organ insufficiency frequently lead to growth retardation and cardiovascular complications already before transplantation and

Our experience of toxoplasmosis in a series of 33 heart transplant cases was reviewed. One fatal infection was observed in a "mismatched" seronegative recipient of a heart from a seropositive donor. Two similar mismatched cases were given pyrimethamine prophylaxis and remained well. One organ recipient was found to have primary toxoplasmosis during the immediate preoperative assessment. The patient was treated with conventional therapy and remained asymptomatic in the postoperative period. Secondary reactivation of chronic infection and diagnostic confusion caused by passively acquired antibod

In this study, we examined the influence of pre-existing obesity (weight more than 120 per cent of ideal body weight) on outcome after renal transplantation. Among 263 cyclosporine-treated recipients of renal allografts, 223 (85 per cent) were nonobese and 40 (15 per cent) were obese prior to transplantation. Obese and nonobese recipients were similar with regard to age, sex, renal diagnosis, history of prior transplant, donor source, pretransplant blood pressure, pretransplant antihypertensive agents, diabetic recipients with insulin requirements more than 40 units per day and pretransplant s

Orthotopic cardiac transplantation has been performed in 15 consecutive neonates and children since 1987. Diagnoses include hypoplastic left heart syndrome (5 patients), critical aortic stenosis with small left ventricle (1 patient), complex cyanotic heart disease (6 patients), and cardiomyopathy (3 patients). Twelve patients survived operation and have been followed from 1 to 45 months. Patients less than 6 years of age are managed with cyclosporine +/- azathioprine; in older patients steroid weaning is attempted. Monitoring for rejection is performed with serial echocardiography in patients

Women on regular dialysis are usually infertile, but contraception should not be neglected. Pregnancy is invariably complicated and poses excessive risks, with an uncertain and low chance of success. Even when therapeutic abortion is excluded, the live birth outcome at best is 19%. Renal transplantation usually reverses abnormal reproductive function and comprehensive pre-pregnancy counseling is essential, with discussion of all implications, including the harsh realities of long-term maternal survival. In this survey of 2,309 pregnancies in 1,594 women, therapeutic abortion was undertaken in

Since cyclosporin A was introduced clinically, transplantation of solid organ grafts, has become a routine therapy for untreatable endstage-diseases of various organs, such as kidney, liver, heart and lung. Nowadays the most frequent cause of mortality and severe morbidity in transplant recipients is not graft rejection but infection. During the first three postoperative months organ recipients are extremely endangered for infectious diseases. Patients receive high dosages of immunosuppressive therapy, because immunogenecity of the graft is rather high. In course of the following months the al

The timing of renal transplantation in infants is controversial. Between 1965 and 1989, 79 transplants in 75 infants less than 2 years old were performed: 23 who were 12 months or younger, 52 who were older than 12 months; 63 donors were living related, 1 was living unrelated, and 15 were cadaver donors; 75 were primary transplants and 4 were retransplants. Infants were considered for transplantation when they were on, or about to begin, dialysis. All had intra-abdominal transplants with arterial anastomosis to the distal aorta. Sixty-four per cent are alive with functioning grafts. The most f…

The authors review the indications and outcome of liver transplantation for hepatic cirrhosis of various aetiologies in adults, on the basis of their experience and that of the literature. Up to 1989, they performed 107 liver transplantations in 93 adults, including 70 patients with cirrhosis: primary biliary cirrhosis (PBC) in 20, primary sclerosing cholangitis (PSC) in 2, secondary biliary cirrhosis in 1, post-necrotic cirrhosis in 35, alcoholic cirrhosis in 5, metabolic cirrhosis in 7. The best indications are PBC with rising bilirubinemia (greater than 5 mg/dl) or portal hypertension and P

Nocardia is an increasingly important opportunistic pathogen in immunosuppressed patients, especially solid organ transplant recipients. Infection is most often caused by Nocardia asteroides and presents as lung disease in 80% to 90% of infected transplant patients. Radiographic findings are nonspecific and include nodular infiltrates, cavitation, and pleural effusion. Dissemination to other organs, especially to the central nervous system (CNS) and skin, occurs in up to 40% of transplant recipients. A presumptive diagnosis is most rapidly made by the direct visualization in tissue, lower resp

To date, more than 200 human heart-lung transplantations (HLT) have been performed in USA and Europe. The main indications are still primary pulmonary hypertension and Eisenmenger's syndrome but the intervention has also been employed in cases of parenchymatous pulmonary disease. At operation, the thoracic organ is replaced. The recipient's heart can frequently be donated. Recent reviews suggest acceptable two-year survival following HLT of approximately 60%. The direct economical costs of transplantation have been stated to be approximately 17,000 pounds but to this must be added the expenses

Since the successful clinical reintroduction of heart and lung transplantation in 1981, more than 350 of these procedures have been performed worldwide. Although survival following this operation is less than that reported for heart transplantation, the results are improving. It is clear that the increased technical difficulty of the procedure combined with the exquisite susceptibility of the transplanted lung to postoperative injury from infection, rejection or other causes account for these differences. In this report we provide an overview of the experience in heart-lung transplantation and

Successful transplantation of the endocrine pancreas in Type I diabetes with complete correction of the glucose metabolic abnormalities may prevent progression or development of the microangiopathic complications of the disease. This could be achieved by transplantation of the whole pancreas as a vascularized graft (the only available clinical approach at present), transplantation of isolated islets or of fetal pancreas. Since transplantation of the pancreas as a vascularized graft is associated with a significant peri-operative morbidity and requires immunosuppression to prevent rejection it

Clinical application of heart transplantation goes beyond 20 years of experience. The results achieved on the first 8,000 transplanted patients are impressive with a 70% survival rate at 7 years of follow-up. Indications for transplantation are less restrictive and include diabetics and patients older than age 60. Pulmonary hypertension still remains difficult to define. Immunosuppressive therapy, now including OKT3 monoclonal antibody in addition to conventional drugs and cyclosporine, lessened the frequency and severity of rejection and infection but still need improvements. The path of successful organ transplants has been sown with considerable ethical, legal, social, and moral dilemmas. Some of these dilemmas have been addressed to the public's satisfaction, but many questions are left to be answered. The possibility of prolonging a life through high-technology medicine such as transplantation and through the use of drugs, treatment, and mechanical devices has become an everyday occurrence. However, the need for organs and tissues has created an environment in which hard choices must be made. We must all be involved in the decision-making process, as these are …

A pregnancy after an orthotopic liver transplantation was associated with abruptio placentae and preterm delivery at 27.5 weeks. The mother's immediate postpartum period was complicated by a midgut volvulus through a mesenteric defect in the Roux-en-Y loop. Both mother and infant survived after prolonged hospitalization.

There is surprisingly little about the problems of transplant nephrectomy reported in the literature, despite the frequency of the procedure and the recognition that it can be a difficult technical challenge. Of 480 kidneys transplanted during a ten year period, 154 (32 per cent) were later removed. One hundred and eleven (72 per cent) nephrectomies were performed because of uncontrollable acute rejection within three months of transplantation. Thirty-eight (25 per cent) nephrectomies were performed because of graft failure due to chronic rejection and five (3 per cent) were carried out for ot

Renal transplantation is usually accompanied by an improvement in reproductive function. The possibility of conception in women of childbearing age emphasizes the need for counseling, and couples who want a child should be encouraged to discuss all implications, with the advice based on strict guidelines. If a recipient becomes pregnant, she must be monitored as a high-risk patient. Management requires particular attention to BP control, renal function, and all infection, as well as fetal surveillance. Just under 40% of conceptions do not go beyond the first trimester, but of those that do, gr
