The effect of standardized, computer-guided templates on quality of VA disability exams.

BACKGROUND: The costs and limitations of clinical encounter documentation using dictation/transcription have provided impetus for increased use of computerized structured data entry to enforce standardization and improve quality. The purpose of the present study is to compare exam report quality of Veterans Affairs (VA) disability exams documented by computerized protocol-guided templates with exams documented in the usual fashion (dictation). METHODS: Exam report quality for 17,490 VA compensation and pension (C&P) disability exams reviewed in 2005 was compared for exam reports completed by

A magnetic resonance imaging study of modifications to the infraclavicular brachial plexus block.

A previously described infraclavicular brachial plexus block may be modified by using a more lateral needle insertion point, while the patient abducts the arm 45 degrees or 90 degrees. In performing the modified block on patients abducting 45 degrees, we often had problems finding the cords of the brachial plexus. Therefore, we designed an anatomic study to describe the ability of the recommended needle direction to consistently reach the cords. Additionally, we assessed the risk of penetrating the pleura by the needle. Magnetic resonance images were obtained in 10 volunteers. From these image

A real-time optimal inverse planning for Gamma Knife radiosurgery by convex optimization: description of the system and first dosimetry data.

OBJECTIVEThe authors developed a new, real-time interactive inverse planning approach, based on a fully convex framework, to be used for Gamma Knife radiosurgery.METHODSThe convex framework is based on the precomputation of a dictionary composed of the individual dose distributions of all possible shots, considering all their possible locations, sizes, and shapes inside the target volume. The convex problem is solved to determine the plan, i.e., which shots and with which weights, that will actually be used, considering a sparsity constraint on the shots to fulfill the constraints while minimi

ECGSight: A Digital Caliper Application for Teaching Electrocardiogram Interpretation in Undergraduate Medical Education.

The interpretation of the electrocardiogram (ECG) is a core competency for medical students and is traditionally taught using paper ECG tracings and physical calipers. As clinical practice increasingly shifts toward digital ECG records, the use of physical calipers on computer screens has become impractical, often leading students to rely passively on automated machine-generated interpretations. To address this gap, we developed ECGSight, an open-source, interactive application built using R and Shiny. ECGSight functions as a digital caliper, enabling users to upload any ECG image, calibrate t

A visual imagery paradigm for BCI strategies using imagined flickering patterns.

Steady state visually evoked potentials (SSVEPs) are a popular type of control signals in brain-computer interfaces (BCIs), in which they are typically elicited by observing a visual stimulus flashing at a specific frequency. For some patients, using SSVEP as control signal for a BCI can be difficult, for instance if they are unable to focus their gaze over the visual stimuli. To address this issue, some approaches were presented to design a gaze-independent SSVEP-controlled BCI but some difficulties have been reported, for instance for patients suffering from locked-in syndrome. In this work

On the need to better specify the concept of "control" in brain-computer-interfaces/neurofeedback research.

Aiming at a better specification of the concept of "control" in brain-computer-interfaces (BCIs) and neurofeedback (NF) research, we propose to distinguish "self-control of brain activity" from the broader concept of "BCI control", since the first describes a neurocognitive phenomenon and is only one of the many components of "BCI control". Based on this distinction, we developed a framework based on dual-processes theory that describes the cognitive determinants of self-control of brain activity as the interplay of automatic vs. controlled information processing. Further, we distinguish betwe

Three-dimensional augmented reality surgical navigation with hybrid optical and electromagnetic tracking for distal intramedullary nail interlocking.

BACKGROUND: Intramedullary nail (IMN) fixation improves functional outcomes for the diaphyseal fractures of long bones. However, locking the distal holes of the IMN is challenging. Although several navigation methods have been proposed, an intuitive, radiation-free and accurate navigation method that does not suffer from problems of hand-eye coordination has not yet been described. METHODS: This paper proposes an augmented reality (AR) surgical navigation method with hybrid optical and electromagnetic (EM) tracking for distal IMN interlocking. Model experiments are designed to evaluate the AR

Work system factors influencing physicians' screen sharing behaviors in primary care encounters.

OBJECTIVE: While the use of electronic health records (EHRs) in primary care has increased dramatically, its potential benefits need to be considered in light of potential negative impacts on physician-patient interactions and the increase in physician cognitive workload. This study aims to understand work system factors contributing to physicians' use of the EHR as a communication tool during primary-care encounters. METHODS: We interviewed 14 primary care physicians on their use of EHRs as a communication tool in patient visits. A qualitative content analysis guided by the work system model

Damage to white matter pathways in subacute and chronic spatial neglect: a group study and 2 single-case studies with complete virtual "in vivo" tractography dissection.

The exact anatomical localization of right hemisphere lesions that lead to left spatial neglect is still debated. The effect of confounding factors such as acute diaschisis and hypoperfusion, visual field defects, and lesion size may account for conflicting results that have been reported in the literature. Here, we present a comprehensive anatomical investigation of the gray- and white matter lesion correlates of left spatial neglect, which was run in a sample 58 patients with subacute or chronic vascular strokes in the territory of the right middle cerebral artery. Standard voxel-based corre

Esthetics and smile characteristics from the layperson's perspective: a computer-based survey study.

