Anatomy and blood supply of the lower four cranial and cervical nerves: relevance to surgical neck dissection.

This study is a continuation of previous work searching for possible anatomic reasons to explain variable and usually unpredictable postoperative pain and dysfunction after the same nerve losses with similar neck dissection operations. The study consisted of dissections of 19 deceased unpreserved elderly subjects arterially injected with dyed latex. Of the 19 subjects, 14 had brain stem and cervical spinal cord dissections, and all had neck dissections. The findings suggested two possible anatomic reasons for the pain and dysfunction: (i) The intracranial anatomy of the lower four cranial nerv

[Spontaneous dissection of the cervical arteries].

The spontaneous cervical artery dissection (SCAD) is one of the main causes of cerebro-vascular accident in the young adult. It may results from minor cervical trauma. The pathophysiological basis of the neurological complications remain unclear. For that reason, the treatment of SCAD is often empirical. In a retrospective study (1984-1994), we have reviewed 20 cases admitted to Notre-Dame Hospital for a SCAD confirmed by angiography. The initial clinical diagnosis was missed in 65% of the cases despite a suggestive symptomatology (headache 80%, hemiparesis 65% and Horner 45%). We have studied

Two cases of variations in inferior thyroid arterial pattern and their clinical implications.

Vascular variations are the most common ones in humans. Inferior thyroid artery arises from the thyrocervical trunk in 90.5%, from subclavian in 7.5%, and very rarely from the common carotid, aortic arch, brachiocephalic, internal thoracic, pericardiacophrenic, or vertebral. Thyroid ima artery is more common variety found in up to12.2% of the population. Two cadavers dissected in the anatomy department are presented with variations in the blood supply of the thyroid gland. The first case was a 61-year-old man with middle thymothyroid artery arising from the common carotid on the right side and

Minimizing neurovascular complications during image-guided percutaneous cryoablation of a cervical spinal aneurysmal bone cyst using protective doxycycline sclerotherapy: a case report.

BACKGROUND: Aneurysmal bone cysts (ABC) are rare, locally aggressive bone tumors primarily observed in pediatric patients. Surgical curettage is the treatment of choice. Image-guided percutaneous cryoablation (CYOA) is a recently implemented alternative technique in cases not amenable to surgery. CYOA may be limited if the lesion is close to critical neurovascular structures. In this case report, a cervical spinal ABC was successfully treated using CYOA in combination with complementary and protective image-guided percutaneous doxycycline sclerotherapy (DS) to dissect and treat the portion of

Arterial peculiarities of the thoracolumbar spinal cord in rabbit.

The aim of this study was to investigate the arterial blood supply of the thoracolumbar spinal cord in rabbit. The study was carried out on twenty adult New Zealand white rabbits. Ten rabbits were used in the corrosion technique and ten rabbits in the dissection technique. After the killing, the vascular network was perfused with saline. Batson's corrosion casting kit no. 17 © was used as a casting medium. After polymerisation of the medium, in ten rabbits the maceration was carried out in KOH solution, and in ten other rabbits, formaldehyde was injected by the dissection technique into the ve

[Dissecting aneurysm of the anterior cerebral artery: report of a case].

A 43-year-old man suddenly experienced severe headaches and involuntary flexion-extension movements of four limbs, which were followed by hypertonic extension of the limbs lasting for a few hours. Two days later, he experienced generalized tonic seizure without loss of consciousness. After the seizures, he remained hemiparetic on the right side. His past medical history was non-remarkable, and the histories of hypertension, diabetes mellitus, head trauma and significant infectious diseases were all denied. Cerebral angiography performed 22 days after the onset showed a segmental, irregular nar

Microvascular anatomy of foramen caecum medullae oblongatae.

The foramen caecum (FC) is a triangular-shaped fossa situated in the midline on the base of the brain stem, at the pontomedullary junction. Although this area is known to have a very high concentration of brainstem perforating vessels, its microvascular anatomy has not been studied in detail. The purpose of this study was to detail the microvasculature of this territory. Twenty unfixed brains were injected with silicone rubber solution and dissected under a microscope equipped with a camera. The origin, course, outer diameter, and branching pattern of the perforators were examined. The total n

Quantitative Analysis of Intracranial Vertebrobasilar Dissecting Aneurysm with Intramural Hematoma After Endovascular Treatment Using 3-T High-Resolution Magnetic Resonance Imaging.

OBJECTIVE: Quantitative measurements of intracranial vessel walls are reliable in 3-T high-resolution magnetic resonance imaging (HR-MRI). However, few reports have assessed the arterial wall after endovascular treatment (EVT) by 3-T HR-MRI. This study aimed to quantitatively analyze vessel walls in vertebrobasilar artery dissecting aneurysms after EVT. METHODS: From May 2012 to December 2015, a total of 21 patients with 21 intracranial vertebrobasilar dissecting aneurysms (VBDAs) were enrolled in this consecutive study. All the VBDAs were characterized by intramural hematomas (IMHs ≥5 mm) an

Cervical chordoma en bloc resection and staged suprascapular nerve transfer reanimation: illustrative case.

BACKGROUND: Cervical chordomas offer unique challenges to en bloc resection given their proximity to complex anatomy. Achieving tumor-free margins often requires extensive reconstruction that can limit radiotherapy dosing and postoperative visualization due to metal artifact, compounded by the unavailability of cervical carbon fiber implants. Multidisciplinary multistage approaches focused on oncological resections and maintenance of neurological function can improve quality and quantity of life. The authors present such an approach to the resection of a cervical chordoma with nerve transfer f

Unusual morphology of an aberrant right subclavian artery: a case report and systematic review of cadaveric studies.

BACKGROUND: An aberrant right subclavian artery (ARSA) is a rare vascular anomaly defined by the origin of the right subclavian artery from the aorta, distal to the left subclavian artery (LSA). An ARSA was found in an 83-year-old female during cadaveric dissection for anatomy teaching, which had an unusually narrow lumen compared to previously reported cases. MATERIALS AND METHODS: The specific morphology of this ARSA variant was characterized by further dissection and measurements. A systematic review of cadaveric case reports with quantitative evaluation of ARSA dimensions was conducted fo

Carotid Webs: An Unusual Presentation of Fibromuscular Dysplasia.

Carotid webs are abnormal luminal projections at the carotid bulb associated with blood flow stasis, artery dissection, and subsequent complications. Carotid webs are considered to be a rare variant of fibromuscular dysplasia (FMD). Young individuals with symptomatic carotid webs are found to be associated with ischemic stroke. The incidence of the carotid web is low, and it is rarely reported. Only 150 cases of FMD have been reported so far. FMD is a non-inflammatory and non-atherosclerotic arteriopathy. The most common arterial beds involved are renal and extracranial carotids. Presentation
