Exploring the Role of the Head Rotation Sit-Up Test in Identifying Epley-Responsive, Non-Classical Presentations of BPPV.

Background and Objectives: The Dix-Hallpike test (DHT) is the standard diagnostic maneuver for posterior semicircular canal benign paroxysmal positional vertigo (BPPV). However, some patients present with positional symptoms compatible with BPPV yet show no observable nystagmus on the DHT. We introduced the Head Rotation Sit-up Test (HRST) as a symptom-based maneuver and hypothesized that it would identify DHT-negative patients who nonetheless benefit from canalith repositioning. This study aimed to explore the potential role of the HRST. It was introduced as a complementary, symptom-based man

Using the Modified Vestibular/Ocular Motor Screening Tool to Identify Blast Exposure Effects in Military Service Members.

INTRODUCTION: Military service members training with heavy weapons systems are exposed to low-level blast (LLB) overpressure, sometimes resulting in acute symptoms similar to mild traumatic brain injury (mTBI). There is a need for a quick and effective screening tool to identify early deficits and changes associated with blast exposure and minimizing potential ensuing effects. Recently, researchers developed the modified Vestibular/Ocular Motor Screening (mVOMS), designed to assess signs and symptoms of mTBI. modified Vestibular/Ocular Motor Screening consists of four physical movement conditi

Development of a new maneuver to rapidly restore positional nystagmus from benign paroxysmal positional vertigo fatigue in patients with posterior canal-type benign paroxysmal positional vertigo: a single-arm prospective interventional clinical trial.

PURPOSE: Posterior canal-type benign paroxysmal positional vertigo (pc-BPPV) is diagnosed when positional torsional nystagmus is induced by the Dix-Hallpike test (DHT). When the DHT is repeated, positional nystagmus disappears. This phenomenon is known as BPPV fatigue. In cases where positional nystagmus cannot be induced by the DHT because of BPPV fatigue occurring just before a medical examination, or where positional nystagmus is not observed during the first DHT, we cannot make a diagnosis of pc-BPPV because the DHT can no longer induce the positional nystagmus typical of pc-BPPV. Multidimensional Feature Analysis of Meniere's Disease and Vestibular Migraine: Insights from Machine Learning and Vestibular Testing.

OBJECTIVE: Differentiating between Meniere's disease (MD) and vestibular migraine (VM) is challenging due to overlapping symptoms and limited diagnostic tools. Traditional statistical methods often rely on physician judgment and struggle with complex, high-dimensional data. This study applies the random forest (RF) machine learning algorithm to enhance the clinical differentiation between MD and VM. METHODS: We retrospectively analyzed data from 36 VM (26 female) and 100 unilateral MD patients (51 female). The data were anonymized and labeled. Symptomatic and examination parameters were selec

Reliability of the visio-vestibular examination for concussion among providers in a pediatric emergency department.

BACKGROUND: Visio-vestibular examination (VVE) deficits are common following pediatric concussion. Guidelines recommend assessing these deficits on all potentially concussed youth given their diagnostic and prognostic value, however test psychometrics of the VVE in the emergency department (ED) setting are unknown. Our objective was to determine the inter-rater (IRR) and test-retest reliability (TRR) of the VVE in a pediatric ED. METHODS: We enrolled 155 patients (112 IRR; 43 TRR) age 6-18 years with head injury presenting to the ED of a tertiary care children's hospital. Exams were performed

The assessment of the impact of a training process on the habituation of the vestibular-vegetative system, using a special rotational test as a condition of maintaining flight safety.

OBJECTIVES: The authors formulated a hypothesis that, on completion of the training program by cadets, their vestibular habituation would increase, measured by the time of the duration of the test on the unlocked looping, in contrast to the initial values and the control group. MATERIAL AND METHODS: The research involved 35 male cadets, first-year students of the Polish Air Force Academy in Dęblin. The examined persons were on average 20 years of age. They were divided into 2 groups. Group A (N = 25, tested) continued training on Special Aviation Gymnastic Instruments (SAGI). The cadets were

Preliminary report: neural firing patterns specific for Meniere's disease.

OBJECTIVE: To describe the application of a new, objective diagnostic test for Meniere's disease. INTRODUCTION: Electrovestibulography (EVestG) is a complex, newly-developed test paradigm that searches for neural firing patterns that may be diagnostic for particular neural disorders. EVestG system was previously "trained" to distinguish Meniere's disease from other patients on a set of training data. In this paper we illustrate its diagnostic application in a new group of unknown subjects. SETTING: Collaborative Academic Bioengineering Research Centre. STUDY DESIGN: Prospective, blinded hum

Persistent spontaneous nystagmus following a canalith repositioning procedure in horizontal semicircular canal benign paroxysmal positional vertigo.