BACKGROUND: The paradigm shift from occlusion to esthetics places more emphasis on the subtle relationships among the teeth and the interplay with the soft tissues. The authors of this study quantified the ideal and maximum acceptable deviations for smile characteristics. METHODS: The authors created a survey by using a digital image editing software package, which enabled raters to manipulate intraoral photographs featured in the survey. They altered smile characteristics in photos of a sex-neutral face showing nasal tip to mentolabial fold. The authors administered an electronic survey in B

Text - background polarity affects performance irrespective of ambient illumination and colour contrast.

In a series of experiments, proofreading performance was consistently better with positive polarity (dark text on light background) than with negative polarity displays (light text on dark background). This positive polarity advantage was independent of ambient lighting (darkness vs. typical office illumination) and of chromaticity (black and white vs. blue and yellow). A final experiment showed that colour contrast (red text on green background) could not compensate for a lack of luminance contrast. Physiological measures of effort and strain (breathing rate, heart rate, heart rate variabilit

RLAgent: retrieval-augmented LLM agents for code-free RNA region-ligand interaction modeling.

Background. Unlike full reading, 'skim-reading' involves the process of looking quickly over information in an attempt to cover more material whilst still being able to retain a superficial view of the underlying content. Within this work, we specifically emulate this natural human activity by providing a dynamic graph-based view of entities automatically extracted from text. For the extraction, we use shallow parsing, co-occurrence analysis and semantic similarity computation techniques. Our main motivation is to assist biomedical researchers and clinicians in coping with increasingly large a

SLICE (SMARTS and Logic In ChEmistry): fast generation of molecules using advanced chemical synthesis logic and modern coding style.

While virtual libraries of synthetically accessible compounds have exploded in size to many billions, our capacity to extract valuable drug leads from these vast databases remains limited by computational resources. To overcome this, we developed SLICE SMARTS and Logic In ChEmistry), a powerful new tool designed for the agile exploration of massive chemical spaces. SLICE enables the fast, "à la carte" generation of virtual compound libraries through chemist-defined reaction chemistries and readily available building blocks. Its user-friendly, no-code graphical interface, the SLICE Designer, al

Transcranial focused ultrasound to V5 enhances human visual motion brain-computer interface by modulating feature-based attention.

A brain-computer interface (BCI) enables users to control devices with their minds. Despite advancements, non-invasive BCIs still exhibit high error rates, prompting investigation into the potential reduction through concurrent targeted neuromodulation. Transcranial focused ultrasound (tFUS) is an emerging non-invasive neuromodulation technology with high spatiotemporal precision. This study examines whether tFUS neuromodulation can improve BCI outcomes, and explores the underlying mechanism of action using high-density electroencephalography (EEG) source imaging (ESI). As a result, V5-targete

Real-Time "Eye-Writing" Recognition Using Electrooculogram.

Eye movements can be used as alternative inputs for human-computer interface (HCI) systems such as virtual or augmented reality systems as well as new communication ways for patients with locked-in syndrome. In this study, we developed a real-time electrooculogram (EOG)-based eye-writing recognition system, with which users can write predefined symbolic patterns with their volitional eye movements. For the "eye-writing" recognition, the proposed system first reconstructs the eye-written traces from EOG waveforms in real-time; then, the system recognizes the intended symbolic inputs with a reli

Effect of mindfulness meditation on brain-computer interface performance.

Electroencephalogram based brain-computer interfaces (BCIs) enable stroke and motor neuron disease patients to communicate and control devices. Mindfulness meditation has been claimed to enhance metacognitive regulation. The current study explores whether mindfulness meditation training can thus improve the performance of BCI users. To eliminate the possibility of expectation of improvement influencing the results, we introduced a music training condition. A norming study found that both meditation and music interventions elicited clear expectations for improvement on the BCI task, with the st

MIRD Pamphlet No. 28, Part 1: MIRDcalc-A Software Tool for Medical Internal Radiation Dosimetry.

Medical internal radiation dosimetry constitutes a fundamental aspect of diagnosis, treatment, optimization, and safety in nuclear medicine. The MIRD committee of the Society of Nuclear Medicine and Medical Imaging developed a new computational tool to support organ-level and suborgan tissue dosimetry (MIRDcalc, version 1). Based on a standard Excel spreadsheet platform, MIRDcalc provides enhanced capabilities to facilitate radiopharmaceutical internal dosimetry. This new computational tool implements the well-established MIRD schema for internal dosimetry. The spreadsheet incorporates a signi

Evaluation of in-house, haptic assisted surgical planning for virtual reduction of complex mandibular fractures.

PURPOSE: The management of complex mandible fractures, i.e. severely comminuted or fractures of edentulous/atrophic mandibles, can be challenging. This is due to the three-dimensional loss of bone, which limits the possibility for accurate anatomic reduction. Virtual surgery planning (VSP) can provide improved accuracy and shorter operating times, but is often not employed for trauma cases because of time constraints and complex user interfaces limited to two-dimensional interaction with three-dimensional data. METHODS: In this study, we evaluate the accuracy, precision, and time efficiency o

SPAR-EEG: Selective Pass-Wise Artifact Reduction for Wearable Single-Channel EEG Denoising.

Single-channel electroencephalography (EEG) is attractive for wearable neurotechnology and brain-computer interface (BCI) applications, including assistive interfaces and clinical monitoring, but artifact suppression is difficult when auxiliary channels, artifact labels, or user-selected clean baseline segments are unavailable. We introduce SPAR-EEG, a self-contained framework for Selective Pass-wise Artifact Reduction in single-channel EEG. The framework applies three artifact-specific attenuation passes to each EEG epoch: a variational mode decomposition (VMD)-based pass for high-frequency e