IMPORTANCE: Nystagmus can occur spontaneously from multiple causes. Direction-changing positional nystagmus on the supine roll test is a characteristic clinical feature in horizontal semicircular canal benign paroxysmal positional vertigo. One of several mechanisms of spontaneous nystagmus is plugging of the otoconia, which has been described as a canalith jam. OBSERVATIONS: We evaluated a 52-year-old woman with a history of geotropic variant of horizontal semicircular canal benign paroxysmal positional vertigo on the right side who had been treated with a modified Lempert maneuver 3 months e

Ocular vestibular evoked myogenic potentials via bone-conducted vibrations applied to various midsagittal cranial sites.

OBJECTIVE: This study applied bone-conducted vibration (BCV) stimulation to various midsagittal sites on the cranium to optimize stimulation conditions for ocular vestibular evoked myogenic potentials (oVEMPs). STUDY DESIGN: Ten healthy subjects (7 men and 3 women, aged 25-32 yr) without previous ear disorders were enrolled in this study. Each subject underwent oVEMP test using BCV applied to 4 cranial sites on the midsagittal plane, namely, the Fpz (above the glabella), Fz (the midline forehead at the hairline), Cz (vertex), and inion. In addition, the linear distances from the vestibule to

Lateralised vestibular hypofunction: canal paresis and handedness.

In this retrospective study it was found that 549 out of 1175 patients seen for vestibular assessment within an 8-year period within a district hospital service showed lateralised canal paresis as determined by caloric testing. It was found that there was a tendency for canal paresis, indicative of vestibular hypofunction, to be on the side of handedness. This bias showed statistical significance, (chi-squared, p < .005), for the patients overall, as well as for the 65 left-handers considered separately, (p < .05). Surprisingly, this form of bias does not appear to have been reported previousl

Active high-frequency vestibulo-ocular reflex and seasickness susceptibility.

OBJECTIVES/HYPOTHESIS: The vestibular autorotation test (VAT) examines responses to active head oscillations at frequencies between 2 and 6 Hz in the horizontal and vertical planes while the subject is fixating a visible target. At these frequencies, the vestibulo-ocular reflex (VOR) is the main source of eye movement for ocular stabilization, although other visual and somatosensory information interacts with the response. Because the neural mismatch theory places emphasis on multimodal sensory interactions as the cause of motion sickness, using the VAT, which measures eye movements resulting

[Sinusoidal harmonic acceleration test in normal Chinese and 400 dizzy patients].

Forty healthy youth, 20 aged Chinese volunteers and 400 dizzy patients were tested with SHAT at frequencies of 0.01-0.64Hz. The peak velocity was 50 degrees/s. The commonly used three parameters: phase, gain, asymmetry were calculated by computer. Alertness tests in the darkness demonstrated a gain increase and a decrease of phase lead with increasing frequency. The bilateral responses were symmetrical in normal volunteers. The asymmetry value was in the limit of 5 degrees. The values of all the parameters made no sexual difference. The phase value with 0.01Hz in the aged volunteers was a litt

Whiplash, postural control, and the inner ear.

Many patients with "whiplash syndrome" experience unrelenting neck stiffness and pain. This abnormal muscular tension is postulated to be causally related to a central disorder of postural control, which has evolved secondary to injury of the inner ear labyrinthine structures. Moving platform posturography was used to demonstrate the presence or absence of a static or dynamic equilibrium disorder in 48 patients who had experienced the oscillation forces induced by a rear-end automobile collision. Other vestibular tests were used to document dysfunction of the semicircular canals and the otolit

[Original survey and analysis method of the vestibulo-ocular transfer using the microcomputer. Contribution to the diagnosis of vestibular pathology].

This report presents a new and well-tolerated technique that quickly tests the labyrinthic system by moving the head, via a helmet mounted on a servo controlled torque motor, with a sinusoidal stimulus small enough (10 to 15 degrees of amplitude) to avoid the triggering of quick phases. A personal PC (Olivetti M24) digitizes the amplified and filtered eye and head movement data for storage and analysis. Compared to the low frequency patterns of stimulation by the sinusoidal acceleration test (SHA), our procedure, because of the reduced mass of inertia, investigates the bandwidth of frequencies

Late onset multiple sclerosis. A clinical study of 16 pathologically proven cases.

In 70 pathologically proven MS cases, we found 16 cases with onset after 45. Sex ratio (3/1) was the same in the late onset cases and early onset control group. There was no positive family history of MS. The diagnosis was more frequently never considered in late onset cases (3/16) than in control cases (3/54). Mean duration of the disease was 8.6 +/- 6 years (8.1 +/- 6 in female patients). This was significantly shorter (P less than 0.05) than in the control group. In addition, in remittent-progressive courses and in progressive courses, it appeared that the duration of the disease was shorte
